The miner was beginning to start going down the ladder. Missed the ladder rung and fell 11 feet onto the concrete ground below. The miner needed eight stitches to EE's head. Sustained a hairline fracture to EE's left arm and broke EE's right heel.
Employee missed second step on dump truck and strained/tore a ligament on his knee.
Stepped through grate on drive over pit. Hurt knee, started swelling. Sent to the Dr. on 6/12/06, because swelling didn't go down. Dr. gave RX & released him for full duty as tolerated. On 6/23/06, still swollen, went back to the Dr., now he will have surgery.
WAS MOVING AUGER IN TUNNEL. PULLED AND LIFTED & PULLED MUSCLES IN LEFT FOREARM.
PUTTING 4"" CHANNEL ON RACK, CHANNEL ROLLED CAUSING HIM TO TWIST HIS HAND OVER BACKWARDS. WENT TO EMERGENCY ROOM AT 9:00 AM ON SATURDAY FEBRUARY 24, 2001. REPORTED ACCIDENT ON MONDAY, FEBRUARY 26, 2001.
BAGGING PRODUCT, WAS REMOVING CUT OFF PAPER FROM THE BAG MACHINE, GOT FINGER IN THE DEVICE THAT CUTS OFF PAPER.
