On 3/10/2026 6:30 p.m. management was made aware that the surface trash compactor storage container had material inside on fire. This metal container measures eight feet by 22 feet and is used to transport trash from the mine property to an offsite dump. MSHA was notified and the fire was extinguished. There was no damage to property or injury due to this incident.

On 3/6/2026 3:20 a.m. management was made aware power had been lost to the slope hoist. This being the secondary escapeway, all mine personnel were evacuated using the primary escapeway. MSHA was notified when power restored at 4:02 a.m. No miners were injured due to this incident.

The EE walked to a roadside berm that had 2 feet wide opening to allow water to exit the roadway into a ditch. While standing at this opening, the ice underneath EE broke, causing EE to lose EE's balance. EE then slid down the ditch wall 4 feet before EE's left foot contacted a tree branch. This contact resulted in a dislocated and fractured ankle.

On 2/4/2026 an employee was using a spud bar to clean material from a clogged chute at the surface crusher when EE injured EE's right pinky finger. EE refused treatment at time of incident, but after seeing a doctor on 2/26 & 2/27 it was determined EE's finger had been fractured.

On 12/18/2025 the vertical shaft hoist (primary escapeway) was taken out of service due to not being able to close the surface landing door. No miners were in the mine when this issue was found. It was determined a bolt on the landing door track needed replaced.

On 12/1/2025 MSHA was notified that the vertical shaft hoist was being taken out of service due to intermittent issues. The brake alarm was resolved after adjustment per manufacturer specification. There was also a problem with a control wire, which was changed out with a spare. No miners were injured due to this incident.

A Kiln laborer suffered a partial thickness burn to the palm of left hand. This injury happened while they were using a metal pipe to remove hot material from the discharge end of kiln no. 2.

On 2/3/2025 at 11:20 a.m. the shaft elevator was taken out of service due to having run down over the travel position. A broken wire feeding an electrical switch was determined to be the cause, and no miners were in the car. MSHA was notified at 11:47 a.m. A spare wire was used to replace the broken one and the elevator was put back in service at 1:15 p.m.

On 10/2/2024 at 9:32 a.m. the shaft elevator was taken out of service due to a door switch failure. The elevator was located at the surface landing with no miners on the car when the switch failure was found. MSHA was notified at 9:54 a.m. The switch was replaced and put back in service at 11:26 a.m.

The EE was loading wooden cribbing blocks weighing approximately 20 pounds when they reported pain in their right shoulder. It was determined the blocks were being lifted in the bed of the service truck without lowering the tailgate.

The slope hoist operator reported a slack rope as the operator lowered the hoist car at 1:40 pm on 7/22/2024. The hoist was being used to lower supplies into the mine, with no miners on the car. After inspecting it was determined the car had jumped track, due to a rock in its travel way.

On 4/1/2024 at 3:55 pm the elevator was determined to have a hydraulic brake system failure caused due to overheating. MSHA was contacted at 4:14 pm. There were no miners on the elevator and the mine was evacuated using the secondary escapeway. Elevator was put back in service on 4/3/2024 at 6:15 pm.

No new incident was reported to management to support this claim. Employee stated on 11/13/ 2023 scaling causes pain in their back.

A mechanic states while assisting during the changing of screens at the Wash Plant EE rolled right ankle. As EE was handling screens weighing less than 5 pounds and approximately 12" x 12" in size, EE's right ankle turned after stepping on a rock.

At 1:50 pm on 6/23/2023 the primary escapeway elevator was taken out of service. It was determined an encoder had lost the position of the elevator during a weekly inspection. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 6/22/2023.

At 2:45 pm on 5/17/2023 the primary escapeway elevator was taken out of service. It was determined a hydraulic valve had failed causing the brake to lock. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 5/18/2023.

EE reported an injury to left ankle after descending step at the wash plant. As the mechanic approached one of the stairs landings, they turned right putting pressure on left foot already on the landing. This step and turn resulted in pain to their left ankle.

At 5:05 a.m. management was notified the secondary emergency escape hoist was out of service due to a belt being burnt off the motor's alternator. MSHA was notified around 5:34 a.m. No miners were in the mine at this time. Repairs were made and the hoist was put back in service at approximately 6 a.m. MSHA was notified hoist back in service shortly after 6 a.m.

At approximately 8:15 p.m. the main slope hoist stopped working due to an electrical issue. MSHA was notified at 8:40 p.m. No miners were in the mine during the time hoist was out of service. Repairs were made and the hoist was back in service at 1:45 a.m.

