Person was checking tailgate and he ran into the truck and cut his chin. This is reportable only because the cut required stitches. He was not paying attention and was running from the tailgate to the front of the truck.

EE WAS PULLING SIDING OFF BUILDING AND A PIECE FELL AND HIT HIM ON THE RIGHT SHOULDER.

EMPLOYEE WAS OUTSIDE AND WIND BLEW SAND UNDER HIS SAFETY GLASSES INTO HIS RIGHT EYE. SUPERFICIAL CORNEAL ABRASION.

PREVIOUSLY I THOUGHT EE WAS USING FACE SHIELD & HE WAS WEARING ONLY SAFETY GLASSES.
