A Pea gravel conveyor belt failed causing the top section to collapse down. Belt had a catwalk on the side with proper rails for employees to travel for maintenance. There was no indication of structural damage or defects where the failure occurred.

Pea Gravel Conveyor structure failed at the front section and collapsed down. There was no indication of structural damage or defects where the failure occurred.

Operator was pushing dirt with dozer and dirt fell on operators foot on platform of dozer, where operator was sitting.
