Employee was walking across the stockpile area when the wind blew dust into EE's eye. Employee went to optic facility who told EE it looked like EE had removed the object from EE's eye prior to visit. They examined EE and gave EE some drops for the eye and said EE would be fine. The drops were prescription thus the reason for this report as the incident was minor.

Employee was changing the skirting on the 53 x conveyor when EE's hand slipped and hit the tail pulley guard causing a 1" cut to EE's right palmar surface. Fabricated tail pulley guard.

Employee was ascending the catwalk to reach the #2 conveyor when their foot slipped. Employee swung their arm out to stabilize themselves and in doing so struck the 53X chute with a flat head on blow. (No cut, laceration or puncture). Employee said they thought they were fine but condition worsened.

Aggravation of injury reported on 5/1/13. Employee twisted knee while installing silt fence aggravating his knee injury. Went to Dr on 7/17/13 who told him to take 2 days off work, then report to work with no restrictions. This report is submitted to add 2 missed days to the accident.

EE was stepping off of a mini excavator and twisted his knee.No real pain or discomfort at time so he didn't call the accident in. Called 2 days later to say he had pain. Sent him to clinic and the diagnosed a strain. He returned for a follow up exam and they requested a MRI. MRI revealed he had a torn meniscus so today he was scheduled for surgery on 5/9/13.

Employee was stooping over to go under the #6 conveyor (en-route to repair a flashing on the # 2 conveyor hopper) when his hard hat fell off. He picked the hard hat up then stood to replace it and struck his head on the conveyor framework.

EE was placing a roll of belting (to be used on a conveyor) onto the fork lift and when he removed his pry bar it pinched his finger. (Pry bar did the pinching). The fork lift was not in motion, cut caused by the pry bar.

Employee was cleaning rock spillage in the screen room with another employee (shoveling rocks back onto the screen surface). The other employee threw a rock toward the screen and struck the employee on top of the head. Employee states hard hat had fallen off as he was coninually reaching down.

EE was digging soil from a footing he was forming. When he rotated to throw the dirt from his shovel, his foot did not pivot causing him to twist his knee. The injury did not bother him for over a week before the pain began to intensify. Became reportable today 11/2/06.

A U-belt had jumped a pulley and EE was pulling on the belt while another employee was prying the belt over to the next pulley when the shaft went over center catching EE's hand between the belt and the pulley.
