1604 QUARRY & PLANT Metal/Non-Metal

Employee was bending over to pick up a trash bag from a box that was on the floor. The employee did not notice a metal pipe that was next to the box on the floor and as the employee bent down to pick up a trash bag, EE's left forehead came in contact with the end of the metal pipe resulting in a laceration to the top left side of EE's forehead.

As the employee was stepping out of the raw mill, the employee placed a hand on the raw mill tire hook and disrupted the accumulated dust causing it to become airborne. The airborne dust entered the airstream of the cooling vent fans and blew onto the employees face and under safety glasses causing dust to enter left eye.

Employee was walking backwards down the backside of the clinker dome access ramp when EE felt a strain in EE's right knee. The employee continued to experience pain in EE's knee and was referred to an orthopedic specialist for further diagnosis and evaluation.

Employee was opening the water truck cab door to open the water tank fill valve when the tip if EE's right fifth finger got wedged in between the cab door frame and the water fill tank valve resulting in a laceration.

Employee was using a prybar to open the southeast door to raw mill separator. The prybar slipped causing EE's middle finger to become wedged between the prybar and the platform handrail.

Employee was working on the conveyor gallery reaming some holes at the time of the incident. EE had left the impact gun on the top beam of the conveyor while coming down to grab some tools. EE had a hand on a lower beam at ground level and the impact gun that was left up top fell smashing EE's finger.

EE straddled the finish mill 1 pinion shaft bearing drive line guard to access the oil level plug located on the opposite end of the platform. They had their left leg on the platform and as they swung their right leg behind them they felt a pop in their right knee as they planted their foot on the platform. There is ladder access on the opposite end to access the oil plugs.

The metal fork (1 inch X 24 inch) that supports the empty bags on the portland bag applicator was bent in a downward angle approximately 2 inches and as the employee attempted to lift up and straighten the fork the employee felt a pop below right shoulder

Employee was walking on a flat wet paved road carrying 2 shovels over right shoulder when EE's right foot slipped causing EE to lose balance and fall to the ground in a backward twisting motion.

Employee was using a shovel to move a large chunk of clinker from the back of the clinker hopper grating to the front. As the employee was using the shovel to move the chunk of clinker, EE felt a pain in lower back.

Employee was pulling down with a pipe wrench in right hand, while holding a 1/2 inch pipe fitting with a pair of channel locks in the left hand. The employee heard a pop in the back area of left shoulder while applying downward force with right hand while trying to loosen fitting.

Employee was engaging locking mechanism to secure the raw mill doors when some dirt from area above EE fell behind the upper part of EE's safety glasses frame and entered the employee's left eye.

Employee was standing on a ladder closing a door shut on ball mill inlet throat. The door was not secured in place and as employee stepped down the ladder the door swung open striking employees hard hat.

A production utilityman was cleaning the walkway along the lime/gypsum conveyor belt. While cleaning the walkway, they slipped on material and fell onto elbow causing a laceration requiring 4 stitches.

Maintenance employee was performing tracking adjustments on the palletizer belt. The employee had a hammer in right hand and was holding on to the belt frame with left hand. As the employee began to tap on the belt roller with the hammer, the hammer was caught by the moving roller and the employee's upper arm became pinched between the roller and conveyor belt.

Employee was removing the bottom of an air slide pan that was with 3 other employees. They were using 2 come-a-longs to lower the airslide pan. The come-a-longs were then removed leaving the airslide pan supported by the pipe structure. The employees lifted the pan, the pan slipped and employee reached out at an awkward angle and felt discomfort in stomach.

Employee was holding a 3/4 inch conduit strap with left fingers and a cordless band-saw in EE's right hand. The band-saw was partially resting on a cardboard box, as EE began to cut the conduit strap the cardboard box shifted causing the band-saw blade to come in contact with EE's left index finger causing a laceration to right index finger requiring 3 stitches.

Employee proceeded to walk to the north side of preheater tower and on the way back to work area, right foot got caught on a scaffold gate that was laying on pathway floor causing right leg to twist resulting in a trip and fall on right knee landing on scaffold pipe knuckle attachment that was secured to scaffold gate.

