On or about 02/03/2015 EE dropped a roller on right foot. EE continued to work for a couple of weeks before reporting such to office. On 02/18/2015 EE reported to office that he hurt his right foot on or about 02/03/2015. EE went to doctor on or about 02/20/2015. He learned that he had a minor fracture on right foot. EE was informed to be off work on or about 02/23/2015.
