On 12/18/2025 the vertical shaft hoist (primary escapeway) was taken out of service due to not being able to close the surface landing door. No miners were in the mine when this issue was found. It was determined a bolt on the landing door track needed replaced.

On 12/1/2025 MSHA was notified that the vertical shaft hoist was being taken out of service due to intermittent issues. The brake alarm was resolved after adjustment per manufacturer specification. There was also a problem with a control wire, which was changed out with a spare. No miners were injured due to this incident.

A Kiln laborer suffered a partial thickness burn to the palm of left hand. This injury happened while they were using a metal pipe to remove hot material from the discharge end of kiln no. 2.

An employee felt pain in their left knee while descending the stairway at the LKD Load-Out. Employee states they missed a step, but no damage was found to the steps.

Management was made aware at 5:00 am that the vertical hoist was being taken out of service. MSHA was notified at 5:03 am. It was determined a bolt had come out of the linkage with the bottom landing door. Repairs were made and put back in service at 10:00 am.

While removing a multi tone screw EE's left leg was stuck when load shifted.

On 2/3/2025 at 11:20 a.m. the shaft elevator was taken out of service due to having run down over the travel position. A broken wire feeding an electrical switch was determined to be the cause, and no miners were in the car. MSHA was notified at 11:47 a.m. A spare wire was used to replace the broken one and the elevator was put back in service at 1:15 p.m.

On 10/2/2024 at 9:32 a.m. the shaft elevator was taken out of service due to a door switch failure. The elevator was located at the surface landing with no miners on the car when the switch failure was found. MSHA was notified at 9:54 a.m. The switch was replaced and put back in service at 11:26 a.m.

The EE was loading wooden cribbing blocks weighing approximately 20 pounds when they reported pain in their right shoulder. It was determined the blocks were being lifted in the bed of the service truck without lowering the tailgate.

The slope hoist operator reported a slack rope as the operator lowered the hoist car at 1:40 pm on 7/22/2024. The hoist was being used to lower supplies into the mine, with no miners on the car. After inspecting it was determined the car had jumped track, due to a rock in its travel way.

Employee was assisting another employee align a medal pin to be inserted in the stick boom of the rock crusher. When the pin was struck with a sledgehammer, dust entered the employee's eyes that was helping with the alignment.

An employee made management aware on 6/26/2024 that EE received an injury to right knee on 6/25/2024. EE states working on crusher and towards the end of shift EE's knee felt sore.

On 4/1/2024 at 3:55 pm the elevator was determined to have a hydraulic brake system failure caused due to overheating. MSHA was contacted at 4:14 pm. There were no miners on the elevator and the mine was evacuated using the secondary escapeway. Elevator was put back in service on 4/3/2024 at 6:15 pm.

At 6:20 a.m.the primary escapeway hoist was taken out of service. It was determined residue on the hoist cable was engaging the two-block surface safety switch prematurely. MSHA was called at 6:45 a.m. The manufacturer of the elevator has engineered and installed a new 2 Block.

At 4:48 p.m. The primary hoist was taken out of service. It was determined residue on the hoist cable was engaging the two-block safety switch prematurely. MSHA was called at 5:09 p.m. Cable was cleaned, and hoist put back in service at 11:30 p.m.

At approximately 12:30 a.m. management was made aware the secondary slope hoist was not operational due to a transformer failure. This transformer is located on the surface. MSHA was notified at 12:49 a.m. and the mine evacuation started at 1:00 a.m.

The primary escapeway hoist was taken out of service at 10:50 a.m. Residue on the hoist cable was engaging the two-block surface safety switch prematurely. MSHA was called at 11:12 a.m.

Reportable hearing loss as a result of audiometric testing.

On Monday 11/13/2023 management was made aware that an employee had been put on restricted duties. This employee had been previously released with no restrictions back to full duty on 1/10/2023. No new incident was reported to management to support this claim. Employee stated on 11/13/ 2023 scaling causes pain in their back.

A mechanic states while assisting during the changing of screens at the Wash Plant EE rolled right ankle. As EE was handling screens weighing less than 5 pounds and approximately 12" x 12" in size, EE's right ankle turned after stepping on a rock.

At 1:50 pm on 6/23/2023 the primary escapeway elevator was taken out of service. It was determined an encoder had lost the position of the elevator during a weekly inspection. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 6/22/2023.

At 2:45 pm on 5/17/2023 the primary escapeway elevator was taken out of service. It was determined a hydraulic valve had failed causing the brake to lock. MSHA was notified, underground production was stopped, and all miners were evacuated to the surface using the secondary escapeway. MSHA was notified that the elevator was back in service on 5/18/2023.

EE reported an injury to left ankle after descending step at the wash plant. As the mechanic approached one of the stairs landings, they turned right putting pressure on left foot already on the landing. This step and turn resulted in pain to their left ankle.

At 5:05 a.m. management was notified the secondary emergency escape hoist was out of service due to a belt being burnt off the motor's alternator. MSHA was notified around 5:34 a.m. No miners were in the mine at this time. Repairs were made and the hoist was put back in service at approximately 6 a.m. MSHA was notified hoist back in service shortly after 6 a.m.

At approximately 8:15 p.m. the main slope hoist stopped working due to an electrical issue. MSHA was notified at 8:40 p.m. No miners were in the mine during the time hoist was out of service. Repairs were made and the hoist was back in service at 1:45 a.m.

At 8:45 a.m. management was notified the secondary emergency escape hoist was out of service due to electrical issues. MSHA was notified around 9:15 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at approximately 4 p.m. MSHA was notified hoist back in service shortly after 4 p.m.

