Worker clocked out in office and while walking to his truck to go home slipped on snow covered ice. Fell on knee's and hand's. Bruising left knee.

While sweeping stairs right knee twisted and became sore. Today the employee reported that doctors determined that low potassium caused the pain.

Employee was walking in parking lot before work on his way to shop. While walking he did not notice a board sticking out from another employee's pickup. He walked into board which cut his forehead.

Employee was greasing rams when his foot slipped and dropped approximately 12"". His left hand was holding front grill of bulldozer. He jerked left hand and his shoulder popped. At the time since there was no medical treatment other than ice/heat and aspirin it was not reportable. As of June 8, it was reported to us that there is now a bicep tear not found at the first doctor visit.

Tripped and fell while cleaning up work area. Fell with arms outstretched and broke right wrist.

MACHINE PEDAL KICKED BACK INTO LEG.

GRINDING DRILL BITS AND A PIECE OF METAL GOT UNDER SAFETY GLASSES AND INTO RIGHT EYE.

SMASHED RIGHT MIDDLE FINGER WHILE CLEANING OUT ROCK CHUTE.

WHILE TIGHTENING BOLTS ON BULLDOZER PADS PULLED BACK MUSCLE.

HIDDEN ICE UNDER FRESH SNOW. HE FELL WALKING TO TRUCK.

EE WAS CHECKING ROCK BIN AND HIT HIS HEAD ON OVERHEAD PIPE.

GRINDING WITH 7"" GRINDER & GOT DUST IN HER LEFT EYE.

WIND BLOWING DUST IN FAB AREA & EE FAILED TO REPORT INJURY FOR 2 DAYS.
