Over-exertion in wielding or throwing objects

Supervisor was trying to dislodge a snow obstruction in a transfer chute. Was swinging a sledgehammer overhead and felt a pull in Right Shoulder.

Over-exertion in lifting objects

The employee lifted a heavy shaft alone instead of waiting for help. The employee went to the doctor and was diagnosed for a Groin strain.

Contact with electrical current

Supervisor was changing out a blown fuse. Failed to deenergize power source. When removing hand from source, touched the fuse and received an electric shock.

Over-exertion in lifting objects

Dewatering Screens went down, we had to shovel off dewatering screens, belts, troughs etc. Could feel my arm shoulder and back getting sore but kept shoveling just took breaks in between. The pain and tightness continued to get worse days after. Finally I got to the point I couldn't move my neck felt like a really bad pinched nerve. Made Dr. Appt. Nov 2, 2022.

Individual was in man lift with harness on and tied off was loading totes of collector in main tank. When reaching through hand rail on man lift EE felt a pop in left bicep/Shoulder. Individual went to the Doctor this morning and was diagnosed with most likely a pulled bicep tendon. 1 week of rest with pain meds if pain persists after 1 week than X-Rays and further Diagnoses.

Over-exertion in lifting objects

The individual was shoveling sand when EE felt something in EE's bicep and shoulder area. EE worked on 10/12 day shift called in after the shift said EE would not be in on 10/13 for work. EE was going to the doctor.

Struck against a moving object

Miner was operating a wheel loader moving material in the pit for several hours. Upon exiting the machine the miner said back was stiff and sore. The condition worsened over the holiday weekend and the miner went to the walk in clinic on September 4th. Doctor advised they should be excused from work for September 4th. Employee did not report injury until September 4th.

Over-exertion in lifting objects

Employee was lifting and moving rolls of drainboard for application to the new Truck Dump wall. As employee lifted one (approx. 53 lbs.), felt a pull/tear in right arm.

Struck by flying object

Contractor was removing bushing from lower bucket arms of a wheel loader. The Contractor was observing progress from behind the loader bucket. When the bushing came free it bounced off the opposite bucket arm, the tire and struck the Contractor above the right eye lid causing a cut requiring 2 stitches.

Miner was operating a wheel loader in pit loading sand into hopper. After filling bucket at the bank and backing away he had double vision. Miner believes it occurred when the bucket contacted the bank.

EE INFORMED ME ON 10/13/04 HE HAD INJURED HIS ELBOW WHILE LOADING A BOAT. THE EE WAS WALKING ALONG A CONVEYOR WHEN HE MISSED A STEP & FELL. hIS ELBOW CONTACTED THE CONVEYOR FRAME AS HE FELL. HE FINISHED HIS SHIFT & WENT TO DR AFTERWARDS. THE ELBOW IS STILL SWOLLEN & HE WILL SEEK FUTHER MEDICAL TREATMENT.

EE WAS PRYING WITH A PIPE TO CLOSE A RECLAIM GATE. THE PIPE SLIPPED, PINCHING HIS THUMB BETWEEN THE PIPE AND THE RECLAIM GATE.

EMPLOYEE WAS CUTTING A PIECE OF CONVEYOR BELTING KNIFE SLIPPED LACERATING HIS LEFT THUMB. THE CUT WAS ABOUT 1"" LONG. IT REQUIRED STITCHES TO CLOSE.

Struck by falling object

EE WAS ASSISTING REMOVING A PIECE OF STEEL(ABOUT 100 LBS). THE STEEL WAS BEING LOWERED TO THE GROUND. IT SLIPPED, BOUNCED OFF A PIECE OF CONVEYOR BELTING AND STRUCK THE EMPLOYEE IN THE FOREARM . THE IMPACT CAUSED A LACERATION ON THE FOREARM WHICH REQUIRED STITCHES TO CLOSE.

EE STRAINED MUSCLES IN HIS BACK WHEN HE ATTEMPTED TO OPEN A OVERHEAD DOOR. THE DOOR WAS MOTORIZED, BUT BOTTOM EDGE HAD BECOME FROZEN TO THE GROUND AND WOULDN'T OPEN UNDER POWER OF MOTOR. WHEN LIFTING ON DOOR, EE STRAINED BACK MUSCLES.
