WHILE PERFORMING MAINTANCE ON THE CRUSHER AND ATTEMPTING TO REMOVE WEAR PLATE FOR REPAIRS, THE WEAR PLATE FELL APPROX 4 TO 6" STRIKING EE LEFT RING FINGER ABOVE FIRST KNUCKLE RESULTING IN A FRACTURE OF THE FINGER TIP AND 1/4" LONG LACERATION ON SIDE OF FINGER TIP.

End of shift clean-out of material lodged in feeder grizzly bars, as attempting to remove material, slipped on grizzly bars, which resulted in EE falling and striking left side of head. EE received abrasion on left cheek and laceration on left side of forehead.