At 7:03 a.m. management was notified that the wheel brakes on the mobile crane used for the secondary hoist were not working properly, and it was taken out of service. Evacuation of the mine was started and the first attempt to contact MSHA was at 7:33 a.m. A second attempt was made which was successful. Repairs were made and the crane was put back in service at 7:48 a.m.

At 3:49 p.m. on 11/2/2022 management was notified that the boom extension at the secondary hoist was not working properly, and it was being taken out of service. Evacuation of the mine was started and MSHA was notified at 4:07 p.m. On 11/6/2022 the boom has been replaced and the secondary hoist is back in service. MSHA was notified at 9:06 pm on 11/6/2022.

At 10:50 a.m. on 11/1/22 management was notified that the door on the emergency capsule was not operating properly, and it was being taken out of service. The mine was evacuated and MSHA was notified at 11:10 a.m. Repairs were made, and the hoist was put back in service on 11/2/22 at approximately 2:00 p.m. MSHA was notified the hoist was back in service at 2:28 pm on 11/2/2022.

At 3:42 p.m. management was notified that there was a mechanical issue with the secondary escape hoist motor and was being taken out of service. The motor was getting too much fuel. MSHA was notified at 3:46 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the emergency hoist was put back in service at 5:00 p.m. MSHA was notified at 5:05 p.m.

At approximately 2:45 p.m. an underground powder loader was injured. EE was reaching in the passenger side of a parked side-by-side utility vehicle when a pick of loose material fell from the rib it was parked next to. The piece of loose material struck the miner in the back causing several fractures. MSHA was notified at 4:10 p.m. due to ongoing patient care at scene.

At 3:00 p.m. management was notified that there was a mechanical issue with the emergency brake on the secondary emergency escape hoist and it was being taken out of service. MSHA was notified at 3:27 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at 6:09 p.m. on 8/04/2022.

At 1:00 p.m. management was notified that the engine for the secondary emergency escape hoist was out of service due to mechanical issues. MSHA was notified at 1:25 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at 3:30 p.m. on 6/29/2022. MSHA was notified hoist back in service at 3:50 p.m.

At 2:30 p.m. management was notified that the engine for the secondary emergency escape hoist was out of service due to mechanical issues. MSHA was notified at 2:59 p.m. and the mine was evacuated using the primary escapeway. Repairs were made and the secondary emergency hoist was put back in service at 4:06 p.m. MSHA was notified hoist back in service at 4:06p.m.

At 12:25 p.m. management was made aware the secondary emergency hoist was out of service due to a boom function loss. The mine evacuation started at 12:26 p.m., MSHA notified by phone at 12:39 p.m. (Case # 03295234). All miners were transported safely to surface using the primary escapeway. Repairs were made and MSHA notified hoist back in service at 5:20 p.m.

An employee was walking at ground level and stepped in a 6 inch x 6 inch hole with left foot. This caused EE's right leg to slide out from under EE putting pressure on the left leg. When EE was trying to get up, EE heard a pop and felt their left leg give out. The ground conditions in this area were wet and muddy.

At 12:45 p.m., management was notified that the key for the secondary escape hoist had been lost. Production was stopped and all miners were transported to the surface by the primary slope hoist man car. At 1:12 p.m., MSHA was notified that the loss of this key interfered with the use of the secondary escape hoist. At 1:22 p.m., Management was notified that the key had been found.

At 9:33 AM, the secondary escape way was taken out of service due to damage to the computer screen plug located on the boom truck. Production in the mine was stopped and all miners were transported safely to the surface by means of the slope hoist man car. The hoist was put back into service about 2:45 PM on 6/16/21.

Due to mechanical reasons, the main shaft hoist has been taken out of service at the Maysville Operation I.D. # 1507101. No miners were injured and no miners were underground when the hoist was taken out of service. After talking to employee, a call was made to report this outage.

Two employees were empting the Vac truck. The affected employee was inspecting the hopper when some material came out and splashed material in eye.

Reportable hearing loss as a result of yearly audiometric testing.

An employee was operating a 772 haul truck underground when EE said they felt ill. EE radioed in to the day shift supervisor EE did not feel well and was told to park the truck where EE was at two north crusher. Once arriving on surface employee was transported by local EMS to the hospital. EE was then admitted and a medical procedure was completed the following day.

MSHA was notified at app. 7:27 a.m. that the elevator was out of service. The cause was determined to be a key switch input wire. MSHA was again notified at app. 8:41 a.m. that the elevator had been repaired, tested and put back in service. No miner was on the elevator when this happened.

Due to a failed door switch the Elevator was reported down to MSHA around 8:00 p.m. on the above date. The switch was replaced and the Elevator was released to run at 1:00 a.m. on 1/10/2019.

Due to rainy weather, gravel was washed into the safety brake system of the hoist causing a mechanical failure around 3:30 a.m. on 1-1-2019. MSHA was notified at this time. The safeties were cleaned thoroughly and tested multiple times to verify proper function. The hoist was released to run and MSHA was contacted.

Employee had exited underground office going next door to the break room to start the day shift crew. Shortly after leaving the office the employee was found by another employee lying on the floor in front of the break room unable to remember how employee had gotten there or what had happened. Employee had several abrasions on the facial area and a cut on lip.

The Hoist was giving a Main Brake Hold Alarm. Due to oil sample,the oil in the Main Brake Thrustor was changed. The oil level was checked in the Thrustor and discovered that it was low. After adding ¾ of a gallon, the Thrustor began performing as it should. Oil plug was tightened to stop leaking and Hoist was back in operation.

Employee experienced pain in right knee while stepping up on a platform. Restricted Duty.

