INJURED WAS CARRYING TWO FIVE GALLON BUCKETS OF OIL TO THE ROOF BOLT MACHINE WHEN HE PULLED SOMETHING IN HIS MIDDLE BACK MUSCLE STRAIN STRAIN TO BACK. THE INJURY WAS NOT REPORTED AT THE TIME O F THE ACCIDENT, LATE REPORT.

INJURED WAS DRIVING SHUTTLE CAR HAD COME TO THE DUMP AND UNLOADED COAL. HE WAS IN THE PROCESS OF GETTING OUT OF THE CAR TO CHANGE SEATS TO GO BACK TO MINER WHEN THE CAR ROLLED BACK CAUSING HIM TO TWIST HIS LEFT KNEE. DIAGNOSIS TWISTED KNEE, TENDER TO PALAPATION AT KNEE AND 1/3 OF FEMUR.
