Employee was changing hammer tips in the hammer mill which is part of his regular job duties. The shank of the hammer mill dropped down pinching the employee's finger between the disc and the shank. Employee received a cut to the ring finger on the left hand which required stitches.

Employee was putting air in a truck tire. The sidewall on the tire blew out into the employee's face. Employee received scratched eye & concusion to right ear and minor cuts on the face. Employee was performing his normal duties.

EE WAS WORKING ON MACK DUMP TRUCK CHANGING THE FRONT SPRINGS. HE WAS USING AN AIR TOOL AND THE HOSE CAME OFF OF THE TOOL, SLAPPING HIM IN THE EYE. RECEIVED A SCRATCHED EYEBALL.
EE WAS CHECKING A NOISE IN THE MILL. EE OPENED THE DOOR OF THE MILL CHECK NOISE. HE TOLD ANOTHER EE TO ENGAGE THE CLUTCH ON THE MILL NOT REALIZING THE ENGINE WAS STILL RUNNING. EE S ANKLE/FOOT GOT CRUSHED/BRUISED IN THE MILL.
EE WAS CLEANING UP AT THE PLANT SITE. HE WAS MOVING A TAIL PULLEY AROUND ON BACK OF THE SERVICE TRUCK WHEN HE SLICED THE END OF HIS MIDDLE FINGER.
EE WAS COMING DOWN OUT OF MACK HAUL UNIT AND SLIPPED OFF BOTTOM STEP, FELL BACK AND HELD ON TO RAIL. STRAINED HIS BACK MUSCLES.
EE OCCASIONALLY HELPS WITH BULLDOZING & CLAIMS THAT THE BOUNCING & JARRING FROM THE DOZER CAUSED SEVERAL DISCS TO RUPTURE IN HIS BACK.

EMPLOYEE WAS TAKING AN OIL COOLER OFF OF A 988 LOADER. A PIECE OF FOREIGN MATERIAL FELL INTO THE EMPLOYEE'S LEFT EYE.
THE WIND WAS BLOWING VERY HARD THE DAY OF THE ACCIDENT AND BLEW A PIECE OF METAL INTO THE EE'S EYE. EE WAS WEARING SAFETY GLASSES WITH EYE SHIELDS. EYE BECAME IRRITATED.
