A Roof Fall was found along the 270 mains in #7 entry. the fall measured 18ft wide 40 feet long and approx. 12ft high. spad #29+40
Had hand resting on canopy of bolter rock fell out struck him on the pinky finger and broke it. Employee improperly placed hand on canopy.
Removing pinner steel from the roof a rock broke loose and fell and hit him on the cheek cutting it put 8 stitches. Employee failed to scale loose rock.
Employee was retrieving a rope and chain from under a rock that was on the ground. When the rock fell forward and caught the employee and pushed him to the ground. The rock ended up on his lower legs and back.
Employee stated he was doing repair work to the feeder on #4. He was cleaning the feeder flights and bolts holes out as he was laying them on the side frame of the feeder. At this point, one feeder flight slid off the feeder and struck the employee in the left lower.
A roof fall occurred in the primary escapeway along the 270 mains in the #5 entry. The fall measured 20ft long, 18ft wide and 12ft high. The fall is located 1870ft from the nearest section.
Employee was struck by rock while cleaning out his dust box in the #5 entry on #1 section. The employee received a linear non-displaced fracture to the distal fibula. The employee was first given a full release but on 4/30/14, information was received on the fracture.
Employee stated a rock fell a short distance while he was on top of the continuous miner repairing a water hose going thru the belly of the machine. The employee continued to work but on 5/5/14, doctors placed him on restricted duty. As of 5/19/14, doctors have him off work.
Employee stated he strained his left shoulder while using a jumper cable for changing out ram car batteries. He was taken off work on 4/18/14.
Employee stated he was bolting in the #5-left x-cut when a rock fell and struck him on the upper right arm causing a laceration that required 11 sutures. The rock measured 19"" by 11"" by 1"" to 1"" in thickness
Employee stated he was walking to get a golf cart tire off the tire rack. When he turned to walk away, he tripped over a piece of angle iron and fell to the ground. Employee continued to work until 4/2/2014 when doctor reported he had a broken left #6 rib.
A Roof Fall occurred on the #2 unit face area inby spad 49+60 along the 2nd south submain off the Sturgis Mains in the #3 entry. The fall measured approx. 20' wide, 40' long and 6' plus high.
Employee was examining belt lines and was exiting her golf cart from nearly a prone position and twisted her knee. She continued to work until 5/2/14 when she had a medical procedure done on knee.
Employee stated he was pulling up miner cable with left arm and felt pain as he was pulling on cable, in his lower back.

Employee stated he was struck by a rock operating the remote control miner. The rock struck his left knee and measured apprx. 2' by 1' by 2"" thick. The rock came off the miner TA. 1. After further consideration and because the Insurer found the injury compensable, this is being reported as a lost time injury.

A roof fall occurred in the #5 entry spad # +3606 of the Cypress Creek main. The fall was just in the intersection and appro. 6 plus feet high. the fall is appro. 2500ft from the nearest section.
C Panel, #7 entry, spad 11+80 off the Blueberry Hills submain out by section appr. 1000ft. A rock fall was found measuring appr. 19' by 20' by 6' plus high.
A rock measuring 4ft by 3ft by 3in thick fell from between pins and struck employee in the right leg above ankle.
Employee was in #6 entry behind his roof bolter when the miner pulled through the wing curtain and struck the employee in the right buttock.
EE was repairing oil leaks on a truck that is equipped with many accumulators. An attempt was made to release all of the system's stored pressure. There were 3 hoses that already been repaired and no pressure was present. While removing the 4th hose, stored pressure was present, causing hose to fly into EE's face and strike him in lower eye, had safety glasses on.

A roof fall occurred in the #6 entry spad # +3796 of the 1st NE main. This fall was located in a cross-cut adjacent to the travel road. This fall was located approx 15,000 feet from any working section. Size 19' by 35' by 6 plus feet high.
There is no specific incident related to this illness. The EE believes after conferring with his Dr. because he is often on his knees, it could be that due to grinding dirt into his skin and infection developed. Although he first missed work 10/12/10 he did not report to the mine it was work related until 11/12/10.
A rock fall was found at 11:10AM on the James sub maines, #7 entry, spad # 37+80. The fall is approx. 6' plus high, 90' long by 19' wide, plus in connecting crosscut four an additional 40' by 19' wide. The location is approx 3,300 feet from any working section in the return old works.
Employee stepped in hole carrying oil cans back to his golf cart. He strained his right knee.
Employee was changing a shear pin in conveyor of feeder. While removing the cover in haste because the cover was stuck, he struck his finger and lacerated his left middle finger. **As of 9/13/10, the Reportable became lost time.

