Employee had right hand on mast. Carosel closed pinching thumb on right hand. Employee lost skin on side and tip of thumb requiring skin grafting. Thumb also had fracture.
THE OXYGEN HOSE ON THE TORCH DEVELOPED A LEAK FILLING HIS GLOVE WITH OXYGEN. A SPARK IGNITED THE GLOVE, CAUSING 1ST & 2ND DEGREE BURNS ON HISLEFT HAND
EE HIT BUCKET TOOTH WITH HAMMER. A SMALL PIECE OF THE TOOTH CAME OFF CUTTING HIS LEFT LEG. RESULT 3 STITCHES IN LEFT LEG.
THE EMPLOYEE WAS LIFTING A SCREEN TO ANOTHER EMPLOYEE TO CHANGE SCREENS ON PLANT. HIS FINGER WASCUT, RESULTING IN 5 STITCHES TO THE LEFT INDEX FINGER.