Employee was using an air lance to poke through a thermocouple port opening on a plugged preheater tower cyclone, when the air was turned on to the air lance, hot material poured out of port opening splashed on employees Nomex hood around the right ear and face area. The heat from the hot material radiated through the Nomex hood and caused burns to both these areas.

At the request of the Alamo employee, a contractor tossed a 22 lbs. box of ceramic fiber insulation across the raw mill roller pair to the Alamo employee. The Alamo employee had left arm extended and lying across the roller pair seal air fan hose. The employee heard a pop in elbow area as arm hyperextended when the box made contact with the left arm.

Employee had just closed air valve on an air line on 3rd floor of preheater tower. As EE was walking away from area, EE felt dust fall into left eye. Employee is unsure where dust came from.

Employee was opening a port on preheater tower cyclone, when a piece of coating fell and forced some material to come out of port. Some of this dust got into employees right eye.

Employee was tapping on the seat of a metal tipping gate with a hammer to try and loosen material because the gate would not fully close. During this process the gate closed crushing employee's right index finger against hammer and gate flap.

Employee was cleaning the preheater tower using the water blaster. During this cleaning a large chunk hit water blaster lance causing it to twist while employee was engaged with gun trigger. The trigger part of the gun also twisted along with employee's left hand ring finger, causing a fracture of tip of left ring finger.

EMPLOYEE WAS CLEANING ON PAVED AREA ALONG DRAINAGE DITCH, MIS-STEPPED AND LANDED ON KNEES.

EMPLOYEE WAS LIFTING CEMENT MOLDS CONTAINER OUT OF MOISTURE CABINET WHEN EMPLOYEE FELT PAIN IN NECK AND SHOULDER AREA.

EMPLOYEE WAS DESCENDING FROM HAUL TRUCK LADDER WHEN EE'S FOOT SLIPPED ON THE LAST MUDDY LADDER RUNG CAUSING EE TO FALL TO GROUND ON EE'S BACK. DURING THIS EE INCURRED A LEFT WRIST AND LEFT KNEE SPRAIN.

EMPLOYEE WAS WORKING IN THE RAW MILL. EMPLOYEE WAS HOLDING A LINER PLATE IN PLACE AS THE BOLTS WERE BEING CUT OFF FROM PLATE FROM THE OUTSIDE. THE PLATE SLIPPED AND AS THEY ATTEMPTED TO GRASP THE PLATE IT SLIPPED AND CUT THROUGH THE GLOVE CAUSING A LACERATION ON THE LEFT HAND PALM THAT REQUIRED 4 STITCHES.

EMPLOYEE WAS LIFTING A GUARD COVER TO RE-INSTALL IT ON THE MASONRY PACKER CONVEYOR WHEN EE FELT A PAIN IN LOWER BACK. THE GUARD WAS IN THE UPRIGHT POSITION LEANING AGAINST THE BELT FRAME, BUT HAD A FOOT OF CEMENT AROUND IT CAUSING IT TO BE HEAVIER THAN IT NORMALLY IS.

EMPLOYEE HAD JUST CLEANED A MATERIAL SPILLAGE INTO BOBCAT BUCKET. EMPLOYEE HAD ALREADY CLIMBED ONTO BOBCAT, BUT AS EE TURNED TO SIT INTO THE CAB OF THE BOBCAT, EE'S FOOT SLIPPED OFF OF STEP AND WEDGED BETWEEN BOBCAT BUCKET AND FRAME OF BOBCAT CAUSING EE'S RIGHT FOOT TO TWIST.

Employee was shoveling clinker out from under a belt conveyor counterweight. EE right foot slipped on a piece of clinker, causing ee to jerk to catch self and twisted back in the process.

A piece of plywood shielding board became dislodged and fell onto individuals L pinky finger resulting in a fracture.

EE was working changing a line on compressor, standing on skid at compressor. EE re-positioned right foot and left foot slipped on the steel and fell backward striking head on the concrete resulting in laceration requiring 8 staples.

EE was attempting to make a curtain over door window of excavator with a towel and slammed finger in door causing a fracture of right ring finger

EE was lifting a piece of metal plate flooring, approx 4'x5', weighing approx 100# to access work area. As EE slid the plate, EE lost grip & it dropped onto the fingers of EE's right hand. EE snatched EE's hand out from under the metal plate which caused the skin of EE's right forefinger to tear back, requiring 4 stitches. EE was not wearing any gloves at the time.

