Employee was hooking a lifting chain to unload a piece of steel pipe from a trailer and stepped in between the pipe and loader bucket as the loader was stopping. Leg got pinched between the pipe and top of the loader bucket.
A spring loaded door to an enclosure pinched finger, resulting in laceration.
EE was performing maintenance work on an 5/14/09 endloader. When he hit his head on the endloader causing a scalp laceration on the top of his head. He was taken for treatment at 8:12 am and received staples to repair the laceration. Return to duty 5/15.

EE REPORTED NUMBNESS IN BOTH HANDS INITIALLY TO HIS FAMILY DOCTOR, WHO DIAGNOSED AS BILATERAL CARPAL TUNNEL SYNDROME. EE THEN INFORMED EMPLOYER. THIS INJURY IS BEING REPORTED BASED ON THE USE OF NIGHT SPLINTS, ONGOING MEDICATIONS, AND CONTINUED TREATMENTS BY DOCTOR.
