Witness: EE was on the 1st or 2nd step of a small ladder w/backward fall protection when he "blacked out".*He slipped from ladder landing on feet & fell on his back*. He never lost consciousness. We are waiting for medical records to determine the cause.*On 12/18/07 he had surgery & he died on 12/19/07*

THE BACK CARGO DOORS OF THE VAN WERE OPEN BUT THE FRONT DOORS WERE CLOSED THERE WAS A HOLD 1/2 WIDE BY 1 1/2

A CRANE LIFTED THE COUNTERWEIGHT. VICTIM REACHED FOR A TIE AND HAD THE UPPER PART OF HIS BODY UNDER THE COUNTERWEIGHT. PART OF THE CRANE RIGGING BROKE. THE COUNTERWEIGHT FELL ON THE VICTIM.