Employee was cleaning the burner pipe on the kiln with an air lance. As the employee was extracting the lance from the port, something fell and made an impact on the lance causing it to flip up and around and hit EE on EE's left wrist causing a hand sprain.

EE was inside the kiln moving a 2x12 board from across the kiln to the floor of the kiln. He picked up the board and tossed it to the side when it ricocheted off the rigging and landed on the top of his right foot causing his foot to swell. The ee was wearing steel toe boots at the time of the incident.

EE was repositioning forks on the forklift and felt a pop in his arm.

EE was walking down stairs carrying 2 empty 1 gallon buckets. He felt a pop in his right knee. He waited a few minutes and didn't hurt or bother him so continued on to collect samples. Didn't report until 2/9/15. Went to doctor & released to regular duty. Requested to return to doctor on 2/23/2015.

Employee was climbing down from 980 loader and reached the 2nd step when his foot slipped off of the step causing him to fall back onto the concrete slab. Employee was complaining of lower back pain.

Employee was clearing build up of material from discharge box for routine pm inspection when material that was above his hand broke free and fell and pushed his hand down onto the lip at the bottom of the door opening causing a laceration. The employee was wearing leather gloves and did not require stitches.

EE & contractor was working in the flow control gate area. After removing a valve they used a come-along to raise it from the floor to a working surface of just over 2 feet. The ee had his hand positioned on each side of the valve unit as the contractor was removing the come-along from above. The come-along slipped & fell down, landing on the ee's left thumb,causing a fracture.

EMPLOYEE WAS CHECKING THE PREHEAT TOWER WHEN AN AIR CANNON DISCHARGED PUTTING HOT AIR ON HIS LEFT ARM AND LEFT SIDE OF FACE. CAUSING BURNS.

EMPLOYEE WAS PLACING SCRAP METAL IN THE DUMPSTER AND CUT HIS LEFT RING FINGER REQUIRING STITCHES TO CLOSE WOUND.

EMPLOYEE WAS PULLING WEEDS AND CUT FINGER. THE CUT REQUIRED 3 STITCHES TO CLOSE.

THE EMPLOYEE, TRANSIT DISPATCHER FROM SCALE, WAS WALKING TO HER CAR AND TRIPPED ON A WATER HOSE, AND FELL TO GROUND. ACCIDENT RESULTED IN A FRACTURED RIGHT ELBOW AND LEFT KNEE CONTUSION. RETURNED TO WORK THE NEXT SHIFT.

EMPLOYEE WAS CHECKING THE TARGET ON A TIPPING GATE THAT LOOKED LOOSE, WHEN GATE ARM MOVED AND CUT HIS FINGER. REPORTED DUE TO STITCHES. NO LOST TIME.

Drill bound up and kicked back causing drill to strike head. Causing laceration above right eye.

EMPLOYEE WAS SHOVELING MATERIAL ON SOUTH SIDE OF BUILDING AND STRAINED HIS LEFT SHOULDER. SURGERY IS NEEDED TO CORRECT THE CONDITION. LOST TIME STARTED ON DECEMBER 13, 2012.

EE received a splinter in (L) hand palm, that became infected

EMPLOYEE WAS CLEANING CEMENT MIXER WHEN DOOR CAME DOWN ON RIGHT HAND CAUSING A CUT ABOUT 2 INCHES LONG. REPORT DUE TO STITCHES BEING REQUIRED.

Holding the discharge hose while the shot was being loaded when the boom suddenly jerked backwards causing the hose to pull his arm backwards hurt right shoulder.

Employee alleged that on 08/02/2011 he was squatting down in a small area and put all his weight on his right foot breaking his right big toe. Employee did not report the accident until 08/08/2011.

EMPLOYEE WAS STANDING ON THE NORTH SIDE OF TAIL PULLEY RESTING FOOT ON THE TOE PLATE, UNDER THE TAIL PULLEY SUPPORT FRAME. BELT TRAVELING UPWARDS CAUGHT THE TOE OF THE BOOT AND PULLED HIS FOOT INTO THE STRUCTURAL IRON.

