Fall from ladders

The miner was beginning to start going down the ladder. Missed the ladder rung and fell 11 feet onto the concrete ground below. The miner needed eight stitches to EE's head. Sustained a hairline fracture to EE's left arm and broke EE's right heel.

Employee missed second step on dump truck and strained/tore a ligament on his knee.

Stepped through grate on drive over pit. Hurt knee, started swelling. Sent to the Dr. on 6/12/06, because swelling didn't go down. Dr. gave RX & released him for full duty as tolerated. On 6/23/06, still swollen, went back to the Dr., now he will have surgery.