Employee was carrying 2 full 5 gallon oil cans and experienced sharp pain in his back.
Six employees traveling 90 day primary escapeway. The covered mantrip that they were traveling on rubbed a roof bolt and the back cover was pulled loose. The SCSR storage box mounted under the cover struck an employee in the face and right eye.

Mantrip at 2E turn on travel road had stopped to communicate with person at scoop charger. The mantrip was put in park and operator left mantrip. The mantrip slipped out of gear and rolled forward and struck employee in the left hip.

The employee was installing roof support to the roof when a rock (18"" by 10"" by 0.0"" to 0.5) fell and struck him in the upper left arm. #1 unti, #7 room, just inby 12+60 spad number. First aid was given to the employee and he was taken outside for further treatment. He was given 5 stitches.

EE was lifting a cross-over duct work onto M15 miner. The EE felt pain in his lower back. He finished the shift and worked several days after. On 2/16/09 he went to the doctor and doctor would not release him to return to work. On 2/17/09 this became a lost time ""Reportable"" incident. The EE had tools and equipment that was there to help lift, but chose not to use them.

@2 unit, #5 entry, miner operator reported he saw a small orange glow and heard a noise ar the face. Investagation by company, State and MSHA and it was determined that it was a sulfar ball. No methane ot dust issues in the area or with the mine
Employee was watching roof bolter advance up to next row of bolts to install with hand on top side of canopy. A rock fell 3' x 2' x 1"" - 2"" thick and struck him on right hand cutting his middle finger and severing his index finger.

An unintentional roof fall occurred in the 4th SE submain #5 entry spad # 30+06 and this is an idle area. Size of fall was 25' long X 20' wide X 20' high. Type of roof was shale.
Employee stated when he lifted a battery out of the #6 golf cart located in the pit area he felt a burning sensation across his lower back and into his legs.
EE stated he had just completed installation of his roof bolt and moved the boom over to install the third roof bolt. Before starting installation of the third roof bolt he stepped back towards the roof bolter to change the bit on the drill steel. While he was in the process of changing the drill bit two rocks fell from the mine roof and struck his right leg between ankle and knee.
A roof fall occurred in the 4th southwest submain #7 entry spad #29+16 at the 3-way intersection. Size of the roof fall is 18'W x 18'L x 6'T. Type of roof support is 5' fully grouted tension rebar bolts.
EE was performing his preshift exam He found some loose rock located in the crosscut between No.3 & No.4 entry that needed to be taken down. After scaling down the rock, he turned to walk away when a piece of rock approx 3'x2.5'x3.5"" fell out striking him on the back of his right leg. He finished his shift then report it. The rock caused a small fracture in his leg.
EE stated that while bolting in the face of #5, sectional pinner steel lodged into the roof. As he turned to speak to the scoop man, he placed right hand on the boom of the bolter. The steel dislodged from the roof & fell on his right hand between the 1st & 2nd fingers near the knuckles. The steel caused a laceration that required 2 stitches to close. He was not wearing right PPE.

