Employee was changing a roller on the refuse conveyor belt. Employee bent over to pick up the old roller and when employee lifted roller, employee felt pain in employee's back.

Employee was climbing steps on a Komatsu end loader and EE's footing slipped and EE fell.

Employee was welding conduit under a beam. Employee lost balance and fell and hurt back and left leg. Employee did not report this till 5/6/16. *Employee has not missed any work. **Employee went to doctor on 5/19/16. They took employee off work.

Employee was helping pull drill steel from mast onto the boom truck. The drill steel swung free striking door of operators cab as employee was exiting cab. The door struck ee on left side of head.

Employee was moving a piece of scrap pipe across the 2nd floor when he felt pain in his right shoulder.

Employee was coming down stairway when he slipped and lost his footing. He grabbed a hold of handrail hurting his left wrist and middle back.

Employee was cleaning bottom floor of dump putting materials in wheelbarrow. When he picked up wheelbarrow he felt pain in his back.

Employee was on top of dryer screen hooking chain for maintenance and right foot went through plastic door.

Employee was removing a shaft for a belt leveling harness from the back of a pickup and felt pain in his back. Employee worked until 7-31-15 then went to his Doctor. They took him off work on 7-31-15.

Employee was changing screen basket. They had removed old screen and getting new screen ready to install they rolled the screen on its side and employee felt pain in groin.

Employee had removed a cutting edge from a loader bucket. As he was picking up cutting edge to get out of the way he felt a pain in his back.

Employee climbing onto cyclone to work on a 10"" elbow. He pulled himself up with his arms and felt pain in his back.

Employee was putting a liner in a wash box when he burnt him self with a plasma cutter.

Employee was changing bottom belt on belt line. He was carrying a belt roller and felt pain in his back.

Employee was changing oil in pumps he picked up full Runyon bucket and felt pain in his lower back.

Employee was working in basement of the plant cleaning when he stepped into a drain ditch that the cover was off of his right foot went into the drain ditch. Employee worked until 9/8/14 then went off work.

Employee was weed eating around the belt line when he saw a snake. He stepped backwards and tripped and twisted his left ankle.

Employee was changing batteries in a 773 rock truck when he felt a sharp pain in his lower back.

Employee was removing a bolt from a 12"" slurry line when he twisted around he felt pain in his back.

Employee was working on dilute pump. While he was positioning wear ring to go in pump the wear ring slipped and caught his right middle finger between ring and shaft. He received 3 stitches on his finger.

Employee stepped into crack at concrete joint and twisted his left ankle while entering plant

Employee was getting in to fuel truck to get his gloves, his left foot slipped on step he caught himself with his left hand on the steering wheel and pulled muscle in his back.

A non injury belt fire occurred at the No 2 clean coal silo. The fire was reported immediate when found.

Employee was loading a piece of grating in the back of a truck, when he cut his middle finger on his right hand, he received 3 stitches.

Employee was changing oil in pumps, he was lifting runyon bucket and twisted around he felt pain in his lower back.

EE was walking in 2nd floor to check on refuse belt when he slipped on ice and fell on his left shoulder. Pulled muscle in shoulder.

EE was helping to load a air compressor into the bed of a pickup truck. When he stepped up onto the truck bed he felt a sharp pain in his right ankle.

Employee was placing metal in metal rack. While lifting metal he felt pain in his lower back.

Employee was using oxygen and acetylene torch doing repairs. The acetylene hose was leaking and ignited in side of his welding glove and burn his left hand.

Employee was on the deck of the locomotive when a belt roller can fell from over head belt structure striking the hand rail on locomotive and glancing off and hit him in the right shoulder.

Employee was on a elevated platform. When he pulled on a handle to open a gate the handle broke. Causing him to fall off platform on his back.

Employee was getting onto dozer when he felt pain in his lower back. Employee worked until 9/25/13 then went off work.

The connecting band on a drain pipe came loose and struck employee in left side of face on his cheek.

Employee was relocating a 6"" pipe. He picked up the end of pipe pushing it while another employee was pulling on the opposite end. He felt pain in his lower back. Employee continued to work until 4-29-13.

