Employee was hooking a lifting chain to unload a piece of steel pipe from a trailer and stepped in between the pipe and loader bucket as the loader was stopping. Leg got pinched between the pipe and top of the loader bucket.

EE was performing maintenance work on an 5/14/09 endloader. When he hit his head on the endloader causing a scalp laceration on the top of his head. He was taken for treatment at 8:12 am and received staples to repair the laceration. Return to duty 5/15.