EMPLOYEE WAS TEARING OUT REFRACTORY IN THE PREHEAT TOWER WHEN A LARGE PIECE HIT HIM ON HIS LEFT FOOT CAUSING A CONTUSION.

EMPLOYEE WAS REMOVING A SEAL AIR PIPE AND GUIDING PIPE WHEN THE PIPE CAUGHT SIDE OF HIS HAND CUTTING THE PALM.

Employee was replacing Hydraulic cylinder of cross-over bridge at scale #4. Cylinder outer sleeve slipped from retaining U-bolts, pinching left forearm between forklift and sleeve as he was securing the bottom plate, causing a compound fracture of the left forearm.

EMPLOYEE STOOD UP AND TURNED FROM KNEELING POSITION ON SLOPED FLOOR OF CONVEYOR GALLERY AND EXPERIENCED A POP AND PAIN IN HIS LEFT KNEE.

EMPLOYEE ALLEGED THAT ON 04/16/2010 HE SLIPPED ON STAIRS WITH IDLER ON A DOLLY AND INJURIED THE SAME ANKLE THAT WAS INDICATED ON THE REPORT OF 12/12/2008 E-DOCUMENT # 730864. ACCIDENT WAS NOT REPORTED UNTIL 05/11/2010. POSSIBLE CONTINUATION OF ORIGINAL FRACTURE OF CALCANEUS FROM 12/12/2008.

EMPLOYEE WAS CLEANING CHALK BIN WHEN A CHUNK OF CHALK FELL AND HIT HIM ON THE LEFT SHOULDER.

EMPLOYEE WAS HOLDING HAND ON OPPOSITE SIDE OF A DOOR DRILLING SIGN. DRILL BIT WENT THRU SIGN AND INTO LEFT HAND. WOUND REQUIRED 2 STITCHES.

EE was bending over to pick up a 6" C-Channel & felt a sharp pain in his back.

Employee was hit by a 3' piece of angle iron that fell from above. Angle iron hit employee's right hand 4th finger on the nail.

EMPLOYEE SLIPPED IN RESTROOM AND SLIGHTLY TWISTED HIS LEFT KNEE.

EE laid down in his vehicle during a break and didn't wake up. He suffered a non work related heart attack.

Employee was entering finish mill #1 by descending a portable ladder. The ladder came away from the wall and employee fell back, twisting his right ankle when landing on the floor of the mill. Work activity was mill liner changing.

Employee received flash burns to the eyes, neck, and face while working in finish mill #1 removing liners. Liners are cut into 250 lbs pieces for removal.

WHILE CLEANING, INLET EMPLOYEE WENT TO PULL AIR LANCE OUT AND FELT A SHARP PAIN ON SHOULDER BLADE AREA.

EMPLOYEE WAS MOVING SPRINGS DOWN DOME 2 STEPS TO REBUILD FEEDER. STRAIN IN LOWER BACK. (DCN: 220093440024-turned to a lost time reportable case of 12/9/09)

While cleaning chute to sand bin, employee positioned himself wrong and felt his back pop.

Employee was lifting a bucket of material when he complained of a pain in the neck area. This condition is being treated by his personal doctor and has been recommended for surgery. The condition is a result of a pinched nerve in the neck area. The condition is not a result of an accident. Report is for record only.

FOR RECORD ONLY. Employee of Uvalde Concrete Company picked up a load of cement in transport truck. While on our property he said that he was having trouble breathing and did not feel good. EMS was called and the driver was taken to a hospital and admitted for heart surgery. There was no accident and the above was NOT our employee.

Cement dust from N13 dust collector bag change out got in employee's eye.

While cleaning preheater floor 2B, closing north door, door got stuck and while helping door it broke loose smashing finger with door latch.

Employee alleges that he received a carpal tunnel injury while shoveling spilled over cement material from the gallery onto the conveyor at the finish mill to cement silo conveyor on August 20, 2007. Worker's comp. has denied the claim. THIS REPORT IS FOR MEMO ONLY.

Employee was removing a conveyor splice joiner pin with vice grips and a hammer. The hammer glanced off the vice grips and struck the top half of his left index finger causing a contusion of the left hand.