Employee was shoveling belt and bruised hand with shovel.

EE was changing oil in a pump when he picked up a 5 gal. bucket of oil and felt pain in his lower back.

Employee was walking past flocr bin when he stepped onto slick spot on floor his feet flipped out from under him he. Fell backswords onto floor hurt his upper back and neck.

EE was getting on a dozer when he twisted his right ankle.

EE was installing a chain flight in wash box while he was lining up flight he got his right thumb caught between the flight and chain sprocket in sprained his thumb.

Employee was cleaning plant floor. When he stepped on a bolt causing him to slip and fall his left leg went under his body and twisted his left knee.

EE was loading belt structure onto a flatbed truck when he felt a sharp pain in his right wrist.

A non-collision roll over accident occurred. When unit CT 135 was topping out a uphill left hand turn when the truck experienced a non collision roll over. The driver received 6 stitches over his right eye.

Employee was cleaning ditch out shoveling and strained his back.

EE was welding on a end loader bucket using a bottle jack to hold a piece of metal in place when he cut his ring finger on hand on the jack handle. 12 stitches.

Employee was cleaning 2nd floor of plant when a piece of 4"" piece of metal fell from 3rd floor where it was being cleaned hit him on his right shoulder. Bruising it.

EE was carrying a bucket of oil when he stepped on a bolt laying on the floor. He lost his footing and slipped and fell, hurting his back and left wrist.

EE was pulling a water hose across the cat walk back into the plant and stained his right shoulder.

EE was cutting tape loose from a water connected hose with a utility knife. When the knife slipped and cut the top of his left hand between his thumb and forefinger. EE received 4 stitches, no lost time.

EE was moving a dryer screen across floor, when his left hand slipped on the sharp edge of the dryer screen, cutting his left hand. He received 4 stitches, no lost time.

Employee was washing floor with water hose after using it he threw hose down and nozzle hit the floor causing hose to come on full stream and hose whip lashed up and nozzle hit him over the left eye.

Employee was going down the stairs from 8th floor to 7th floor when he slipped and caught with his left hand hurting his left shoulder.

Employee was cleaning ditches in the plant basement with a shovel. He lost his balance and fell to his left knee. He felt pain in his lower back and left knee.

Employee was loading a skid steer loader onto his truck when his thumb was caught between the bucket on the loader and the frame on the truck mashing his right thumb. He received 20 stitches.

EE was taking chain fall down as it was being handed down by a co-worker it slipped and hit the EE on the right hand cutting his right middle finger. EE received 1 suture- EE had no lost time.

Employee was leaving work in his personal vehicle after completing his shift when he struck the rear end of a parked Company vehicle.

Employee was prying on a gear box with a pry bar. When the bar slipped, his hand slipped off the bar and his hand hit his nose and fractured it.

EE was working on a forklift and the fork on the forklift fell off and hit him on top of his left foot.

EE was unstopping a pipe in the plant. When he put his arm in the pipe, a rock broke loose and hit his right arm. It sprained his shoulder.

Working in shop and droped a piece of metal on his pinky finger on his left hand. He had to receive 13 stitches. Came back to work the next day no lost time.

Changing Torque in D015/D8R Dozer. He was removing the torque when the drive line fell and struck him on the left knee.

Employee was working on magnetic separator drum, changing endcap, when the chain slipped and drum fell and hit his left hand.

Employee was carrying a 5 gal. sample bucket down stairs to the basement when his boot sole hung on step grating and he fell on steps. It strained or pulled muscle in his right knee.

He was fueling dozer when he slipped and fell of of the dozer tracks as he started to fall he jumped and landed on his right leg it stand on pulled muscles in his right leg.

Entering plant elevator the employee pulled outer doors together and his right hand was between them. It broke tip of his ring finger on his right hand.

Employee was bending conduit with pipe bender. When the binder broke causing him to fall on concrete floor. hurting his right leg. was not reported until 11/2/07. Went off work 11/1/07.

