Employee stepped onto the bumper of the service truck and EE's knee gave out.

Employee was swinging a sledge hammer to free a steel component. As the hammer made the upswing it hit a piece of steel causing the hammer to deflect as it traveled back to the ground. The employee was struck by the handle of the sledge hammer in his shin. The result was a bruise that became infected over the weekend.

Employee was using a wrench to tighten the bolt that was holding a cutting edge on his loader bucket. The wrench slipped and the employee lost his balance.

Employee slipped on some wet material under the conveyor in the pit in July 2013. He felt fine after the incident and did not report it to the company. The employee started to have some discomfort with his left knee in early August 2013 he was place on work modifications after an MRI on 8/12/2013. with a sprained ACL/PCL Physical Therapy for a treatment plan.

Employee was welding on the jaw crusher. He stopped welding and lifted his hood to remove some slag and a hot metal piece hit him in the corner of the eye. The employee returned to the job after being checked out by a doctor that same day.

The employee was lifting a piece of steel out of a steel rack with another employee. He needed to pick his end of the steel up a little to get over a lip at the end of the steel rack and when he did he felt pain in his left arm.

Crusher repairs: The supervisor welded a pad eye on the jaw plate and the employee attached a come-along to the pad eye. During the work activity the employee increased the tension on the chain and the weld holding the pad eye in place failed. The chain from the come-along hit the employee's elbow.