Employee stated that when he stepped out of a piece of equipment (mini-trac), his left foot came down on a piece of rock causing him to twist his left knee. The employee did not start missing work until 12/5/06. This was after a 2nd doctor instructed him to take an MRI which is scheduled for 12/12/06.
Employee stated that while working in the battery shack, he turned to pick up a tool off the ground and twisted his left knee in the process. Because of swelling in his knee, he had a MRI done and later had arthoscopic surgery to repair a cartilage. He continued to work until 1/11/07.
EE was in the process of installing a roof bolt on the opposite side of the operator on #3 unit. As he began to drill the starter hole, the steel became lodged in the hole. He then reached up with his left hand and right hand to pull the steel down. When he did this, the steel came loose and caused the bottom of his right hand to hit the pot of bolter. Pinky finger, 9 stitches.
EE was working 3rd shift assisting in preparation for the continuous miner crew to run coal for the following shift. While in the process of cutting off some excess back-up ventilation curtain that had been overlapped, his utility knife slipped out of the curtain and cut the index finger on left hand. Note: He was wearing gloves and the injury required 11 stitches.
An unintentional roof fall occurred at spad #16+36 on the 4th submain east, #mmu-002-0, #2 unit, #1 entry in return. The intersection had been previously cribbed out due to a slip condition, the adjacent crosscut fell riding over the cribbed intersection. The area was approx. 20'in length, 60'wide & 10-15'in height. The fall will be cleaned up at a later date.
Employee was operating a continuous miner when a ram car trailer's pull bracket hooked into the water hose of the CM, and pulled the injured out into the intersection.
Brow of highwall rolled in bending the fan housing air sweep that extends from the surface into the entry of the main return.
Miner Operator was tramming a continuous miner to the No.7 entry. As he was maneuvering the miner, he knelt down in an area that had been cleaned by a scoop. As he was kneeling, his foot dropped down in a hole and he twisted his right knee. He continued to work until 10/6/06 when he was placed on restricted duty by his regular doctor.
Injured stated that while he was driving a spad in the rib a piece fell off that measured approx 65"" x68"" x 6""-12"" striking injured on his left foot. Injured was wearing metatarsal boots. Injured was taken to the ER. Injured has 2 bones broken in his left foot. Inj ret'd to work next day on restricted duty.
An unintentional roof fall occurred at spad #16+36 on the 4th N.E. submain #2 unit #5 #6 #7 engtry. The fall measured 21'x20'x10. It will be cleaned up.
Employee was kneeling down on the rib by the miner waiting on a shuttle car when a piece of the rib/rock rolled off and struck injured on the right forearm causing a laceration. Injured went to the hospital and received 7 stitches. Returned to work the next day.
Injured was riding a golf cart down the hill on D road when the brakes overheated. Injured put both feet down to try stopping the ride when one foot twisted causing the injury.
Injured was servicing shuttle car in front of the feeder when a piece of rock fell from between bolts, hitting the injured on the back of his head causing a 1/2""laceration with some swelling. Injured was taken to the hospital for observation. Injured received 1 staple.
EE was using a 5' pry bar trying to remove a big rock off miner on #2 unit in #7 entry. Another rock underneath the rock he was removing that he was not aware of. As he pryed rock closer to edge of miner rock underneath fell off miner & struck him on top of foot. He had on metarasal boots. x-rays two bones on top of foot were fractured. rock size 3 1/2'x2'x8'.
Injured was roof bolting on #3 unit in #4 entry when a piece of rock fell from between roof bolts, striking injured on the right side of the head just in front of the ear, causing a 2"" laceration. Injured was taken to the nurses statioin for observation. He was then taken to the hospital and received 5 stitches.
Injured was helping put on belt lay on #3 unit. He was crossing the belt structure to get the pulley to hang and pull the belt on when a piece of rock approx. 18"" x 12"" x 1-1/2"" to very thin edge fell and struck injured on the left forearm causing an 8 cm L shaped laceration. Injured was taken to the doctor and received 7 stitches.
EE was prying loose rock down in #6 entry using a 5' prybar. When the loose rock that was pulled down hit the ground, a piece of rock flew up & hit injured on the chin causing a 1"" cut. EE was taken to the hospital & received 5 stitches. He returned to work the next scheduled shift. He was wearing gloves, glasses & under another roof bolt.
Injured stated that he was driving connecting links on #16 shuttle car conveyor chain on #1 unit when a piece of the sledge hammer broke off and struck injured in the neck. The hammer head had some tape on it. There was slight swelling & tender to touch. Went ot Dr office for x-ray. EE had piece of metal in his neck went to RMC for removal.
EE stated that he finished bolting #3 entry on #1 unit, as he went to hang a red ribbon at last bolt a piece of rock fell from between bolts and struck EE in the back of the head, knocking EE into the canopy of the bolter EE received 11 staples and a broken nose. ATRS was still set against the roof. Rock measured 2.8'x2.7'x1=3'
EE stated that he was installing cable bolts when the steel fell out of roof and hit injureds index finger on his left hand between pinner pot and steel causing a fracture to the first joint of finger.
EMPLOYEE STATED THT HE WAS PICKING UP THE MINER CABLE TO THROW IT OVER OUT OF THE WAY WHEN HE FELT A PULL IN HIS LOWER BACK. HE FINISHED THE SHIFT. AFTER HE WENT HOME, HIS BACK CONTINUED TO HURT. HE WENT TO THE DOCTOR. DOCTOR TOOK HIM OFF WORK UNTIL 2/24/06. EMPLOYEE RETURNED TO WORK ON 2/27/06 WITH RESTRICTIONS.

