Struck by falling object

IE & EE were changing out a troughing roller. They were leaning the old roller backwards to remove it, when it slipped out of their grasp, and fell onto IE's right thumb that was on the conveyor frame. IE went to the medical facility where IE received eight stitches and antibiotics.

Trimming a piece of rubber to fit a conveyor transition chute with a utility knife. The knife slipped and cut the top of the left hand.

Slipped on loose soil causing him to lose his footing, broke the fall by extending his right arm to catch himself on a block. Felt pain in right shoulder. Diagnosed as shoulder sprain. Now diagnosed as shoulder tear.

Technician was removing a cutting edge from a wheel loader bucket at Thelen's shop. Shank became dislodged and screw head spun. The screw head sliced through glove and index finger when it spun. 8 stitches required in left index finger.

Using a shop press. the support block shot out, striking the left thumb of injured person, causing damage to thumb and other fingers.

Slipped on ice after lunch break. Work area was covered with snow and was not able to maintain footing. Slipped and fell onto back. After a couple of days felt pain in lower back at belt line.

Slurry pump in the North pit became clogged. Shut off valve did not work. Water in the pump continued to circulate and became hot. Worker opened valve and was hit by hot water on right side of chest, shoulder and neck.

He was kneeling on his left knee replacing a belt on a transfer conveyor. When he stood up he felt pain on the inside of his left knee.

Walking between buildings, slipped on the ice, fell to the ground, dislocated right ankle and broken tibia.

EE was climbing the ladder to enter a backhoe, when he felt a pain in the muscle of his right calf. It felt like the muscle had been hit by a rock.

He was laying on his stomach, lifting screen out of screen deck, and pulled a muscle in the right side of his chest. Incident occurred on 11/15/2007 and was reported on 11/19/2007.

Struck against stationary object

Cutting exhaust hole in side of building with tin snips, cut index finger knuckle on siding of building, requiring 6 stitches in hand.

EE was welding a line for a ready mix hopper. The tack weld he made failed and metal shifted, pinching his left index finger between stell liner and hopper, resulting in a partial amputation and nail evulsion.
