Flash burns (welding)

Although subject was wearing gloves, his wrist was exposed and made contact with the frame of the gravel crusher when he leaned against it. Subject received a thermal burn when weld current arced to his wrist band (watch).

Fall to the walkway or working surface

Employee was changing drag cable in stiff leg crane. He turned to walk toward the crane at which time he tripped over a piece of used drag cable which had not been removed from the work area.

Employee was stepping down off of crane and either missed step or slipped.

Absorption of radiations, caustics, toxic and noxious substances

Operator was fueling a hyd. excavator w/ a fuel equiped w/ automatic shutoff. Automatic shut off nozzle closed, the fuel hose blew at crimp on hose and fuel blew into operator's face.

Fall onto or against objects

Operator noticed that primary screen at plant had stalled. In a hurry to get control he tried to take a short cut under conveyor. Employee's hard hat struck the frame of the conveyor causing him to lose his balance at which time he fell against a beam striking his neck and ear. The ear was lacerated and his neck slightly bruised.

OPERATOR ATTEMPTED TO MOUNT HIS MACHINE. WHEN HE WENT TO GO FROM THE 1ST TO THE 2ND STEP HIS FOOT SLIPPED OFF THE 1ST STEP. HE FELL BACKWARDS HITTING HIS RIGHT HEEL FIRST.

EMPLOYEE WAS WASHING TAIL PULLEY OFF AT WASH PLANT #2. AFTER FINISHING, EMPLOYEE STARTED TO CLIMB UP ONTO THE LOADER HE WAS OPERATING. ON RHW ASECOND STEP UP, THE EMPLOYEES FOOT SLIPPED OFF CA USING THE EMPLOYEE TO FALL BACKWARDS. IN THE PROCESS OF FALLING, THE EMPLOYEE LANDED ON HIS LEFT WRIST AND SPRAINED IT.

THE CONVEYOR LEADING TO WASH PLANT 1 WAS OVER LOADED. THE SKIDSTER THAT IS USUALLY USED TO CLEANOFF THE CONVEYOR WAS DOWN FOR A MECHANICAL REPAIR. TO REDUCE THE LOAD ON THE CONVEYOR, 6 EE MANU ALLY UNLOADED THE CONVEYOR WITH SHOVELS. AS THE EE WAS COMING DOWN, HE LOST HIS FOOTING AND FELL INTO THE BED OF THE PICK UP TRUCK. THE FALL OCCURRED BETWEEN 2-4 FEET.

EMPLOYEE WAS SECURING A SAFETY CHAIN TO CATWALK SHAFT SUPPORTING CATWALK. CATWALK FAILED. EE FELL TO THE GROUND.
