Working on crane barge, knocking board off of pallet with sledge hammer, board broke and hit in the mouth

While putting bolts in pipe flange in ladder on dredge. Holding a bolt when another employee hits it with a hammer. The hammer slipped and struck right pointer finger.

Caught in, under or between a moving and a stationary object

The top row of paddles was installed on dual log-washer shafts. While using a pipe wrench to turn shaft for additional paddle installation the unbalanced weight of the shafts caused the shafts to rotate quickly, the quick rotation caused the pipe wrench to turn with the shaft, pinning employee's wrists between the pipe wrench the adjacent metal enclosure.

Repairing the log washer, walking to the job and fell into. Received laceration to head, and scrapes on both legs.

storing the 2"" X 100 ft water hose used to wash the dredge deck and barges. As ee was pulling the charged hose, ee twisted and felt a sudden lower back pain.

Replacing fitting elbow near sweco shaker from the catwalk. the person holding it dropped it the other tried to catch and smashed left pinky finger

Port deck at gravel conveyor hand wench. Moved conveyor flap out, dogged it off handle at hand wench struck the foreman above his left eye.

Dredge mining winter repair - replacing and realign #3 sand screw liner plate - weight 300-400 lbs. was 4' X 8"" X 1/4"" steel. Dropping liner plate into screw box landing on hand of employee.

Was putting wire rope on gravel barge and the wind took off, pushed the barge and the wire rope got tight, knocked him to the deck of the dredge and landed on right shoulder. Compensated till 4-5-14 then MRI (4-14-14) to rule out fracture or tear

EE was welding a brace on logwasher. EE placed 6' (2x8) board on 3rd rung of ladder and guardrail of logwasher, (unsecured)for platform. Scaffold board slid, causing him to lose his balance and fall into the logwasher, approx 44"" from bottom of the board.
