Employee was pulling on a rubber hose and felt pain in left shoulder. *EE kept working after the incident on 8/23/17. The doctor finally diagnosed EE on 11/15/17 that EE was going to have to have surgery to repair the injury. The surgery was scheduled and successfully completed on 11/22/17. EE will be off work recovering until 12/7/17.

A hired truck driver was having chest pains, numbness in left arm, sweating, and felt nauseous. An emergency vehicle was called and arrived at the site within 10 minutes and transported the person to Meridian Park Hospital to be checked out.
Employee was working above a wiring trough and fell over. Emergency crews were called and took EE to the hospital to be checked out.

Rock was ejected from the bottom of the face of a blast, traveled 600', over two material stockpiles, and struck a truck parked behind the stockpiles. The rock shattered upon impact with the upper driver's side door frame. Fragments of the rock entered the open window and struck the driver, driver's seat, and back window of the truck.

Employee was replacing a gear drive on conveyor. While removing it, he twisted wrong and hurt his back.

The worker stopped his truck to get his hard hat. He thought he put his truck in park. He went to the back of the pickup to retrieve his hard hat. The truck started to roll backward. When he attempted to stop the pickup it rolled over his ankle and came to rest against his side. He had a broken ankle.
