While changing cutting edges on Cat 980G loader, employee improperly lifted end bit and felt a pop in left elbow. Treatment required x ray at emergency and an MRI scheduled for 6-29-18. Restricted to no more than 10 lbs.

Employee was prying a chunk of frozen material from the top side of the screen with a 3' pry bar when it slipped and pinched his left ring finger between the pry bar and the screen hopper box. The weather conditions were below freezing with mixed snow showers.

WHILE HAVING COFFEE AND WAITING FOR STARTING TIME I OBSERVED EMPLOYEE RUBBING LEFT ARM AND IT APPEARED HE WAS HAVING TROUBLE BREATHING. I BELIEVE THAT EMPLOYEE WAS LAY DOWN, AND CALLED 911. EMPLOYEE WAS TAKEN TO HOSPITAL BY AMBULANCE AT OR AAORUND 7:55 AM.
EE WAS REPAIRING A HYDRAULIC LEAK ON HIS EQUIPMENT. AS EE STEPPED DOWN OFF OF THE ACCESS LADDER AND TURNED, HE FELT SOMETHING HAPPEN TO HIS LEFT KNEE. THERE WERE NO OBSTACLES OR LOOSE MATERIAL S OR DEFECTIVE EQUIPMENT THAT CONTRIBUTED TO THIS INJURY.

EE WAS REINSTALLING STEERING LINKAGE WHEN HE PUTHIS FINGER INTO THE BOLT HOLE TO CHECK FOR ALINGMENT. THE LINKAGE SLIPPED DOWN VIRTICALY PINCHING FINGER IN BOLT HOLE.
WHILE USING THE HAND CRANK FOR THE CONE CRUSHER HYDROSET. EMPLOYEES HAND SLIPPED OFF OF THE CRANK AND WITH HYDRAULIC PRESSURE AGAINST THE PUMP THE CRANK HANDLE ROTATED BACKWARDS STRIKING THE EMPLOYEE IN THE ARM. REQUIRING X-RAYS AT CLINIC.
A LARGE ROCK HAD BECOME JAMMED BETWEEN THE SYTRON FEEDER AND THE CONVEYOR RECIEVING BOX SIDES. WHILE TRYING TO FREE THE ROCK, IT BECAME UNJAMMED MOVED BACKWARDS SUDDENLY AND PINCHING EMPLOYEE 'S FINGERS BETWEEN THE ROCK AND THE FRONT OF THE FEEDER.
EMPLOYEE WAS USING A PRY BAR TO LINE UP 4CYL ENGINE TO INSTALL A NEW BELT IN THE MOTOR MOUNT THE PRY BAR SLPPED & PINCHED FINGER BETWEEN BAR & HYD PUMP EMPLOYEE WAS NOT BOTHERED BY IXCIDENT UN TIL 04-29-95 WHEN HIS FINGER NAIL STARTED TO TURN BLACK & HAD AN ODOR

WHILE REMOVING TRACK BOTTOM ROLLERS THE BAR THE EMPLOYEE WAS HOLDING TO SECURE THE MULTIPLYER WRENCH SLIPPED, PINCHING EMPLOYEES RIGHT THUMB, BETWEEN THE BAR AAND TRACK SIDE FRAME.
EMPLOYEE WAS REPLACING THE CUTTING EDGE ON THE BUCKET. INSTEAD OF PLACING THE CUTTING EDGE ON BLOCKA NDLOWERING THE BUCKE TTO THE EDGE/EMPLOYEE USED A FLOOR JACK TO RAISE THE REPLACEMENT EDGE . THE EDGE WAS NOT PROPERLY BALANCED AND SLIPPED OFF OF THE JACK HITTING EMPLOYEE IN LEFT THIGH.
EMPLOYEE REPORTED TO WORK 6-6-94 AND COMPLAINED HE HAD SOMETHING IN HIS EYE. 6-7-94 EMPLOYEE WAS SENT TO DOCTOR AND FOUND A PIECE OF RUST IN EYE. WE ASSUME HE GOT THIS WHILE GREASING THE DRIVE SHAFT ON HIS MACHINE THE PRECEEDING SATURDAY.

WHILE LOWERING A SUBMERSIBLE PUMP INTO THE WATERWITH A CRANE. I ATTEMPTED TO LIVE UP THE TWO DISCHARGE PIPES. WHEN I GRABED THE PIPE HOOKED UP TO THE PUMP. IT BECAME UNLOGGED AND CAUGHT MY HAND BETWEEN THE TWO PIPE FLANGES.
EE WAS PULLING ON AN AIR HOSE AND BACKING UP WHEN EE BACKED UP INTO A SHOP HEATER (NOT IN USE) AND FELL BACKWARDS FALLING ONTO HEAD AND NECK.
WHILE WORKING OR PARTS STORAGE BARN, EMPLOYEE STEPPED ON A WOOD SHINGLE WITH A NAIL IN IT. THE NAIL PUNCTURED BOTH THIS WORK BOOT AND FOOT EMPLOYEE RECEIVED A TETNUS SHOT AND ANTIBIDIS.
EMPLOYEE WAS REPAIRING A SCREEN ON A POWERESCREEN MKI PORTABLE PLANT. HE WAS USING THE STOCKPILE UNDER THE SCREEN TO STAND ON. HE SLIPPED AND ROLLED 10 FT TO THE GROUND, STRAINING OR PULLING A NECK MUSCLE.