Elevator cable slipped on drum causing damage to cable. Cable had to be removed from service and elevator load tested.

The conduit and control wiring had pulled loose from the Mine Level Door Switch. A short circuit was caused when the wires touched which in turn caused a PLC output card fault. The PLC was reset, conduit repaired and a new door switch was installed.

Employee was scaling a rock from the back. The scaling bar slipped and they felt pain and a burning sensation in the right shoulder. Did not report to management till 5/15/17. Had dr visit on 5/19/17, MRI 5/26/17, Physical Therapy ordered 6/6/17. May continue operating mechanical scaler.

Elevator went down finding that a wiring issue was located in the door switch. Wiring was repaired and elevator returned to full service.

While cleaning rock from expansion joints in shaft a rock struck wire on the buffers knocking it loose disabling the elevator.

Crews 1&2 were riding elevator when car stopped at landing. Crew 1 exited car sending crew 2 to landing. Pushed down button that sent car into over travel locking doors. Crews were removed from car at 8:00am.

Employee tripped on a piece of metal pipe that was lying on the ground. Employee tried to catch himself and jammed wrist.

Elevator had stopped abruptly at approximately 45-50 feet from the top landing with production crew aboard. The braking system set automatically due to build up on rails.

Elevator service was disrupted for 4 hours due to a fault in the electrical main breaker. Service was restored approximately 1:30am on October 12, 2015.

During inspection of the elevator hoist a crack was found on one of the brakes shoes. System was immediately taken out of service. Repairs were made and returned to service on 3/31/2014 1:20pm.

Electrical switch on the elevator door malfunctioned. This made the elevator inoperable. Elevator became operational at approximately 6:30 AM

While performing a PM on the elevator a broken brake rod was discovered. This caused the elevator to be out of service.

Elevator was not functioning properly. This issue persisted for more than 30 minutes.

Employee was dismounting a loader when he twisted or pulled something in his lower back.

While operating a haul truck underground the employee ran over a large rock, which jarred the operator.

Employee was stepping up onto a concrete structure when his right knee gave out.

Employee was stepping down onto a conveyor belt, he braced himself with his left wrist. When he stepped down he felt pain in his left wrist.

Employee was attempting to replace a finger at the crusher. Employee rolled his ankle, causing him to smash his pinky finger between the finger and the sidewall of the crusher.

Employee was walking in heading when he slipped and fell backwards onto his shoulder.

Employee was attempting to take out slack from a ratchet strap. The employee tugged with force, and the strap came free causing employee to fall backwards onto his left shoulder.

Employee bent down to pick up a choker and felt a pop in his rt. knee.

At approximately 100 Ft. down the elevator stopped with two miners aboard. The miners were stuck for approximately 1 hour at this location.

A rod near the brake shoe of the elevator was bent. This tripped the safety on the elevator causing it to stop.

Employee bent over to hook fuel hose on the service truck, when he stood up he felt pain in his lower back.

A switch on the elevator door was not working properly. This made the door inoperable.

Limit switches were not functioning properly on the elevator. Although this impaired the elevator, the elevator could still be used to transport workers in the case of an emergency. The on-site supervisor contacted MSHA as a pre-cautionary measure. In hindsight MSHA should not have been contacted, since the elevator was operable.

Heavy rains caused water to go down the shaft and the water damaged some electrical components of the elevator.

The elevator door switch became misaligned. This issue made the elevator inoperable.

An electrical issue occurred at the override portion of the elevator. This issue made the elevator inoperable.

The elevator has been recently upgraded. An employee attempted to complete a PM of the elevator in the normal manner. With the recent upgrades this method did not work (the installer of the upgrades did not notice this issue). When he did the normal procedure, the elevator became inoperable.

The limit switch on the elevator door was not working properly. This allowed the elevator to be inoperable for a period of time.

Employee was stepping over a shaft approximately 3ft. off the ground. While doing so his foot got caught, which caused him to fall forward onto his right arm.

Employee strained his back while putting on a harness style seat belt.

The elevator, which is the main entrance to the mine was not working due to a faulty connection of electrical wires.

While removing an air filter from a powder truck, the employee cut his right palm on a frayed piece of metal.

Regular maintenance was being performed on elevator. While changing magnetic switch, it was found to be a stonger field than original. Replaced with correct switch, which took longer than expected. Returned to full service at 5:30pm.

While operating a face drill, the operator noticed flames behind her in the articulation area of the truck. The operator deployed the fire suppression system, which did not completely extingush the fire. Operator called for help.

Cleaning had been conducted on top and bottom of slope, it is believed that debris became lodged in the pulley of the elevator ropes. This allowed the ropes to become entangled and made the elevator inoperable.

The elevator door was not properly shut, which made elavator door sensor to read on and then off multiple times. This tripped the breaker on the elavator, which caused it to stop.

Emergency hoist became inoperable during shaft hoist repairs.

Mine emergency hoist became inoperable during shaft hoist maintenance.

Mine emergency hoist became inoperable during shaft hoist maintenance.

Injured was walking up stairs to J-North sizer when he felt a pop in his knee.

Hoist out of service after mechanical deficiencies identified. Order 6596688-01 allowed service to hoist as long as recommendations and requirements of the WG Spencer Engineering Inc. report received 11/25/2009 are fully met and maintained.

Breaker tripped causing loss of power to elevator.

Injured was preparing to use water hose to wash under conveyor. Injured asked another employee to turn on the supply valve. When the valve was turned on, injured lost control of the hose which struck her forehead.

Elevator door switch malfunction, caused elevator outage from 2:15pm until 8:00pm.

