Employee was stepping over a large clevis & felt & heard a popping sound in left leg. Employee has a strain to employee's left calf muscle.

Employee was fitting a door on deck of Fairchild battery scoop deck & door fell off deck landing on left foot behind steel toe.

EE stumbled & fell when ee got off mantrip. EE tried to catch self, felt sharp pain in left wrist, wrist is swollen. EE went to doctor & was given FULL DUTY RETURN TO WORK RELEASE. EE followed up with doctor a week later & was taken off work on 08/08/2017 until evaluated by specialist. EE was diagnosed with a ligament tear in left wrist.

Squaring in the first cut with roof bolter, was getting strap out of tray, strap slipped cutting left forearm. Employee was transported to Whitesburg ARH and received sutures & dermabond to close wound. Employee was released without restrictions to return to next shift.

Employee was in process of installing roof support in #4 entry on 004 section. EE just had got drill steel started in the top. EE engaged the up pressure and rock fell striking left elbow. Causing a cut which at ER was glued but ee followed up with physician who took out the glue and put in 3 sutures and having ee off work.

An unintended fall of the roof on the 003 (retreat mining section) in the #4 entry, inby cross-cut number 20. The fall entrapped the #1 MRS unit inby the fall. 103(k) order was issued & later terminated with the mine returning to production. MRS unit was recovered, NO injuries. Section returned to normal production 8:40 AM on 11/04/16.

Employee was carrying timbers in low height 46"-52" over a distance of 200' over irregular bottom. Setting timbers and building cribs in a bleeder. Employee strained back. Employee did not start loss of time until January 3, 2017, and is off waiting for back surgery.

Employee was coming back from getting bolting supplies and hit head on the mine roof while on the lo trac.

Employee was loading pans for pinner into scoop bucket. Bent over & picked them up when a sharp pain in lower back.

Employee was bolting in the #1 entry, EE was pulling on a roof strap to line it up and felt pain/pop in shoulder. Could not raise arm up without pain afterward.

Employee was dusting travel way & the hose busted on rock duster. EE went to get repairman. EE was waiting on car to dump on feeder & noticed a piece of rock hanging loose on top. EE didn't feel comfortable leaving it, so used a slate bar to pull the piece that was hanging. EE did not anticipate what was coming with it. An 80"x 44"x 10" piece of rock fell on him.

Employee was loading timbers on lotrac, picked up timber and strained left shoulder.

Employee was tramming scoop from #3 to #2. EE went through curtain then held curtain up with right arm. A shuttle car was parked in #2 inby locc with boom sticking out into intersection. EE pinched arm between canopy of scoop and boom of shuttle car.

Employee was putting a fork onto a Lo Trac and was going to slide the fork closer to the adjacent fork which was already in place. The fork was not completely in place on the slide bracket and when EE attempted to move it, the fork fell. The fork landed on the tip of EE's finger and mashed it against the mine floor.

Employees were moving a feeder to the 004 section a piece of draw rock fell landing on ee left hand.

Employee had been to the 004 section looking at conditions outby the section which he had some issues with. He walked up to the section via the power entry (#5), passed the power center and turned left to go toward the feeder. As he did so, his left ear struck a car anchor installed in the mine roof. He had a laceration to his left ear lobe which required stitches.

The area where the roof fall occupied was in a transition zone where the shale roof was turning to sandstone. Supported but it fell above the supplemental support. This fall was on a belt line that had been developed 3 years prior to the fall. The only damage was to the beltline for approx. 30 feet.

Area was recognized as bad roof in June. Advancement of the panel was ceased, at that time, and the area was dangered off.

EE has filed a black lung claim with the Kentucky labor Cabinet, Department of Workers Claims. *** There is no accident associated with this claim***

Employee has filed a claim for Black Lung benefits with the U.S. Department of Labor, Office of Worker's Compensation. The Amended Notice of Claim is dated March 27, 2013. *****THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.

Received a hearing loss diagnosis on 12/12/13. Employee was termed on 3/5/13 and is no longer employed with the company. Accident date of 3/5/13 is to accommodate online reporting per MSHA (Denver) instructions.

The EE was helping to change a shuttle car tire. As he and co-worker were prying tire up, the tire jumped off the wheel unit falling onto injured EE's left knee and ankle.