Employee slipped with his right foot while standing on a pile of firm cement, rolling his ankle causing a sprain.

While employee was trying to turn a twisted bag of cement on the palletizer, unit cycled and pushed him to the rear wall of the layer pusher area of the unit. Employee sustained contusion to lower back, lumbago, lower extremity abrasion w/o infection, and contusion of lower leg.

Employee was placing 3 to 31/2 inch steel grinding balls on conveyor belt when two balls rolled together mashing left index finger between them.

Employee was walking from 2nd to 3rd floor of pre-heat tower, to next flight of stairs and hit finger on hand rail.

Employee was stepping down a ladder and His foot slipped on the last step. He did not fall, he just sprained his ankle.

Completing port hole inspection on the pre heat tower. Restriction was found right below port on south 3C. Cleared blockage and went to 4th south level to check port. Opened port & within 15 seconds, material shot out causing burns. Material blew under face shield & hard hat.

Employee alleged that after he had punched out for the shift and walking to his car, he fell down injuring his thigh. There was no eyewitness to the accident. The accident was not reported until 12/01/2006.

Cleaning tracks on D-10 dozer with a bar. The bar slipped and hit the dozer frame and then hit the mechanics right hand resulting in a closed fracture of the navicular bone of wrist.

While cutting grass with the weed eater in high weeded area, an insect crawled up employee's leg and bit him. Insect bite of nonvenomous type.

Employee was walking downstairs and stepped on a clinker ball with foot rolling to one side, resulting in a twisted ankle. Continuation of E-Document 380138. EE grabbed the hand rail to catch himself and pulled the tendions in his right arm. The condition worsened with time and required additional medical follow-up and resistricted duty.

Employee was installing a feed chute at the #1 finish mill throat. Unit was being hoisted up by an air tugger. The cable had an overlay wrap off track when chute was being lowered, the wrap of cable let loose and weight of the feed chute shifted to employee, causing a lumbar strain.

Employee was found at bottom of stairs on ground level in office stairs; cause of alleged accident is unknown. EMS was called and employee was given a clean bill of health and released. Employee's statement: fall resulted from a shove. EMS never treated EE. The local fire dept. rendered first aid; the assesment was elevated blood pressure and heat stress.

EMPLOYEE IS GOING INTO SURGERY ON 02/15/2010 TO CORRECT A SHOULDER IMPINGEMENT THAT OCCURRED 0N 07/04/2006. LOST TIME STARTED ON 02/16/2010.

Partial quote from IE statement, "Cleaning debris from kiln feeder riser. I was being supervised by A.R.S.I., he instructed me to manually clean debris . . . As I was removing a brick fell smashing my right index finger."

E/E was removing bolts from under a cement cooler, as employee pulled on a 3/4 ratchet he felt a pull in his (R) Shoulder

Employee states "while riding on a riding lawn mower a spider bit my right upper leg."

Standing outside of a truck bed reaching inside to pull out a tool. Struck elbow on the truck bed while extracting the tool.

Standing erect reaching approx. 1.5 feet in front to loosen an oil filter when sensed a pain in lower back.

Employee was clearing a pile up of bags. The bags shifted and he slipped striking his knee.

Tightening a bolt when the wrench slipped off striking the face.

EE shoveling spilled material ground to shoulder level and strained shoulder.

Carrying a box descending a stairway. Misjudged last step, fell, impact to left knee.

Staging pallets utilizing an electric lift truck. While reversing the lift truck it over- traveled and pinched the employee's hand between a guardrail and the handle of the lift truck.

Employee was inspecting post maintenance repair of the raw meal feed sampler. During the operational test the sampler mixer unexpectedly activited.

EE had filled a pill press with sample material. He installed the top cap but noticed it was cocked to the side. When he reached to correct it his lab coat inadvertantly caught the power handle causing the machine to activate. This spewed the sample material into his eye.

EE was dispatched to unplug a feed chute. After clearing the obstruction, he turned to manually run the machinery and he felt a pop in his left knee.

Utility personnel dispatched to inspect high-temp alarm on L-23 belt conveyor. When they arrived, the conveyor belt was on fire dropping burning rubber to he ground starting a grass fire.

