A rock wedged in feeder, employee was using a sledge hammer to break the rock. The hammerhead came off handle mid swing and their right hand ring finger slammed down and hit the rock and steel causing fracture to finger.

Employee stepped out of stockpile truck, slipped on ladder hitting left leg (shin area) on truck bed.

Employee bumped left elbow on door as they were entering stockpile truck. Within 10 minutes they noticed large swelling on elbow.

Employee was attempting to roll rock and adjacent rock dislodged and pinched right 4th finger.
Employee stepped out of stockpile truck. Employee thought they stepped on rock and lost their balance. Employee fell on left arm to break fall. Then felt short of breath. Went to hospital ER to get checked.

Employee was holding slug wrench while tightening crusher bolts. Another employee hit wrench. IE lost control of IE's grip on wrench and smashed left hand between wrench and crusher.

Employee was using pry bar to loosen screens. Pry bar slipped & hit on the head. Cut required 6 staples.
Employee was using left hand to hold plow bolt head while turning nut with impact wrench. Bolt spun in left hand causing deep cut requiring 19 stitches.

Employee reached gloved hand around fan motor shroud when he was replacing hood prop.Fan blade cut tip of rt index finger. He was taken to ER where finger was stitched.

Employee was helping load shot. Carrying buckets of stone to use as stemming. He felt pull to groin area. Thought he was going to be Ok but could not get out of bed next morning.

While running Hydraulic stone splitter, when blade of hydraulic stone splitter was engaged (depressed) the stone sprung back and smashed left pinky finger on left hand and smashed nail, resulting in removal of finger nail with 5 stitches.
While changing conveyor roller, employee #1 dropped roller, striking employee #2 in back of head and neck causing laceration requiring stitches.

Miner went to turn flat rock on conveyor and got Right Hand (glove only) wedged under rock, when miner went to pull hand free, he bruised/ strained wrist bone on that hand. Conditions were dry that day.

EE WAS CLIMBING UP STEPS TO 980C LOADER. HE WAS CARRYING GREASE GUN AND 3 TUBES OF GREASE. ON 2ND STEP UP HE LOST HIS BALANCE AND FELL BACK LANDING ON HIS DERRIERE. THE NEXT DAY HE HAD OCCATIO NAL BACK PAIN AND THE 2ND DAY (SAT) HE WAS UNABLE TO WALK COMFORTABLY.

EMP WAS CRIMPING TERMINAL ON BATTERY CABLE USING A CRIMPING TOOL. A PIECE OF METAL CAME OFF HEAD OF CRIMPING TOOL WHEN IT WAS HIT BY HAMMER. METAL PIECE EMBEDDED INTO EMPS FINGER. EMP WAS USIN G THE TOOL PROPERLY. DR REMOVED PIECE OF METAL. WOUND REQUIRED TWO STITCHES.

ANOTHER EMPLOYE9 DROVE SERVICE TRUCK I NAME WAS RIDING IN AND PARKED IT IN A MUD PUDDLE. HE NOT WANTING TO GET HIS FEET WET, CLIMBED FROM THE BED ONTO THE TRUCK HOOD. HE WALKED ACROSS THE TRUC K HOOD AND WHEN3DISMOUNTING, TRIPPED ON THE ANTENNA, LOST HIS BALANCE AND FELL ON HIS RIGHT ELBOW.
EMPLOYEE WAS CH5NGING A 10:00X22 TRUCK TIRE.THE RIM FELL ON HIS TOE.HE WAS NOT WEARING REQUIRED HARD-TOED SHOES AT THE TIME.THE RIM FRACTURED HIS BIG TOE ON THE LEFT FOOT.
