Fall from ladders

EE states, ""Getting down off sand screw wind blow the ladder and I fell to the ground and hit elbow and shoulder.""

Over-exertion in lifting objects

Injured employee and two other employees were lifting a 5 foot by 5 foot by 1/4 inch thick piece of steel (approx 275 lb) onto a truck. Injured employee felt something strained in his shoulder. Doctor diagnosed a rotator cuff tear on June 7.

Employee states, ""I slipped going down the stairway."" He is on work restrictions: No lifting greater than 35 lbs, limited work which involves kneeling, limited work which involves squatting.

Struck against a moving object

Employee has been operating a front end loader in the pit between 8 and 10 hours, 5 days a week. On July 21 he said he had a sore back and needed to see a doctor.

Employee was trying to drive out a drive chain link pin. The pin shattered and a piece of it cut the top of his right hand - seven stitches.

4/18/08, EE states, he was welding a leaned forward and hit his head on a spray bar, jamming his neck. 5/9/08 EE requested to see a physician because his neck was still sore. Physicaian returned EE to work with no restrictions and with a referral for physical therapy.

EE was doing maintenance on screen tower. Quoting EE's accident report: Putting tools in work box, on plant, had hand on edge of box, reached for a tool and the lid closed on hand (wind).

Employee was installing 1'x 4' urethane screen panels, pounding them in place with a hand sledge hammer. He struck himself in the hand with a mis-directed blow.

Employee was removing a flat rock from the head box over the screen. As he rolled it to the edge of the screen it shifted to one side and pinched his hand between the rock and the frame of the screen. Impairment: lifting with right hand limited to 2 lb.

USING A JACK AND BLOCKS TO RAISE A SAND STACKER, EE'S SHOULDER BECAME SORE. HE THOUGHT IT MIGHT GET BETTER AFTER USING IT, BUT IT KEPT HURTING SO HE WENT TO THE DOCTOR TO HAVE IT CHECKED. EE WAS OFF 1 DAY AND RETURNED WITH RESTRICTIONS OF LEFT-HANDED-WORK-ONLY UNTIL HE HAS AN MRI.

AFTER GETTING OUT OF THE SKID STEER HE WAS RUNNING - HE SLIPPED ON SOME ICE AND FELL AGAINST THE TIRE HURTING HIS NECK.

WHILE STANDING OR WALKING ABOUT 20' FROM DRIVE OVER HOPPER, A HAUL TRUCK PULLING ONTO HOPPER DROVE OVER A ROCK SHOOTING IT FROM UNDER TIRE HITTING EE'S LEG (R) BREAKING IT.

WHILE WORKING UNDER A 88-B DRAGLINE CRANE, HELPING TO LOOSEN SOME BOLTS, THE WRENCH BROKE. HE STUMBLED BACK TWISTING HIS RIGHT ANKLE CAUSING A BAD FRACTURE.

SLIPPED AND FELL ABOUT 5' WHILE REPLACING A CONVEYOR BELT. TRIED TO CATCH HIMSELF AND PULLED SHOULDER OUT LANDING ON GROUND.
