7 recordedOn 5/12/14 an event occurred at this mine which resulted in the entrapment of two (2) miners. Upon subsequent recovery it was determined that both miners had perished. The injured were fatally struck by coal/rock. An investigation is on-going. The causes are unknown.

EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.
The injured was fatally struck by a hydraulic cylinder. An investigation is on-going. The causes are unknown.
992 G end loader counterweight/fuel tank fell suddenly, pinning employee against the ground.
