Over-exertion in lifting objects

Employee was changing a roller on the refuse conveyor belt. Employee bent over to pick up the old roller and when employee lifted roller, employee felt pain in employee's back.

Fall from machine

Employee was climbing steps on a Komatsu end loader and EE's footing slipped and EE fell.