While driving scoop, EE hit a hole or a rock on roadway causing him to be bounced up from the seat then back down straining his lower back.

While roof bolting, a piece of draw rock fell and struck employee on the arm at the elbow above his Kevlar sleeve. This resulted in a laceration requiring 16 stitches.

A fellow employee had a rock partially picked up with a slate bar, injured worker bent over to roll rock over with his hands. He then felt a sudden sharp pain in his lower back.

While cleaning off the roof bolter, EE pulled a strap that was hung up. When it came loose , EE felt pain in his left arm. EE cont. to work until 12/12/12, at which time he claimed that the pain was so bad that he could not continue until his injury was repaired. He is awaiting an appointment with dr.

EE said he thought something smelled hot on truck. He said he got out to open the hood, when he pulled on the hood to open he said he felt a sharp pain in his lower back. He said he did not want an ambulance so we took him to the E.R. He was x-rayed but it did not show anything so he was referred to his regular Dr.

Employee had just taken receptacle loose from scoop and was getting down from the top of the scoop to put on charge. When he stepped off the scoop he twisted his left ankle.

Employee was helping arrange soap tanks so that they could be unloaded with a forklift. As he was moving the tote, he felt a sharp pain in his lower back. The employee continued to work until 6/19/12. At this time, the employee was taken off work and remains off work until test can be conducted.

Employee was trying to break a bolt loose in the deck of a diesel scoop. When bolt broke loose, his wrist struck the frame in deck of scoop causing a laceration.

During retreat mining on the 001 MMU, and mining the last lift in #6 pillar, draw rock fell on the head of the CM. This pushed the head of the miner into the gob, to the extent that the miner would not tram on its own. No injuries resulted.

EE was moving loader in the reverse direction and hit the embankment causing an injury to his neck. The EE also lifted or carried (2) 5-gallon containers of oil, straining his neck.

A call was made to the MSHA Hot-Line stating that a roof fall had occurred on the continuous miner on the MMU No. 001-0 Section. This call was made in error. The report is to comply with the part 50 reporting of accidents.

The EE was using a piece of diesel equipment (low trac) The EE had stopped the Eq. and while getting out of Eq. he slipped and fell hitting his left hand against the machine, which resulted on a break to the 3rd finger on his left hand. On 12/6/11 the Dr. determined that minor surgery would be required to repair the break to the EE's finger.

The miner was loading top structure in a trailer in preparation for the next belt set-up. When placing a piece of top structure on the trailer, he felt a sharp pain in his right shoulder.

Employee was setting a timber. He got down on his knees to set the timber. While on his knees, he twisted around and said he felt his knee pop.

EE was hanging a curtain when he lost his footing and fell on his left shoulder. He fell against the rib and was complaining of shoulder pain. EE went to the doctor and he continued to work at his regular job until 10/5/11. He had surgery on 10/5/11.

Employee was loading supplies for the roof bolter. As he was turning around, he lost his balance and fell twisting his back as he fell. He continued to work until 06-06-11 when he went to Pineville Hospital. He then went to see another doctor on 6-7-11. This doctor had him off work until 6-9-2011.

Employee was reaching upwards installing a rib bracket when he felt a sharp pain in his lower back. Employee finished his regular shift. He went to the doctor the next day.

Employee was closing airlock door on roadway behind mantrip with a trailer attached. Mantrip had went through door and the employee started to close the door behind it. The door caught the trailer catching the employees hand between the two. This resulted in stitched to the employees hand.

Employee was walking across the section at a hard pace, when he felt a sharp pain in his lower back.

Employee was moving a channel strap, when it caught his glove causing it to cut his left wrist. (4 stitches)

A small fire was discovered inside the surface maintenance office by security personnel at approx 10:30am. The mine was idle & no employees were present at the time of the fire. The fire was contained to this building only. The local Fire Dept was notified & upon arrival the fire was extinguished in approx 10 min. There were no injuries or fatalities associated with this fire.

Employee was pulling an old piece of miner cable by hand. As the employee pulled the miner cable, he was walking backwards when he backed into a low hanging metal strap which resulted in a laceration to the back of the employee's neck. Laceration required (5) stitches.

EE was pulling cable slack by hand when he felt a sharp pain in his lower back. EE continued to work until 3-15-10 (last shift).