EE was walking down a stairway transporting an induction fan. While carrying the fan he felt a strain in his back.

EE was preparing to cleanout a discharge chute with a hand held air hammer. When he pulled the air hammer trigger, escaping air stirred up a cloud of fine material. This fine material then entered into his eye.

Note--Event did not become reportable until 7/12/2004. EE was walking down a stairway (hands free) and turned the corner and his left knee gave way.

EE WAS FITTING PLATES ON THE PRECIPITATOR AND SOMETHING FELL OR BLEW INTO HIS EYE.

Using a cutting torch, a welder had cut too far into an overhead metal plate without leaving an uncut portion as "safety margin" for final, assisted cut. The plate broke free and struck welder in right eyelid in a swing-type motion. Upper eyelid was torn in a "V"-shape requiring sutures.

Standing on 1 foot block jack-hammering (overhead) refactory from vessel on preheat tower, 6th floor. Bit jammed in refractory. While extracting bit it freed up unexpectantly and EE fell to the ground.

EMPLOYEE WAS LEANING OVER BEAM WITH AN IMPACT WRENCH AND TWISTED CAUSING PAIN IN HIS BACK.

Removing a brake cooler line on a TR-70 haul truck, wrench slipped, top of middle knuckle (right hand) struck drive shaft line.

Please note--Injury became reportable 4-23-04. Walking on level ground, hands free, employee slipped and fell. Small clinker balls present.

Lifting 3 pieces of rebar under 40 lbs. back injury may be pre-existing. EE said he felt the same way three years ago after sleeping in a car for 10 hours. He stated he may have twisted it.

Removed a 70lb bag of cement off a pallet (approx. 46" above ground). Placed on a scale at ground level. Picked up bag and turned entire body and back gave-way.

EMPTYING A 3/4 FULL 55 GAL. BARREL OF SOLID MATERIAL INTO A LOADER BUCKET, THE BARREL WOULD NOT PULL OUT. MANUALLY EXTRACTED BARREL STRAINING LOWER BACK.

Incident became reportable 17 Apr 2004. Pulling 1" hose with Chicago fitting attached to the end over a railing when the end of the hose flew up and struck mouth. On 17 Apr 04 Denist pulled tooth (right central incisor).

NOTE: Event did not become reportable until 6/30/04. EE was walking inside the clinker cooler. He tripped and fell down on his left knee.

WHEN CLEANING FEED CHUTE OF MATERIAL BUILD-UP, A CHUNK FELL AND HIT WRIST. EMPLOYEE DID NOT REQUEST MEDICAL ATTENTION UNTIL 1/2/04.

THE EE OPENED AN INSPECTION PORT ON THE PREHEAT TOWER. SOME HOT MATERIAL CAME OUT & GAVE A 1ST DEGREE BURN ON HIS RIGHT WRIST. 10/8/03 DR. CHANGED DIAGNOSIS TO 2ND DEGREE BURN

EE ALLEGES THAT HE STUMBLED AT THE SAND HOPPER ON A SLOPED PIECE OF CONCRETE. HE PERFORMED HIS JOB DUTIES WITHOUT RESTRICTION FOR 6 WEEKS. HE REQUESTED TO SEE A DOCTOR ON 7-1-03. UPON DR'S RECOMMENDATION AN INVESTIGATION WAS LAUNCHED TO VALIDITY OF CLAIM. CLAIM APPROVED DUE TO LACK OF EVIDENCE 7-15-03. THIS REPORT WAS THEN FILLED OUT.

THE EE STATES THAT HE WAS WRITING ON A DESK, WHEN HE STRAIGHTENED UP & TURNED HE RAN INTO A DOOR THAT HE HAD FAILED TO OPEN ALL OF THE WAY. HE DID NOT REQUIRE MEDICAL CARE UNTIL 5/28/03.

THE EMPLOYEE STATES THAT HE WAS DESCENDING THE LADDER ON HIS LOADER HIS FOOT SLIPPED. HE DID NOT FALL BUT HE FELT A TWINGE IN HIS BACK.

EE WAS HOLDING A FRAME IN PLACE FOR INSTALLATION. A CO WORKER MISSED THE FRAME AND HIT THE EE IN THE BACK OF THE HAND. THE EE DID NOT GO TO SEE A DR UNTIL 4-11-03.