At 8:45 a.m. management was notified the secondary emergency escape hoist was out of service due to electrical issues. MSHA was notified around 9:15 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at approximately 4 p.m. MSHA was notified hoist back in service shortly after 4 p.m.

At 7:03 a.m. management was notified that the wheel brakes on the mobile crane used for the secondary hoist were not working properly, and it was taken out of service. Evacuation of the mine was started and the first attempt to contact MSHA was at 7:33 a.m. A second attempt was made which was successful. Repairs were made and the crane was put back in service at 7:48 a.m.

At 3:49 p.m. on 11/2/2022 management was notified that the boom extension at the secondary hoist was not working properly, and it was being taken out of service. Evacuation of the mine was started and MSHA was notified at 4:07 p.m. On 11/6/2022 the boom has been replaced and the secondary hoist is back in service. MSHA was notified at 9:06 pm on 11/6/2022.

At 10:50 a.m. on 11/1/22 management was notified that the door on the emergency capsule was not operating properly, and it was being taken out of service. The mine was evacuated and MSHA was notified at 11:10 a.m. Repairs were made, and the hoist was put back in service on 11/2/22 at approximately 2:00 p.m. MSHA was notified the hoist was back in service at 2:28 pm on 11/2/2022.

At 3:42 p.m. management was notified that there was a mechanical issue with the secondary escape hoist motor and was being taken out of service. The motor was getting too much fuel. MSHA was notified at 3:46 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the emergency hoist was put back in service at 5:00 p.m. MSHA was notified at 5:05 p.m.

At approximately 2:45 p.m. an underground powder loader was injured. EE was reaching in the passenger side of a parked side-by-side utility vehicle when a pick of loose material fell from the rib it was parked next to. The piece of loose material struck the miner in the back causing several fractures. MSHA was notified at 4:10 p.m. due to ongoing patient care at scene.

At 3:00 p.m. management was notified that there was a mechanical issue with the emergency brake on the secondary emergency escape hoist and it was being taken out of service. MSHA was notified at 3:27 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at 6:09 p.m. on 8/04/2022.

MSHA was notified hoist back in service at 3:50 p.m.

At 12:25 p.m. management was made aware the secondary emergency hoist was out of service due to a boom function loss. The mine evacuation started at 12:26 p.m., MSHA notified by phone at 12:39 p.m. (Case # 03295234). All miners were transported safely to surface using the primary escapeway. Repairs were made and MSHA notified hoist back in service at 5:20 p.m.

An employee was walking at ground level and stepped in a 6 inch x 6 inch hole with left foot. This caused EE's right leg to slide out from under EE putting pressure on the left leg. When EE was trying to get up, EE heard a pop and felt their left leg give out. The ground conditions in this area were wet and muddy.

At 12:45 p.m., management was notified that the key for the secondary escape hoist had been lost. Production was stopped and all miners were transported to the surface by the primary slope hoist man car. At 1:12 p.m., MSHA was notified that the loss of this key interfered with the use of the secondary escape hoist. At 1:22 p.m., Management was notified that the key had been found.

At 9:33 AM, the secondary escape way was taken out of service due to damage to the computer screen plug located on the boom truck. Production in the mine was stopped and all miners were transported safely to the surface by means of the slope hoist man car. The hoist was put back into service about 2:45 PM on 6/16/21.

EE radioed in to the day shift supervisor EE did not feel well and was told to park the truck where EE was at two north crusher. Once arriving on surface employee was transported by local EMS to the hospital. EE was then admitted and a medical procedure was completed the following day.

Due to rainy weather, gravel was washed into the safety brake system of the hoist causing a mechanical failure around 3:30 a.m. on 1-1-2019. MSHA was notified at this time. The safeties were cleaned thoroughly and tested multiple times to verify proper function. The hoist was released to run and MSHA was contacted.

The Hoist was giving a Main Brake Hold Alarm. Due to oil sample,the oil in the Main Brake Thrustor was changed. The oil level was checked in the Thrustor and discovered that it was low. After adding ¾ of a gallon, the Thrustor began performing as it should. Oil plug was tightened to stop leaking and Hoist was back in operation.

The conduit and control wiring had pulled loose from the Mine Level Door Switch. A short circuit was caused when the wires touched which in turn caused a PLC output card fault. The PLC was reset, conduit repaired and a new door switch was installed.

The on-site supervisor contacted MSHA as a pre-cautionary measure. In hindsight MSHA should not have been contacted, since the elevator was operable.

Employee was tilting Getman basket to attach a spool of cable to mounting brackets. Latching bar was in upright position and when he tilted basket the bar went past center of gravity and bar fell stiking employees left hand cutting his 2 middle fingers. 8 stitches were required.
