Miner was installing wheel hub on truck. Miner pinched small finger on left hand while attempting to align valve stem. Required 3 stitches on small finger of left hand.

On 4-11-07 employee pulled on bin gate that was slightly frozen straining right shoulder. Treated with physical therapy with no lost time until worker's compensation decision that surgery was needed. Surgery on 08-16-2007.
On 5-4-07 slipped off step while climbing off water truck. Caught self with right arm to avoid fall, straining shoulder. No lost time as tried therapy to rehab shoulder. Did not respond so Worker's Comp authorized shoulder surgery on 7-23-07. Return to work date not determined at this time.

NEXT MORNING WHEN ASKE D HOW HIS ARMWAS HE SHOWED BRUISE AND SOME SWELLTO INSIDE OF RIGHT ELBOW. HE STATED HE HAD STRENGTH NOT MUCH PAIN.

EMPLOYEE WAS OPERATING 631B SCRAPER WHEN HE HIT A BUMP IN THE ROAD. EMPLOYER SAID HE FELT SOMETHING POP IN HIS BACK HE STOPPED HIS MACHINE AND COLLAPSED TO THE GROUND AS HE STEPPED OFF.