Injured was operating 11030 face scaler when it began to overheat. Injured notified service technician and shut the machine down. After waiting 15 minutes for the machine to cool down, injured opened radiator cap to check coolant level. When cap was opened, hot coolant sprayed, causing 2nd degree burn.

Conrols tripped and and needed to be reset.

Door switch failed causing elevator to malfunction.

Breaker tripped causing loss of power to the elevator.

The governor cable was causing the safety switch to trip on the top of the car causing the elevator to malfunction. Tightened the spring on the car and has taken care of the problem.

While in a kneeling position the employee stood up after resetting a breaker and felt a pain in his right knee. Right knee strain.

On 6/3/2008, the entire plant and mine lost electrical power from the supplier. The power outage caused loss of power to the vertical hoist at the mine, however the back diesel power kicked on immediately, and the hoist was operable again. The mine was evacuated at that time. Full power has been restored

Elevator control cable was cut while repairs to the compensating chains were performed.

Safety door switch malfunction caused interruption to elevator service for more than 30 minutes. Repairs made to switch to return elevator to service.

On 2/12/2008, the entire plant and mine lost electrical power from the supplier. The power outage caused loss of power to the vertical hoist at the mine, however the back diesel power kicked on immediately, and the hoist was operable again. The mine was evacuated at that time. Full power has been restored.

While returning to pick up truck to retrieve lunch box the employee slipped on snow and ice on the Hoist House parking lot. Employee was preparing to take a Getman underground via the slope. 2-3 stitches required.

Safety switch malfunction caused interruption to elevator service for more than 30 minutes. Tension on switch spring was adjusted to eliminate problem.

Employee was performing maintenance on feeders. While walking down the catwalk the mechanic walking in the back noticed the mechanic in front started slumping over, dropped to his knees, and fell (and/or rolled) between the top rail and mid rail of the hand rails. The victim fell to the ground 13 feet. He stated that he had blacked out. All medical test showed no abnormalities.

Employee was using grinder to prepare feeder pan for welding repairs when the grinder kicked out striking employee on right leg.

SCR of main drive motor is bad. Replacing SCR & starter on motor. Parts on site. Approx time of return to service 11/28/2005. Firing pulse generators bad & need to be rebuilt. Man hoist & emergency hoist will be manned & ready until elevator is returned to service. Spoke with the Lexington KY MSHA office reporting elevator outage on 11/21/2005 @ 7:00am.

High speed bearing went out on elevator drive. Will be out of service for 2 days until repairs are made. Man hoist as well as emergency hoist will be manned and ready 24 hours a day until elevator is returned to service. A message was left at the Lexington Kentucky MSHA office on 8/16/2005 informing of the outage.

Elevator will be down for scheduled maintenance on 5/1/2005 through 5/5/2005. Rails which the elevator car travels will be replaced. Man hoist as well as the emergency hoist will be manned and ready 24 hours a day until elevator is returned to service. A call to the Lexington Kentucky MSHA office was made on Friday Arpil 29, 2005 and spoke with Randy Dotson informing of the outage.

Due to a fault in the surge suppresor on a incoming 4160 volt line, the power to the mine elevator was interrupted at 7:20 p.m. Saturday evening (3/5/2005). Repairs were made and power restored at 9:15 a.m. Sunday morning (3/6/2005).

Employee was walking, went to grab handrail, missed handrail due to bi-focal glasses and fell into handrail cutting lip and chipping tooth.

2 EEs were in shop loading a brake drum for 631D Cat tractor onto wheel hub located on a pallet. Two boards were being used to slide drum over hub when one board slipped and brake drum landed on EE's right hand thumb.

Employee was using a utility knife to cut a piece of conveyor belting to be used as an airwall when the knife slipped cutting his right lower leg.

Whille pulling on a torque wrench tightening bolts on a crusher employee felt pain in right shoulder.

Employee was installing roof bolt in back located R/N 4 North panel. While attempting to install bolt in drill hole, the bolt got in a bind due to improper machine positioning and slipped out of spinner head causing the bolt to spring towards operator striking his forehead.

Employee was tilting Getman basket to attach a spool of cable to mounting brackets. Latching bar was in upright position and when he tilted basket the bar went past center of gravity and bar fell stiking employees left hand cutting his 2 middle fingers. 8 stitches were required.

ELECTRIC POWER KICKED OFF DUE TO INSULATOR BLOWING IN THE MAIN SUBSTATION YARD APPROXIMATELY 11:20 AM. ALL MINE PERSONNEL MET AT THE FRESH AIR BASE. POWER WAS RESTORED APPROXIMATELY 5:15 PM. N O INCIDENTS.

EE WAS DISMOUNTING A PERSONELL CARRIER UNDERGROUND, WHILE STEPPING OFF USING 3 POINT CONTACT, SPRAINED LEFT ANKLE. NO LOOSE ROCK WAS FOUND IN AREA OF INCIDENT.

EE WAS OPERATING VARIOUS HEAVY EQUIPMENT ALL DAYDRIVING THE MACHINERY TO THE WASH BAY. EE WAS IN GOOD SPIRITS AT THE END OF SHIFT & DID NOT COMPLAIN ABOUT ANY PAIN OR DISCOMFORT. ON 9/23/03 APPROX 8PM EE CONTACTED OFFICE AND INFORMED THEM HE HAD A LUMP ON HIS BACK & DUE TOBACK PAIN WOULD BE UNABLE TO RETURN TO WORK THE NEXT DAY.

HAD TO REPLACE GOVENOR CABLE ON ELEVATOR. ELEVATOR COULD RUN MANUALLY IF NEEDED.

