While detrashing the yard, the employee slipped on a mud puddle and fell backwards. Employee felt pain and stiffness in left hamstring.

Nokomis Quarry Company · Struck against stationary object

While stepping on a stone EE twisted his ankle/leg. EE continued to work; ankle did swell up at home EE went to doctor could not determine any fracture. EE then went to another doctor and again no findings. EE then went to a speciaiist and it was determined that he had a hair-line fracture but was able to continue to work. EE did wear a boot for support but was able to work.

While using wrench EE was struck by tool when using pressure to loosen bolt. EE went to family doctor who evaluated EE and returned him to work. MD instructed EE to return in 8 weeks. EE returned to MD for x-ray no positive findings. EE later returned to MD and in late December 2004. EE had 5 days off work for the soreness in the left elbow.
