SUBJECT WAS OPERATING A SHUTTLE CAR (#1), AT THE #6 HEADING, LAST OPEN BREAK INTERSECTION. THE OPERATOR OF THE #2 SHUTTLE CAR MADE THE TURN TOWARD THE MINER, STRIKING THE #1 SHUTTLE CAR IN THE DECK AREA, HITTING SUBJECT IN HIS RIGHT HIP AND RIGHT LEG.
SUBJECT WAS SHOVELING BELT AT #3 BELT TWO BREAKSINBY OLD 3-A PILLAR SECTION WHEN A PIECE OF DRAWROCK 3"" THICK & 18"" LONG FELL FROM ROOF STRIKINGHIM IN THE UPPER BACK REGION.
SUBJECT ALONG WITH ANOTHER CERTIFIED ELECTRICIAN WAS CHANGING A TIRE ON A SHUTTLE CAR. THEY HAD THE SHUTTLE CAR BLOCK UP ON CRIBS. WHEN THEY TRIED TO TURN THE BOLTS IN THE TIRE, THE CRIBS TURN ED OVER LANDING ON SUBJECT'S BIG TOE OF LEFT FOOT. THE STEEL CAP IN HIS BOOT CUT INTO THE BIG TOE OF HIS LEFT FOOT.
ON 11/2 EMPLOYEE CAME TO MINE OFFICE & REPORTED THAT HE HAD STRAINED HIS BACK WHILE WORKING ON ASPEED REDUCER ON A SHUTTLE CAR ON 10/12. NO INVESTIGATION WAS MADE AT THAT TIME BECAUSE NO ONE W AS INFORMED OF THE ALLEGED ACCIDENT. (PRIOR TO THIS INCIDENT, HE CAME TO OFFICE ON 10/16 TO REQUEST TIME OFF DUE TO AN AUTO ACCIDENT. HE MADE NO MENTION OF MINE INJURY AT THAT TIME).
SUBJECT WAS PULLING SHUTTLE CAR CABLE TO NEW POWER CENTER LOCATION. HE STEPPED INTO SCOOP DIPPER TO RELEASE PULL ROPE. HE SLIPPED ON METAL OF DIPPER, SPREADING HIS LEGS WIDE, STRAINING GROIN.
EE ALONG WITH 4 CREW MEMBERS WERE TRAVELING TO THE SECTION IN A MANTRIP A LOOSE PIECE OF BELT STRUCUTRE WAS LAYING BESIDE THE TRACK. IT BECAMECAUGHT UNDER THE MANTRIP FLIPPING INTO THE MANTR IP STRIKING EE ON THE UPPER CHEST AREA (COLLAR BONE) ARA
SUBJECT WAS OPERATING A ROOFBOLTER. HE PUT HIS LEFT HAND ON THE POT AND A PIECE OF DRAW ROCK FELL, STRIKING HIM ON THE MIDDLE FINGER OF HIS LEFTHAND.
SUBJECT WAS UNROLLING STEEL ROPE, 160' LONG. HE STEPPED ON UNLEVEL BOTTOM, TWISTING RIGHT KNEE.
SUBJECT WAS BRINGING SUPPLY CAR OUTSIDE, WHEN HE HIT A ROCK IN THE ROADWAY, CAUSING HIM TO BE THROWN AGAINST THE MINE ROOF.
SUBJECT WAS SHOVELING SECTION TAILPIECE. SHOVEL CAUGHT A SPLICE ON THE BELT, FORCING IT AGAINST THE TOP AND CATCHING HIS LITTLE RING, AND MIDDLE FINGERS OF HIS LEFT HAND, BETWEEN THE SHOVEL AN D THE ROOF.
SUBJECT WAS OPERATING A CONTINUOUS MINER. A ROCKON THE MINER WAS DISCHARGED OUT OF THE CONVEYOR,STRIKING HIM ON THE LEFT KNEE AND LOWER AREA OF HIS LEFT LEG.
SUBJECT WAS HELPING CLEAN ROCK OFF THE MINER WHEN A PIECE OF ROCK SLID OFF THE MINER, HITTING HIM ON THE RIGHT FOOT.
SUBJECT WAS DRILLING HOLE FOR A BOLT, WHEN PIECEOF DRAW ROCK FELL, STRIKING HIM ON THE RIGHT HAND. HE DID NOT SEEK MEDICAL ATTENTION FOR IN-JURY & HASN'T AS TO DATE OF THIS REPORT; HE CAME BY OFFICE & SAID HE FELT DR. WASN'T NECESSARY.
SUBJECT WAS SHOVELING (CLEANING) BELTLINE WHEN HE BECAME OVER-EXERTED.
SUBJECT WAS ASSISTING EE ON INSTALLING ANCHOR BASES AND CHAIN HANGERS. HE WAS HOLDING HIS END OF THE BASE. EE PUSHED THE BOLT UP WITH THE BOLTER, CATCHING SUBJECT'S FINGER OVER THE BASE, BETWE EN THE BASE AND THE TOP.
SUBJECT WAS SHOVELING COAL OFF A SHUTTLE CAR, WHEM HE JABBED HIS HAND AGAINST A CAP WEDGE STRIKING HIS RIGHT HAND.
SUBJECT WAS TAKING A FLATCAR (SUPPLY CAR) OF BLOCKS TO 003 SECTION. WHEN GOING ALONG #4 BELT, IN BY HALFWAY POWER CENTER, THE MOTOR STARTED SLIDING. SUBJECT ATTEMPTED TO PUMP THE BRAKE, WHILE DROPPING SAND. THEN HE ATTEMPTED TO REGAIN CONTROL BY REVERSING THE MOTOR. HE COULD NOT REGAIN CONTROL SO HE JUMPED OFF THE MOTOR, RUNNING ALONGSIDE OF THE TRACK, STRIKING A TIMBER.
