Struck by falling object
Bolting two pieces of 10"" steel pipes together. Using loader to move one to the other. Loader operator failed to watch mine manager's hand signal. Dropped loader bucket on the head of employee trying to bolt the pipe.
Fall from machine
Sand on bottom step of loader. Slipped into rung and twisted knee.
Over-exertion in lifting objects
Adjusting sandscreen replacement that was being held in place by loader. Strained back by moving screen manually.