AN UNINTENTIONAL ROOF FALL OCCURRED AT SPAD# 8+60 ON THE 3RD NORTHWEST PANEL OFF NORTHWEST SUBMAIN, #2 UNIT (MMU-002-0) #7 ENTRY. THE APPROACHES TO THE FALL HAVE BEEN TIMBERED OFF & THE AREA DANGERED OUT. IT MEASURED 40' X 19.5' X 15'. IT WILL NOT BE CLEANED UP. UNIT WAS MOVED OUTBY FALL AREA.
INJURED WAS WORKING ON THE P5 BOLTER ON #3 UNIT WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK INJURED KNOCKING HIM INTO THE BOLTER CAUSING AN ABRASION/SCRATCH AND FRACTURE ON HIS LEFT CHEEK.
Injured was making a s;ice in the 3c belt when the comalongs come loose and struck injured int he right knee causing a contusion. Injured returned to work the next day. After MRI injured was given restricted duty.
Injured stated that as he was attempting to exit the bobcat he got his right foot caught between the bucket and the frame. He was taken to the doctor and nothing broken, just mashed. He returned to work the next day. Had MRI 9-25-05. After reading the MRI, the doctor wanted him to stay off foot for a week to 10 days. Has not worked since 9-27-05.
Injured stated that he was attempting to back the miner up to clean up some rock that had fell from the roof he got himself caught between the miner and the rib causing contusions to his right leg, left arm and leg. Injured returned to work the next day on restricted duty.

INJURED STATED THAT HE WAS ATTEMPTING TO HELP ANOTHER EMPLOYEE RAISE THE BATTERY LIDS ON THE #4 SCOOP. WHEN THE EE TURNED LOOSE OF THE LID HIS FINGER WAS CAUGHT UNDER THE LID CAUSING A FRACTURE & LACERATION TO THE FIRST KNUCKLE OF THE RING FINGER ON HIS RT. HAND. INJURED WAS WEARING GLOVES & HE RETURNED TO WORK THE NEXT SCHEDULED SHIFT.
Injured stated that as he was attempting to cut a band around blocks, the band broke causing a 2-3"" laceration on his right wrist. Injured was taken to the Dr. & received 6 stitches.

Injured stated that as he went to move a battery on the golf cart that had a post burn off, the battery flew up in his face causing discomfort to his right eye. Injured was wearing his safety glasses. Injured wen to the doctor and received prescription medication.
Injured stated that he hung his foot in the miner cable and twisted his right knee. Injured returned to work on 4-23-05. On 4-25-04 injured bent down and knee popped. Injured went to doctor and returned to work on 4-26-05. Injured has another doctors appt. on 4-29-05.
injured stated that while working on equipment throughout the shift his back started hurting. des not know what he done. it started hurting on 2-22-05. he continued working, it continued hurting and on 2-28-05. injured went to the dr.

On 12-28-04 injured called & said that he went to the ER on 12-24-04 & to the chiropractor 2 times since then. Injured returned to work on his next regular scheduled shift. Didn't become reportable till 12-24-04.