EE WAS CLEARING OFF AN OVERLOADED BELT AND WHILEEXITING ROCK BOX STOOD UP FROM SQUATTING FELT PAIN IN LEFT KNEE. POSSIBLE TORN CARTILAGE.

STRUCK BY A FALLING ROCK. A ROCK FELL ON EE'S HEAD OFF OF THE FACE.

USING SCREW DRIVER AND TOOL SLIPPED AND HIT HAND AGAINST CHAIN. REQUIRED 2 STITCHES.

CUT/PUNCTURE/SCRA EE WAS GETTING OUT OF 11022 SCALER AND HE WAS HOLDING HIS HAND UP TO THE WINDOW TO KEEP THE DOOR FROM SLAMMING AND HE GOT A PIECE OF GLASS IN HIS HAND.

STRAIN OR INJURY BY PUMP STARTED TO SHIFT FROM FLOOR JACK AND EMPLOYEE CAUGHT TO KEEP FROM FALLING. LONGER THAN 40 HOUR RESTRICTION-LOST TIME.

REPLACING GOVENOR CABLE. TWO ESCAPEWAYS WERE STILL MAINTAINED.

STRAIN OR INJURY BY NOT WATCHING FOOTING.

EE WAS TIGHTENING BOLTS AND FELT PAIN IN LOWER BACK.

STRAIN OR INJURY OTHER WAS BRINING THE HIGHSCALER BASKET TO THE GROUND TO TALK TO EE AND THE DOWN LEVER STUCK CAUSING THE BASKET TO HIT THE GROUND JARRING EE.

CAUGHT IN, UNDER SCALING ROCK, AND THE ROCK FELL DOWN PINCHING HIS HAND AGAINST THE BASKET.

TURNED POWER OFF 1:00 PM TO REPLACE INSULATOR ON INCOMING LINE. POWER RESTORED 3:30 PM. MAINTAINED 2 ESCAPEWAYS OUT OF MINE.

EMPLOYEE AND CO-WORKER TIED A ROPE AROUND THE TIP OF THE TIRE ON 36026 FORKLIFT AND SECURED THE MAST ON 36025. AS CO-WORKER WAS PULLING THE TIRE BACK, THE ROPE BROKE AND HIT EMPLOYEE ON THE L EFT SIDE OF THE NECK AND UPPER CHEST AREA.

ELECTRIC WENT OFF AT MAIN SUBSTATION CAUSING ELEVATOR NOT TO WORK. TWO ESCAPEWAYS WERE STILL MAINTAINED.

AN SCR WHICH AIDS IN CONTROLLING THE SHAFT HOIST WENT OUT AT 7:15 PM. THE HOIST WAS BACK IN SERVICE AT 10:45 PM (SAME DATE). TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE.

A NON-INJURY, SELF PROPELLED MOBILE EQUIPMENT FIRE CAT 631D TRUCK OCCURRED UNDERGROUND AT THE A-NORTH CRUSHER ON 10-31-00. THE FIRE LASTED APPROX. 3 1/2 HRS AND WAS EXTINGUISHED BY THE MINE RE SCUE TEAM AT 6:30 A.M. ON 10-31-00.

EE STATED HE WAS TIEING THE BENCH IN AND HIS BACK STARTED TO HURT. HE STATES THAT THERE WS NOT ONE SPECIFIC ACT THAT CAUSED IT TO HURT. THIS ACCIDENT WAS REPORTABLE DUE TO THE FACT EE RECEIVED RESTRICTED DUTY FOR 3 DAYS.

A CONTROL BOARD WHICH AIDS INOPERATING THE SHAFTHOIST WENT OUT AT 5:30AM THE SHAFT HOIST WAS B3ACK IN SERVICE AT 8AM 2 OTHER MEANS OF ESCAPE WERE STILL AVAILABLE

A CONTROL CABLE FOR THE BOTTOM DOOR SWITCHES ANDOVERTRAVEL SWITCHES HAD A CUT IN THE CABLE. THE SHAFT HOIST WENT DOWN AT 7:30 AM AND WAS BACK INSERVICE AT 8:15 AM AFTER REPAIRS WERE MADE. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE.

6EE WAS WALKING DOWN ROADWAY AND SLIPPED ON HYDRALIC FLUID CAUSING HIM TO FALL AND TWIST HIS RIGHT KNEE. FIRST DAY LOST IS 8-2-00.

SUBJECT WAS WALKING TO AN ELEVATOR CARRING A TOOL BOX. WHILE WALKING THROUGH A NARROW WALKWAYHE TURNED SIDEWAYS AND FELT A POP IN HIS RIGHT KNEE.

EE WAS PULLING ON A HOIST CHAIN AND FELT A PULLING IN HIS CHEST ON THE FIRST INCIDENT AND THE SECOND INCIDENT HE WAS PULLING ON A LUG WRENCH AND FELT THE SAME SENSATION. AF6TER THE SECOND INCI DENT HE WENT TO THE DR. AND WAS DIAGNOSED WITH AHERNIA. THE TWO PRECIDING INJURIES TOOK PLACE ON 4-20 AND 5-12. THIS INJURY DID NOT BECOME A LOST TIME UNTIL 5-17 AS THIS IS THE FIRST DAY LOST

AT 11 AM THE TRAILING CABLE THAT SUPLLIES POWER TO THE SHAFT HOIST DEVELOPED MOISTURE IN THE CABLE CAUSING THE HOIST TO BE TAKEN OUT OF SERVICE. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR TO RUN AT 2 PM.