EE WAS APPLYING A LABEL TO A BOTTLE OF ACETIC ANHYDRIDE. WHEN HE DQUEEZED THE BOTTLE IT SQUIRTED SOME INTO HIS RIGHT EYE.

THE EE STATES THAT HE WAS CLIMBING OFF OF A CONVEYOR AFTER CLEARING A JAM WHEN HE TWISTED HIS LEFT KNEE. EE REFUSED FIRST AID OR DOCTOR TREATMENT UNTIL 10-02-02. THEREFORE, IT DID NOT BECOME R EPORTABLE UNTIL THAT DATE.

THE EE WAS REACHING AROUND THE FRONT OF HIS LOADER IN ORDER TO WIPE OF THE WINDSHIELD WHEN HE SLIPPED AND FELL TO THE GROUND.

WORKERS AND SUPERVISOR WERE PULLING I BEAMS FROM THE PREHEATER TOWERS ON THE OUTSIDE OF VESSEL. ONE OF THE BEAMS BECAE STUCK AND THE SUPERVISOR INSTRUCTED WORKERS NOT TO GO INSIDE VESSEL WHILE HE LEFT TO RETRIEVED FALL PROTECTION. AGAINST SUPERVISOR ORDERS 1 WORKER ENTERED THE VESSEL WHILE THE 2 WORKERS BACKS HAD TURNED. THE WORKER SLIPPED AND FELL APPROX. 57 FT.

THE EE AWOKE THE MORNING OF 3/21/02 WITH A SORE NECK. HE ATTRIBUTED IT TO HAVING OPERATED A BOBCAT FOR SEVERAL HOURS ON THE PREVIOUS DAY.

THE EMPLOYEE WAS WALKING ACROSS THE PARKING LOT WHEN HE WAS HIT BY ANOTHER EMPLOYEE IN HIS PERSONAL VEHICLE. THE SPEED WAS LESS THAN 5 MPH.

EMPLOYEE WAS HOLDING A HAMMER WRENCH BEING STRUCK BY ANOTHER EMLOYEE. WHEN THE OTHER EMPLOYEE WAS SWINGING THE HAMMER HE HIT A BOLT, WHICH THE HAMMER BOUNCED OFF OF AND INTO THE HAND OF THE EMPLOYEE.

EE WAS GRINDING ON A SCREEN SUPORT IN A FINISH MILL, WHEN A FOREIGN OBJECT ENTERED HIS LEFT EYE. HE WAS WEARING HIS SAFETY GLASSES AT THE TIME.

EE WAS INSTALLING CONDUIT IN THE COAL RMS BASEMENT. AFTER A COUPLE OF DAYS HE DEVELOPED A RASH ON HIS HANDS.

EMPLOYEE WAS USING AN ELECTRIC GRINDER TO GRIND METAL WHEN HE FELT SOMETHING GO INTO HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES AT THE TIME.

A MOBIL EQUIPMENT MECHANIC WAS STRIKING AND END BIT ON A BULLDOZER WITH A HAMMER IN ORDER TO SEAT IT INTO POSITION. A PIECE OF THE BIT CHIPPED OFF AND FLEW INTO THE OPERATOR'S KNEE.

THE EE WAS DESCENDING A LADDER ON A PORTAL RECALIMER. WHEN HE REACHED THE GROUND LEVEL HE LANDED WRONG AND TWISTED HIS KNEE.

THE EE DEVELOPED A RASH OVER HIS ARMS AND UPPER BODY.

THE EMPLOYEE WAS WORKING ON A RAIL NORTH OF THE KILN INLET WHEN THE WIND BLEW DUST INTO HIS EYES. THE EMPLOYEE WAS WEARING SAFETY GLASSES AT THETIME.

EE WAS CLEANING OUT DEBRIS INSIDE KILN WHEN HE STEPPED INTO SOME HOT DUST THAT GOT INTO HIS SAFETY BOOT, CAUSING A SLIGHT BURN ON THE RIGHT HEEL.

