While taking off a fuel filter on a D6T dozer s/n MH700652, operator dislocated right middle finger requiring it to be reset at urgent care.

Over-exertion in lifting objects

While changing cutting edges on Cat 980G loader, employee improperly lifted end bit and felt a pop in left elbow. Treatment required x ray at emergency and an MRI scheduled for 6-29-18. Restricted to no more than 10 lbs.

Employee was prying a chunk of frozen material from the top side of the screen with a 3' pry bar when it slipped and pinched his left ring finger between the pry bar and the screen hopper box. The weather conditions were below freezing with mixed snow showers.

EE started to climb up access ladder to get in loader during rainstorm. there was mud on bottom rung of ladder foot slipped of ladder striking the ground and twisting.

WHILE REMOVING TRACK PAD BOLTS, THE 1"" TO 3/4"" REDUCED SNAPPED. THE TWO EESWERE USING A 1"" BREAKER BAR WITH EXTENSION WHEN THE REDUCER SNAPPED. BOTH EES FELL, ONE ON TOP OF THE OTHER.