EE WAS OPERATING A CANNON ROOF BOLTER AND WAS IN THE BASKET INSTALLING A BOLT WHEN THE BOOM BROKE CAUSING THE BASKET TO FALL APPROX 10' TO THE GROUND. EE WAS TIED OFF IN THE BASKET WITH FALL P ROTECTION AND ONLY RECEIVED A CUT REQUIRING STITCHES.

EE WAS ATTEMPTING TO SPLICE WIRES TOGETHER ON THE LIGHTING SYSTEM ON 1123 FACE SCALER, WHEN HIS KNIFE SLIPPED AND CUT HIS FINGER. EE HAS 4 STITCHES WHICH MADE THIS INJURY REPORTABLE.

EMPLOYEE HAD TAKEN GUARD OF AIR COMPRESSOR ON THE POWDER TRUCK TO REPAIR A BROKEN AIR LINE. DURING THE TROUBLE SHOOTING PROCEDURE, EMPLOYEE HAD COMPRESSOR RUNNING AND STARTED TO LOOSEN THE LIN E WHEN HIS LEFT LITTLE FINGER CAME IN CONTACT WITH THE FAN. IT CUT AND BROKE THE FINGER, WHICH REQUUIRED STITCHES.

AT 0600 THE SHEAVE ON THE GOVERNOR ROPE ON THE SHAFT HOIST BROKE PREVENTING THE USE OF THE SHAFT HOIST. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR TO RUN AT 1500 .

EE WAS REMOVING CYLINDER LINERS FROM A DIESEL ENGINE WHILE POITIONING THE LINER REMOVER TOOL THE CUSTOMER'S HELPER EE INADVERTENLY ENGAGED THE TRIGGER ON AN IMPACT WRENCH, CAUSING THE INJ URY CUT LEFT RING FINGER

EE WAS DRIVING A CATAPILLAR HAUL TRUCK HEADING TO THE CRUSHER TO DUMP HIS LOAD OF ROCK. THIS EE AND ANOTHER EE TRAVELING BACK TO THE LOADER IN THE OPPOSITE DIRECTION DRIVING A CAT HAUL TRUCK I MPACTED THE LEFT CORNERS OF THE TRUCK BEDS. THE EE RECEIVED A CRACKED STERNUM. THE HAUL ROAD WAS DRY AND VISABILITY WAS GOOD. ACCIDENT OCCURRED IN A STRAIGHT DRIFT. LOST TIME ACCIDENT.

EE WAS DRIVING A CATAPILLAR HAUL TRUCK HEADING TO THE LOADER AFTER DUMPING HIS LOAD AT THE CRUSHER. THIS EE AND ANOTHER EE TRAVELING IN THE OPPOSITE DIRECTION OPERATING A CAT HAUL TRUCK IMPACT ED THE LEFT CORNERS OF THE TRUCK BEDS. THE EE RECEIVED A CRACKED KNEE CAP AND A SMALL LACERATION TO THE HEAD. THE HAUL ROAD WAS DRY AND VISABILITY WAS GOOD. ACCIDENT IN A STRAIGHT DRIFT.

EE WAS WALKING IN THE TIRE BAY WHEN HE STEPPED ON A UNEVEN PIECE OF CONCRETE FLOOR AND TURNER HIS RIGHT ANKLE. HE WAS PLACED ON 2 DAYS OF RESTRICTED DUTY BY A DR.

EE WAS OPERATING A CANNON ROOF BOLTER WHEN HE WAS DESCENDING IN THE BASKET AFTER INSTALLING BOLTS THE WRIST ASSEMBLY BROKE CAUSING THE BASKET TO CREEL UNDER THE BOOM. EE WAS TIED OFF IN THE BA SKET WITH FALL PROTECTION EQUIPMENT AND WAS ONLYTOSSED AROUND IN THE BASKET RECEIVING SOME CUTS REQUIRING STITCHES.

AT 4:00 AM ON 8-1-98 THE SHAFT HOIST WENT OUT OF SERVICE DUE TO A MALFUNCTION OF THE SCR ON THE UPLIFT CONTROL BANK. THE OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR T O RUN AT 8:30AM ON 8-1-98

AT 1905 THE SHAFT HOIST WENT OUT OF SERVICE DUE TO A MALFUNCTION OF THE SCR ON THE DC DRIVE PREVENTING USE OF THE HOIST. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLE AR TO RUN AT 0900 ON 7-7.

AT 3:30AM THE ELECTRIC POWER CABLE WHICH FEED ELECTRIC TO THE SHAFT HOIST MALFUNCTIONED PREVENTING THE USE OF THE SHAFT HOIST. TWO OTHER MEANSOF ESCAPE WERE STILL AVAILABLE. THE SHAFT HOIST WAS CLEAR TO RUN AT 6:5OAM

EE WAS DISMOUNTING A CAT HAUL TRUCK, WHEN IS FOOT SLIPPED OFF THE LADDER AND LANDED ON A ROCK TWISTING IS LEFT KNEE. THIS ACCIDENT OCCURRED IN THE 16-NORTH PARKING AREA.

EE WAS HAULING BLASTED ROCK TO THE CRUSHER WITH A CAT 631D HAUL TRUCK, WHEN AS HE WENT THRU A CROSSCUT THE RIGHT FRONT TIRE BLEW OUT CAUSING THE TRUCK TO HIT THE RIB ON THE RIGHT SIDE. HE SUBS TAINED 2 SMALL CUTS ON HIS HEAD & ONE REQUIRED 2 STITCHES.