Employee was entering plant and stepped on loose coal causing right knee to twist.

Changing rollers on belt line when roller slipped from hanger striking him on his right leg above the knee.

He was using a coal shovel. Shoveling coal in plant and pulled muscle in back.

Tightening bolt on washbox track with 3/4"" drive rachet, applying torque to the rachet employee struck his elbow on metal. EE went off work on 12/4/06.

Employee was taping pipe on 2nd floor platform. He was standing on a tilted platform causing him to fall 2'. He twisted his left knee.

Taking apart utility knife to change blades when the knife slipped & cut right index finger. Got 10 stitches, no lost time. Came back and finished shift.

He stepped on a motor base in the top of an old loadout to look for a hole in the chute and slipped and fell on his back. He fell one foot to the ground, bruising his back.

Unloading a refrigerator from back of truck when one end of refrigerator fell on employee's right knee.

Putting old rollers into bed of service truck he felt pain in lower back.

Employee was getting out of bobcat, slipped and hit his hand on ground spraining left wrist.

Employee was on a ladder when a water hose he was using knocked him off injuring his left heel, right hip, right elbow and lower back. Fell 4 feet. Company was doing audit on accident report and overlooked this one.

EMPLOYEE WAS CLEANING TUB PLUGGED WITH SOLIDS, HE LOST HIS BALANCE & FELL ONTO CHANNEL ON LEFT RIB CAGE, KNOCKING THE BREATH OUT OF HIM. HE LOST HIS GRIP & CONTINUED TO FALL, HITTING HIS BACK TWICE ON STEEL THEN LANDING ON THE 8TH FLOOR. PAIN IN HIS LEFT SIDE AND LOWER LEFT BACK, HE FELL 6'.

EE WAS STEPING UP ON FREEZE PROOF TANK TO CHECK WATER LEVEL IN NO. 2 THICKENER. TANK WAS FROST COVERED. EE FELL BACKWARD AND LANDED ON HIS BACK. IT CRACKED 2 RIBS ON RIGHT SIDE & 1 ON HIS LEFT SIDE.

Working on tail piece, putting inpack rollers in, he strained his lower back.

Working in shop using hammer. Beating on chain. A piece of metal hit him in his left eye.

After emptying silo found some warm coal stuck in silo. Emptied and washed down with water hoses. No damage to silo or any structure or belts.

EE WAS CLEANING COAL OFF OF A MINER TO BE REPAIRED. WHEN HE TWISTED AROUND HE FELT BURNING AND PAIN IN HIS LOWER BACK.

THE EE WAS BETING ON BOTTOM OF COAL HOPPER TRYING TO DISLODGE COAL WHEN RIGHT ELBOW WAS EXTENDED TOO FAR . THIS CAUSED THE INJURY.

FINGERS CAUGHT BETWEEN TWO PIECES OF METAL.

THE EMPLOYEE WAS IN THE PROCESS OF CHANGING A BASKET IN THE DRYER & WHEN HE CUT BOLT LOOSE BASKET FELL & TOOK HIM WITH IT, CAUSING INJURY TO LEFT SIDE OF NECK.

THE EMPLOYEE WAS CLOSING ELEVATOR DOORS ON 6TH FLOOR WHEN HE CAUGHT LEFT RING FINGER BETWEEN DOORS.

THE EE WAS IN THE PROCESS OF TAKING CHAIN LOOSE WITH LADY SLIPPED, SLIPPED FREED & WEDGED HAND UNDER CHAIN & GUARD. LACERATION TO TOP LEFT KNUCKLE RIGHT FINGER. FOUR SUTURES REQUIRED.

EE WAS FITTING A POLY PIPE AND WHEN HE PULLED ON PIPE HE INJURED HIS LOWER BACK.

THE EE APPEARED TO BE MAKING PHASE CHECKS ON A 480 VOLT CIRCUIT WHEN ONE OF THE METER PROBES CAME IN CONTACT WITH (2) PHASES CAUSING A PHASE TO PHASE ARC. EE RECEIVED FLASH BURNS.