INJURED STATED THAT HE WAS TAKING THE COVER OFF THE ATRS ON THE #2 UNIT BOLTER TO CHANGE A HOSE,WHEN THE COVER FLEW UP AND HIT INJURED JUST ABOVE THE LT. EYE CAUSING A 3""LACERATION. INJURED WE NT TO THE HOSPITAL AND RECEIVED 3 STITCHES & RETURNED TO WORK THE NEXT SHIFT.
INJURED STATED THAT WAS TRAMMING THE #1 UNIT SCOOP DOWN THE 1C ROAD WHEN HE RAN OVER AN OBJECT IN THE ROAD AND BOUNCED HIM UP AGAINST THE CANOPYOF THE SCOOP HURTING HIS BACK BETWEEN THE SHOULD ER BLADES. INJURED FILLED OUT AN ACCIDENT REPORTON 09-25-02 AND WENT TO THE DOCTOR ON 9-26-02.HERETURNED TO WORK THE SAME DAY. HAS BEEN GOING TO A CHIROPRACTOR FOR TREATMENTS.
INJURED STATED THAT WHILE HELPING CHANGE A GOLF CART TIRE HE FELT A PULL IN HIS LOWER BACK. INJURED WENT TO THE DR. AND RECEIVED MEDICATION.
INJURED STATED THAT AS HE WAS ATTEMPTING TO GET OUT OF THE WAY OF A CAR CABLE HE TWISTED HIS RT.KNEE. INJURED WORKED UNTIL 9/16/02 AT WHICH TIME HE WENT TO THE DR. INJURED RETURNED TO WORK THE NEXT SHIFT. HE HAS THERAPY. DID'NT BECOME REPORTABLE UNTIL 9/16/02.
INJURED REPORTED ON 9/9/02, WHILE AT THE DOCTOR'S OFFICE, THAT HE THOUGHT HE HURT HIS RIGHT SHOULDER WHILE AT WORK. HE DIDN'T KNOW WHEN, WHERE, OR HOW. HIS SHOULDER STARTED HURTING ON THE NIGH T OF 9/7/02 AND WORSENED ON 9/9. HE WENT TO THE DOCTOR AND RECEIVED MEDICATION AND THERAPY. HAVING MRI ON 9/13. HAD SURGERY ON BOTH SHOULDERS PREVIOUSLY. WORKER'S COMP. SAYS REOCURRANCE.
INJURED WAS LIFTING ROCK OUT FROM BEHIND WHEEL UNIT ON #4 CAR ON #1 UNIT WHEN HE STRAINED HIS LEFT SHOULDER. HE WENT TO THE DOCTOR & HAD X-RAY. SCHEDULED TO GO TO A SPECIALIST LATER, PROBABLY TO HAVE A MRI.
INJURED WAS CUTTING THE FACE OF #1 ENTRY ON #2 UNIT WHEN SOME ROCK FELL ON TOP OF THE MINER, INJURED RESET THE MINER AND CONTINUED CUTTING THE FACE AND A PIECE OF ROCK SLID OFF THE MINER AND HIT INJURED ON THE LEFT KNEE CAUSING SOARNESS. INJURED FINISHED THE SHIFT AND WENT TO THE DOCTOR THE NEXT DAY AND RECEIVED A PRESCRIPTION FOR SOARNESS.
INJURED STATED THAT AS HE WAS CLEANING OFF THE TOP OF THE ROOF BOLTER ON #1 UNIT TO LOAD SUPPLIES WHEN A PIECE OF ROCK FELL AND HIT HIS LT. HAND CAUSING SWELLING AND A CONTUSION. INJURED WENT TO THE UNION HOSPITAL E.R. HE WENT THE NEXT DAY TO SEE ANOTHER DR.
INJURES WAS HELPING CHANGE A BOOM LIFT JACK ON THE #2 UNIT ROOF BOLTER WHEN THE JACK FELL AND MASHED INJUREDS PINKY FINGER ON HIS RIGHT HAND. INJURED WAS TAKEN TO THE HOSPITAL AND RECEIVED 11 STITCHES.
