EE WAS MOVING A WATER HOSE AND PUMP FROM ONE LOCATION TO ANOTHER. AS HE PULLED ON THE WATER HOSE, IT CAME LOOSE AND STRUCK HIM OVER THE LEFT EYE.

EMPLOYEE WAS TRANSPORTING WASTE IN A 777 ROCK TRUCK. WHEN THE LOAD SHIFTED, THE TRUCK CAME OFF THE GROUND AND BACK DOWN. AT THAT POINT THE EMPLOYEE SUFFERED STRAIN TO THE BACK. HE SPENT OVERNIGHT IN THE HOSPITAL.