EMPLOYEE WAS HAND SCLAING OUT OF AMADOR HIGH SCALER WHEN A LARGE SLAB 10' LONG 7' WIDE AND 24' WIDE THAT HE WAS TRYING TO SCALE OFF RIB CAME DOWN AND STRUCK OUT RIGGER OF THE MACHINE. THE MACH INE WENT DOWN ON THAT SIDE AND THE EMPLOYEE WAS THROWN OUT OF THE BASKET. HE WAS WEARING A FALL PROTECTION HARNESS AND WAS SECURED TO THE BASKET BY A 3' LANYARD.HE RECEIVED A FRACTURED SKULL

EE WAS HELPING HAND SCALE WHEN A LARGE SLAB 10' LONG, 7' WIDE AND 24' THICK, THE THEY WERE TRYING TO SCALE OFF RIB, CAME DOWN AND STRUCK THE OUTRIGGER OF THE MACHINE. THE MACHINE WENT DOWN AND EE WAS THROWN FROM BASKET. WEARING FALL PROTECTION. RECEIVED A LACERATION TO THE HEAD AND SOME BRUISING TO THE BODY. MACHINE POSITIONED PROPERLY.

A ROOF FALL OCCURRED IN 38 CROSSCUT OFF F-NORTH BENCH IN THE 2 NORTH PANEL OF THE MINE. THE FALL WAS APROX. 100'L, 30'W. IT WAS ABOUT 4'T AT IT THICKEST POINT & FELL OUT IN A LOT OF PIECES. IT DID NOT FALL ABOVE THE ANCORAGE. IT DID HOWEVERBLOCK THE PASSAGEWAY OUT. THERE WAS NO VENTALATION CHANGE SINCE THE ROOM WAS APROX 60'T. NO ONE WAS INJURED & MSHA INSPECTORS, ON REG. INSP HELP

EE HAD CLIMBED UP ON M-21 BELT COUNTERWEIGHT FRAME TO MEASURE THE BELT WHEN HE LOST HIS BALANCE AND FELL APPROX. 6 FT TO THE MINE FLOOR AND LANDED FEET FIRST ON A ROCK. HE FRACTURED HIS LEFT L EG BELOW THE KNEE. FIRST DAY LOST 10-14-97.

AT 4 AM THE MAGNETIC DOOR SWITCH MALFUNCTIONED ON THE SURFACE DOOR OF THE ELEVATOR. THIS PREVENTED THE USE OF THE ELEVATOR. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE ELEVATOR WAS CLE AR TO RUN AT 9 AM.

EE WAS WASHING ROCK FROM WALL OF CRUSHER, HE LET GO OF HOS WITH ONE HAND TO TURN VALVE OFF AND LOST CONTROL OF HOSE. WATER HOSE STRUCK EE IN NOSE, BREAKING NOSE.

THE EE WAS DISMOUNTING A TRACTOR WHEN HE TWISTED HIS LEFT ANKLE. EE WAS ON RESTRICTED DUTY FOR ONE DAY.

EE WAS DIRECTING OTHER SCALER OPERATOR AS TO WHERE TO PARK MACHINE, WHEN HE TRIPPED OVER ROCK ONFLOOR AND FELL ON HIS LEFT SHOULDER. EE FINISHEDSHIFT, THEN MISSED THE NEXT TWO SHIFTS, MAKING IT A LOST TIME ACCIDENT.

EE WAS HELPING SLIDE A BELT ROLLER THRU A HANDRAIL, WHEN THE ROLLER FLIPPED UP AND CAUGHT EE'S THUMB BETWEEN ROLLER AND HANDRAIL.

EMP WAS GUIDING A FLIGHT BAR INTO POSITION AT STAMLER CRUSHER. HAD HIS RIGHT HAND ON MAINTENANCETRUCK CABLE JUST ABOVE SHEAVE BLOCK AND TOLD TRUCK OPERATOR TO RAISE THE BAR. WHEN CABLE WINCHED UP, IT CAUGHT EMPS FINGER BETWEEN SHEAVE PULLEY AND CABLE. FINGER REQUIRED 5 STITCHES TO CLOSE WOUND.

EMPLOYEE WAS MOVING AN OLD REFRIGERATOR ON A TWO WHEE CART, HE HAD TO PULL CART OVER A DOORWAY AND STRAINED BACK MUSCLES. EMPLOYEE WAS PUT ON RESTRICTED DUTY.

EE WAS HELPING LOAD A FLIGHT BAR, WEIGHING APPRO260 LBS ONT A TRACTOR, WHEN HE FELT A BURNING] PAIN IN HIS RIGHT GROIN AREA. DR DIAGNOSED IT ASA RIGHT INGENAL HERNEA. EE DID NOT REPORT ACC UNTIL 10/01/96. EE IS PRESENTLY WORKING RESTRICTED DUTY PENDING SCHEDULING SURGERY ON 10/08/96. AND WHEN THIS ACCIDENT WILL BECOME A LOT TIME.

EMP WAS DISMOUNTING TRACTOR WHEN HE STEPPED ON ROCK AND TWISTED ANKLE.

EMP REPORTED CARPAL TUNNEL SYNDROME IN BOTH LEFTAND RIGHT HANDS ON 6-5-96. THERE IS NO KNOWN DATE OF INJURY. PAIN STARTED APPROXIMATELY SIX MONTHS PRIOR TO THIS DATE. EMP STATES THAT DR FEELS ILLNESS IS WORK RELATED.

THE EE WAS WORKING ON GEAR REDUCER, CLEANING UP TOP COVER WITHA SPRAY CAN OF BRAKE CLEANER WHEN A MIST OF IT GOT IN HIS EYES.

THE GOVERNOR ON SHAFT HOIST MALFUNCTIONED CAUSING SHAFT HOIST TO BE OUT OF SERVICE FOR LONGER THAN 30 MINUTES. BACK IN SERVICE AT 9:30 A.M.

