WHILE CLEANING FROM UNDER THE CONVEYOR BELT HE OBSERVED MUD BUILDING UP UNDER TAIL PULLEY. HE HAD BEEN TOLD SEVERAL TIMES NOT TO CLEAN THE TAIL PULLEY WHILE THE PLANT WAS RUNNING. RATHER THAN SHUTTING PLANT DOWN HE STUCK HIS SHOVEL IN TO THE TAIL PULLEY, CATCHING THE SHOVEL & PULLED HIS HAND BACK BETWEEN THE METAL FRAM & THE SHOVEL.
