The employee was changing a 440 volt starter. Turned power back on to check starter after replaced. Checked the on/off button and determined new starter worked. He did not shut power down again, elected to hook up motor leads and in the process came in contact with the relay which in affect pulled starter into position causing flash back injuring him.

EE WAS TURNING SAND PUMP WITH A PIPE WRENCH. WHILE TURNING THE SHAFT GOT CAUGHT BETWEEN THE WRENCH AND THE PUMP BASE.
