Fall to the walkway or working surface

A Pea gravel conveyor belt failed causing the top section to collapse down. Belt had a catwalk on the side with proper rails for employees to travel for maintenance. There was no indication of structural damage or defects where the failure occurred.

They will changing the screen in plant #2 and a piece of rust got in his right eye. The washed it out.
