EE was working on the booster pump and was tightening a come-a-long when the hook slipped and came back and struck EE below the right eye. EE needed medical attention and missed one-half day of work. he came back to work the next day and he is doing fine.

EE began working on motor while running and observed a line break in the motor. In an attempt to repair the line leak, the fan blade cut the tip of his right pinky finger off. The fan blade guard was removed during this time.

Accident type, without injuries

EE got into end loader, put it in reverse, went back 8 to 10 feet and struck an unoccupied parked 2004 Ford F-150 pickup truck. EE hit the drivers side rear quarter panel and driver side back cab door.

Struck by falling object

When EE was disconnecting roller, roller fell down and pinched EE's right thumb. When EE pulled his thumb from the pinch point area, it created a medium laceration which required 9 stiches.

EE was repairing the ladder on the dredge over solid ground when EE stepped off ladder onto a large dirt clod causing him to twist and sprain his left ankle.

Employee applied diesel fluid into the wood burning stove. He lit the stove with his lighter. The starter fluid vapors ignited, causing flash burns on his left and right hands and forearms. He sustained first and second degree burns which covered about 5% of his body.

EE WAS ATTEMPTING TO ADD WATER TO HOT RADIATOR ON THE DREDGE. EE REMOVED RADIATOR CAP AND HOT FLUID SPILLED ONTO EE'S LEFT FOREARM, CAUSING 2ND DEGREE BURNS. BURNS COVERED ONLY ABOUT 1% OF HIS BODY.

Fall from ladders

Employee was troubleshooting the 6 by 16 shaker. after checking out the equipment, employee was descending the ladder. His foot slipped on the last step, causing him to fall approx one foot. Employee twisted his back when he fell, causing lower back strain.

CLAIMANT HAS COMPLAINED OF PAIN AND DISCOMFORT IN HIS UPPER EXTREMITIES ON BOTH HIS LEFT AND RIGHT SIDES. HE IS DEVELOPING BI-LATERAL CARPAL TUNNEL SYNDROME IN HIS WRISTS, ELBOWS AND SHOULDERS. THIS CONDITION IS AGGRAVATED BY HIS REPETIVE WORK DUTIES, SUCH AS MOTION GRIPPING AND ELBOW FLEXION OF THE JOYSTICK WHILE RUNNING HEAVY EQUIPMENT.

Ee was stepping out of the loader when his foot missed the step, causing him to lose his balance and to fall backwards. He then grabbed the railing, causing him to pull his neck muscle.

EE WAS CLEANING THE DREDGE WHEN HE TRIPPED ON AN ELECTRICAL WIRE, CAUSING HIM TO FALL FORWARD HITTING HIS HEAD ON AN ELECTRICAL BOX. EE SUSTAINED A LACERATION ON THE MIDDLE PART OF HIS HEAD, ALONG THE HAIR LINE.

EE WAS UNTYING THE DREDGE TO PUT IT BACK INTO OPERATION WHEN THE DREDGE MOVED, CAUSING THE 1"" STEEL CABLE TO TIGHTEN, CAUSING HIS RIGHT HAND TO BE PINNED AGAINST THE 1"" CABE & HANDRAIL. HE SUS TAINED 3RD DEGREE BURNS ON HIS RIGHT HAND PINKY FINGER & 2ND DEGREE BURNS ON HIS RIGHT HAND RING FINGER.

EE WAS INSTALLING NEW MOTOR ON THE DREDGE USING A 5' PRYBAR TO ALIGN MOTOR TO THE MOTOR MOUNTS. PRYBAR SLIPPED, CAUSING EE TO LOSE HIS BALANCE, FALLING BACKWARDS 6' DOWN, HITTING LEFT SIDE OF BODY. HE FELL BETWEEN THE PONTOON AND LADDER, BRUISING LEFT SIDE OF HIS BODY AND RECEIVED A BROKEN RIGHT MIDDLE FINGER AND TORN NAIL BED ON RIGHT MIDDLE FINGER.

EE HAD JUST FINISHED CLEANING HEAVY EQUIPMENT WHEN HE WALKED TOWARDS THE PLANT, HE JUMPED A SMALL WATER DISCHARGE DITCH EAST OF THE PLANT, SLIPPED IN THE MUD AND TWISTED HIS RIGHT ANKLE.

WHILE PLACING SAND AND GRAVEL SCREENS INTO POSITION, SCREEN SLIPPED OUT OF EE'S HANDS CUTTING HIS INDEX, MIDDLE, AND RING FINGER ON HIS RIGHT HAND. IT REQUIRED 13 STITCHES. EE WAS NOT WEARING GLOVES AT THE TIME OF INJURY.

WHEN MOVING AND LIFTING A 10 HORSE ELECTRIC MOTOR FROM THE JOHN BOAT ONTO THE DREDGE, ONE EE LOST HIS GRIP CAUSING EE TO HANDLE ALL THE WEIGHT OF THE MOTOR CAUSING HIM TO TWIST AND STRAIN HIS BACKL. SORE AND STIFF BACK.

PORTABLE LADDER WAS SET ON UNSTABLE GROUND CONDITIONS AND NOT TIED DOWN. EMPLOYEE WAS WELDING CROSS BRACE, LADDER TIPPED TO ONE SIDE MAKING EMPLOYEE FALL 3 FEET TO GROUND. EMPLOYEE PUT OUT LEF T HAND TO CUSHION FALL AND BROKE WRIST. BROKEN LEFT WRIST.