INJURED WAS HELPING GET A PIECE OF ROCK OFF THE #2 MINER WHEN A CORNER BROKE OFF OF IT AND INJURED FELL INTO THE ROCK CAUSING A LACERATION TO HIS LEFT SHOULDER AND RIGHT CHEEK. INJURED WAS T AKEN TO HOSPITAL AND RECEIVED SEVERAL STITCHES.
AT APPROX 1:10 PM ON #1 UNIT AN IGNITION OF GAS OR DUST POSSIBLY OCCURRED. MSHA INSPECTOR INVESTIGATED ACCIDENT. KDMM INSPECTOR INVESTIGATED ACCIDENT. THESE WERE REPORTED TO MSHA DISTRICT OFFI CE IMMEDIATELY.
INJURED WAS WORKING ON #1 UNIT FEEDER CHAIN WHEN A FLIGHT FELL AND MASHED HIS RIGHT RING FINGER. HE WAS TAKEN TO THE HOSPITAL. HE HAD SURGERY; A PIN WAS PUT IN HIS FINGER.
INJURED STATED HE WAS ATTEMPTING TO PICK UP A 4.5' ROUND TIMBER TO SET ON #2 UNIT BELT WHEN HESUPPOSDELY HURT HIS LOWER BACK. INJURED WAS TAKEN TO THE HOSPITAL BY AMBULANCE. A MRI WAS RUN & SHOWED NO DISC DAMAGE. APPARENTLY HE HAS A STRAINED MUSCLE. INJURED IS HAVING THERAPY.
INJURED STATED THAT HE WAS WALKING DOWN THE POWER ENTRY DRAGGING A CABLE WHEN HIS HAT FELL OFF &HE HIT HIS HEAD ON A ROOF BOLT CAUSING A 1""LACERATION. INJURED WENT TO THE DR. & RECEIVED 2 STIT CHES.
INJURED STATED THAT HE WAS TRAVELING DOWN #1 UNIT SUPPLY ROAD IN A SCOOP WHEN A PIECE OF ROCK FELL AND STRUCK HIM IN THE RT. RIB CAGE CAUSING PAIN. INJURED WENT TO THE DR. & HAS A CRACKED RIB.
INJURED WAS RIDING IN #1 UNIT MANTRIP WHEN THE BRAKE CALIBER BLOWED. INJURED JUMPED OUT OF THE TRIP & SPRAINED HIS RIGHT ANKLE.
INJURED WAS HELPING LOAD THE ROOFBOLTER ON #1 UNIT WHEN HE WAS HIT IN THE MOUTH WITH A HANDFULL OF BOLTS CHIPPING HIS 2 FRONT TEETH, AND CRACKING THE 2 BESIDE THEM. INJURED WENT TO THE DENTI ST TO HAVE TEETH REPAIRED.
INJURED WAS OPERATING THE #2 CONTINUOUS MINER ON#1 UNIT WHEN A PIECE OF ROCK SLID OFF THE MINER AND STRUCK INJURED ON HIS RT. ARM CAUSING A 3"" LACERATION. INJURED WENT TO THE DR. AND RECEIVED 6 STITCHES. INJURED RETURNED TO WORK NEXT DAY.
ON 4-30-02 AT ABOUT 9:40PM INJURED WAS ATTEMPTING TO HOOK THE PREP STACKER BELT TO THE ENDLOADER TO MOVE WHEN A LOUD THUNDER WAS HEARD. NO FLASHWAS SEEN. INJURED FELL BACKWARDS ON THE GROUND. HELP ARRIVED SOON, INJURED WAS BREATHING AND HAD A PULSE. THOUGHT HE WAS STRUCK BY LIGHTNING. AFTER TESTS, NO ELECTRICAL CHARGE WAS DETECTED. HAD OTHER PERSONAL MEDICAL PROBLEMS.
INJURED WAS INSTALLING ROOF BOLTS WHEN A PIECE OF ROCK FELL FROM BETWEEN BOLTS & STRUCK HIS LEFT LEG CAUSING A 2"" LACERATION. INJURED WENT TO DOCTOR, RECEIVED 2 STITCHES & WAS RELEASED 4/24/02 BUT INJURED SAID LEG WAS SORE. WE WORKED HIM OUTSIDE FOR 2 DAYS. ON 4/26 INJURED QUIT.
