Gatliff Tipple: 701 MSHA citations Coal

A contractor from Kone Cranes was hired to inspect the crane/hoist on the 3rd floor of the prep plant. The contractor stated that they were climbing a ladder to access the crane when the contractor fell from the 4th rung of the ladder. Contractor was complaining of hip, back and head hurting so was accompanied to the ER to get checked out.

An employee was cutting out some old pipe in prep plant using a hand-held grinder with a cutting blade. When the piece of pipe EE was cutting started to fall, EE reached to catch it and EE's left index finger contacted the spinning cutting wheel and lacerated EE's finger.

Employee was loosening a hydraulic line on the skid steer. When the line broke loose employee hit their hands on an oil barrel that had been cut down for draining oil. The sharp edge of the barrel cut employee's hands between the thumb and the index finger on both hands.

The employee was checking the oil in the Komatsu 500 loader as part of the pre-shift inspection and the access door fell on EE's right hand smashing EE's right middle finger tip resulting in a fractured bone.

The employee was attempting to unclog a clogged pipe using an air hose. When the pressure was turned on to the air hose the line whipped around wildly and toggle fitting cut arm.

The truck driver was delivering stoker coal to the load-out to be loaded on rail cars. EE's truck rolled forward into the truck in front of EE. When the truck driver of the truck in front of EE went to investigate, EE found EE leaning over and unconscious. They called for an ambulance and worked with EE until EMS arrived. EE was pronounced dead after arriving at the hospital.

Equipment operator hurt neck and back when operator hit a pothole while operating a front end loader.

Employee was descending steps of truck dump when the 2nd to the bottom step broke causing them to hurt their ankle.

The injured employee was assisting in moving a mechanism on the 5th floor of the prep plant when leg was caught between the mechanism and the handrail and broke fibula of left leg.

Employee was putting fuel in rock truck when he removed the fuel nozzle from tank on the truck, the nozzle and fuel hose slipped out of his hand striking employee in the nose, breaking his nose.

Injured was holding wrench and was removing a fitting on a hose when wrench slipped striking his index finger on Lt hand causing cut to finger.

Injured was disconnecting a water hose that has Cm644 chemical, which is a coal soap. When injured disconnect the hose the soap splashed into his face getting into his eyes. Employee finished shift and went to doctor that night said eyes was bothering him, and wanted to get eyes checked.

Employee was operating 740B Caterpillar Truck. The truck engine started to regening, so the operator went to lunch in the building, when he came back from lunch the truck engine was on fire. the fire burnt engine and cab on truck. It took about 25 minutes to put out the fire on the truck. there was no injury to any person.

Employee was cleaning the raw coal belt and was going to get a water hose when he slipped and fell hurting his ribs and elbow.

Employee was preparing to change vorsive basket, while removing lid on basket employee lost his footing which caused him to drop the lid mashing his fingers on right hand.

INJURED WAS ATTEMPTING TO MANEUVER THE ATV UP A STEEP BACKFILL SLOPE WHEN HE ENCOUNTERED A ROUGH ROCKY AREA CAUSING THE ATV TO OVERTURN. INJURED RECEIVED STITCHES IN HEAD.

WHILE REPLACING TOP ROLLER AT TAILPIECE, INJURED WAS PRYING ROLLER UNDER TOP OF BELT AND CAUGHT THUMB BETWEEN ROLLER AND BELT STRUCTURE.

WHILE REMOVING FARM STYLE GRASS SEEDER FROM BUMPER OF PICKUP TRUCK ONE OF THE MOUNTING BRACKETS FELL PINNING AND MASHING FINGER BETWEEN BRACKET AND SEEDER.

EMPLOYEE WAS AT THE TOP OF A HIGHWALL WHERE RECLAMATION WORK WAS BEING DONE AT THE FORTNEY BRANCH JOB AND WAS ATTEMPTING TO DESCEND A GRASSY SLOPE PART OF THE HIGHWALL WHEN HE SLIPPED AND FELL , INJURING HIS LEFT HAND.

EMPLOYEE WAS ATTEMPTING TO CUT A RAG FROM OFF THE BROOM WITH A KNIFE AND THE KNIFE SLIPPED, AND HE ACCIDENTLY STABBED HIS LEFT HAND.

EMPLOYEE WAS ASSISTING ANOTHER REPAIRMAN IN REEPLACING BUSHINGS ON A DOZER ROLLER FRAME. THE OTHER REPAIRMAN HIT HTE BUSHING WITH A SLEDGE HAMMER AND THE HAMMER GLANCED, STRIKING THE EMPLOYEE 'S RIGHT HAND, CAUSING A LACERATION.

HE WAS ON ALADDER HANGING PIECE OF SIDING WHEN THE LADDER SLIPPED AND HE FELL TO THE GROUND LOADGING A SPLINTER IN HIS LEFT RING FINGER. SPLINTER HAD TO BE REMOVED AND THE WOUBD SUTTURED.