A ROCK OF UNKNOWN SIZE FELL APPROX 10' ONTO THE BASKER OF #1112 HIGH BACK SCALER. EMPLOYEE WAS TOSSED AROUND IN BASKET AND BRUISED RIGHT SHOULDER AND ARM.

A ROCK OF UNKNOWN SIZE FELL APPROX 10' ONTO THE BASKER OF #1112 HIGH BACK SCALER STRIKING EMPLOYEE ON ARM, HEAD AND LEGS. EMPLOYEE HAD SMALL CUTON HEAD, NOT REQUIRING STITCHES AND SOME ABRASIO NS ON ARMS AND LEGS.

EE WAS COMING OUT OF A BUILDINGIN THE MINE & STEPPED ON A CONCRETE BLOCK. THE BLOCK TURNED AS HESTEPPED ON IT. HE FELL ON HIS LEFT SHOULDER.

EE WAS IN THE PROCESS OF RESETTING THE FIRE SYSTEM IN THE TRANSFER BUILDING. WHILE CHANGING A PIPE IN THE FIRE SYSTEM BUILDING, HE REACHED AROUND THE PIPE AND SUDDENLY FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE WAS ROLLING UP GREASE HOSE END, IT FLIPPED UP AND HIT HIS TOOTH.

BOLT OF LIGHTNING STRUCK INSULATOR ON THE 69,000 VOLT TOWER AT PLANT SUBSTATION. THIS INSULATOR WAS ONT EH LOAD SIDE OF THE OILCIRCUIT BREAKER. LOSS OF POWER CAUSED LOSS OF BOTH HOISTS. POW ER TO THE MINE WAS RESTORED AT POER 1805 HOURS POWER TO PLANT WAS RESTORED AT 1905 HOURS. THE GASOLINE POWERED EMERGENCY HOIST WS OPERABLE DURING THIS PERIOD. COMPANY POLIY WAS FOLLOWED ND

AT APPROX 1900 HOURS, THE CONTACTS OF THE ELEVATOR MAIN LINE CONTACTOR BURNED UP. THIS PREVENTEDTHE USE OF THE ELEVATOR. TWO OTHER MEANS OF ESCAPE WERE STILL AVAILABLE. THE ELEVATOR WAS CLEAR TO RUN AT 2145 HOURS.

EE WAS CUTTING VICTAULIC SEAL I NT WO WITH A UT ILITY KNIFE KNIFE SLIPPED & CUT H IS RT LEG A NEW BLADE HAD JUST BEEN INSTALLED IN THE KNIFE

EMPLOYEE WAS SERVICING FACE DRILL AND WAS STEPPING DOWN FROM REAR OF DRILL. MUD ON HER SHOES ALLOWED HER FOOTING TO SLIP FROM LADDER AT ABOUT 30" ABOVE GROUND LEVEL. SHE LANDED ON HER LEFT FOO T ONTO CLEAN, LEVEL GROUND. SHE WAS USING BOTH HANDS TO DESCEND.

AS THE ELEVATOR WAS DESCENDING INTO THE MINE WITH THE ELECTRICIAN ONLY ON BOARD THE MAIN POWER CABLE WHIPPED INTO A PIECE OF METAL ON THE TOP OF THE ELEVATOR THIS CUT THE CABLE FOR A LENGTH OF ABOUT 3FT ALONG THE LENGTH OF THE CABLE DAMAGING SOME OF THE WIRES INSIDE THE ELEVATOR WAS RETURNED TO AUTOMATIC SERVICE ON 03-04-94 AT 0945 & MANUAL SERVICE ON 03-03-94 AT 2015

AN OVAL SHAPED ROOF FALL MEASURING ABOUT 100' LONG X 35' WIDE IN WIDEST SPOT VARYING DEPTH FROM 2' TO 6', OCCURED IN #16 X-CUT BETWEEN H NORTH AND G NORTH IN 4 NORTH PANE. 6' RESIN ROOF BOLTS ARE USED IN THIS MINE. THE MAJORITY OF THE FALL IS 2' THICK. IT WAS NOT DETERMINED THAT NY OF THE FALL WAS AT THE ANCHORAGE ZONE UNTIL 1-4-94. NO INJURIES OCCURRED.

EMPLOYEE WAS GETTING DOWN FROM 631 CAT TRUCK CARRYING A FUEL NOZZLE. HE WAS HOLDING HAND RAIL WITH LEFT HAND AND NEAR THE BOTTOM, HE JUMPED DOWN TO FLAT FLOOR SURFACE. HIS RIGHT FOOT GAVE WAY AFTER CONTACTING THE FLOOR.

EE HAD JUST HUNG COER ALLS UP ON THE HANGING BASKET IN THE CHANGE ROOM AND WAS GETTING OTHER THINGS OUT OF HIS LOCKER WHEN THE BASKET CHAIN ANCHOR PULLED OUT OF ANCHOR RAIL. BECAUSE THE NUT CO MING OFF, IT LET THE BASKET FALL ABOUT 8 FT AND HIT EE ON THE RIGHT SIDE OF THE FOREHEAD.

CONTRACTOR EE WAS TYING REBAR STEEL FOR CONCRETE POUR AT I SOUTH AND 8 CROSSCUT. A FIST SIZE PIECE OF LOOSE ROCK HIT HARD HAT OF ONE CONTRACTOR EE WITH NO INJURY AND THEN FELL ONTO LEFT SHOULD ER OF THE OTHER CONTRACTOR EE END OF REBAR STRUCK RIB KNOCKING ROCK LOOSE.
