Employee climbed on track of track hoe to show mechanic blown hose. Was getting off and slipped and fell about 3 ft. to ground, landing on un-crushed lime rock hurting right leg.

It was a clear and bright day about 60 degrees. EE, the mechanic was called to the Cat D9R Bulldozer. EE had to replace the track roller cap. EE used a 2 ton Hyd. jack to lift the roller up. After replacing the roller, the jack caused the impact gun to crush the thumb and index finger on left hand.

Mechanic was installing a new dump cable on our dragline. While tightening the cable socket, the cable slipped out of the clamp and struck the mechanic in the face. The mechanic suffered a laceration on his upper lip and a fracture to his upper jaw.