EE STATES THAT HE WAS ENTERING THE PREHEATER ELEVATOR DOOR, & ACCIDENTALLY TRIPPED ON THE LOWER JAM, CAUSING HIM TO FALL & HE FELT PAIN IN THE NECK AREA.

EMPLOYEE WAS ATTEMPTING TO CHANGE OUT A DAMAGED 70 AMP CIRCUIT BREAKER AT THE CRUSHER ELECTRIC ROOM WHEN AN ELECTRIC ARC FLASH OCCURRED CAUSING BURNS TO HIS FACE AND RIGHT HAND. THE ARC FLASH WAS DUE TO THE BREAKER BEING ENERGIZED AND TWO INCOMING HOT LINES CAME IN CONTACT.

EE WAS REMOVING BAGS OF CEMENT FROM THE ROTATIONBELT FOR THE CAR PALLETIZER, WHEN HIS LEFT FOOT CAME BETWEEN THE BAG DIVERTERS, LOST HIS BALANCECAUSING A STRAIN TO THE LEFT THIGH.

EMP WAS HELPING TO UNPLUG THE FIRST STAGE CYCLONE OF THE PREHEAT TOWER WHEN HE WAS ACCIDENTALLY SPLASHED WITH HOT DUST, CAUSING MINOR BURNS TO THE UPPER CHEST AND NECK.

EMP STATES THAT HE WAS WORKING INSIDE THE #1 FM CEMENT COOLER AND WAS IN CONTACT WITH A LOT OF CEMENT DUST, CAUSING AN ALLERGIC REACTION ALKALI BURN TO THE UPPER TORSO.

WHILE WORKING ON A ROADWAY PROJECT IN BEXAR CTY, THE CO WAS BEING ALLOWED BY ALAMO CEMENT TO DUMP EXCAVATION MATERIAL FROM THE ROADWAY ONTO THEIR PROPERTY. A EUCLID WAS DUMP THE MATERIAL IN AN OPEN AREA, DOZER WOULD PUSH THE MATERIAL OFF INTO A HOLE. THE EUICLID BACKED TOO CLOSE TO THE EDGE AND WENT OFF THE EMBANKMENT AND TURNED OVER. THE DRIVER WAS TAKEN TO HOSP. TWO CUTS HIS HEAD

EMP STATES THAT HE WAS WORKING IN THE RAW MILL, MAINLY ON THE KNEES AND NOTICED THAT HIS RIGHT LEG STARTED SWELLING ON OR ABOUT 8-17-95. HE WAS SEEN BY DR ON 8-18-95. UNDERWENT ARTHROSCOPIC SU RGERY 9-11, RETURNED TO WORK 10-7-95.

EMPLOYEE STATES THAT HE WAS REMOVING THE FILTER FROM THE FIRST STAGE CO ANALYZER, WHEN THE SYSTEM PURGED AND BLEW DUST INTO THE RIGHT EYE.

EE STATED THAT HE WAS WALKING DOWN THE PLANT OFFICE STAIRS, CARRYING HIS TOOL POUCH. HEE ACCIDENTALLY STEPPED WRONG, AND FELT A SHARP PAIN IN THE LOWER BACK. HE WAS SENT TODR. ON 4-4-95 AFTER COMPLAINING OF BACK PAIN.

EE WAS USING AN AIR CHIPPING HAMMER TO REMOVE BUILDUP FROM THE RAW MILL BUFFER BIN, WHEN A PIECE OF BUILDUP BROKE FREE, STRIKING THE HAMMER AND CAUSING THE HAND HOLDING THE HAMMER TO STRIKE TH E RIGHT SIDE OF HIS FACE.

EMPLOYEE WAS INSPECTING THE KILN INLET THROUGH THE INSPECTION DOORS WHEN ALL OF A SUDDEN THE SYSTEM WENT POSITIVE & IN AN EFFORT TO GET OUT OF THE WAY HE ACCIDENTALLY STRUCK HIS RT HAND ON A N EARBY HANDRAIL

EMPLOYEE WAS EMPTYING (55) GALLON TRASH BARRELS INTO THE BUCKET OF A FRONT END LOADER, WHEN HE ACCIDENTLY CAUGHT THE RIGHT HAND BETWEEN THE BARREL AND THE BUCKET, CAUSING A STRAIN.
