THE CONE & CONVEOYOR SHUT OFF. CREW CLEANED CONVEYOR & TAIL PULLEY OFF. FOREMAN ASKED IF EVERYONE WAS READY TO START UP. PLANT MAN WHEN TO HIS STATION & FOREMAN TO HIS. FOREMAN MADE EYE CONTACT WITH INJURED. THE PLANT MAN STARTED HIS CONVEYOR & FOREMAN STARTED HIS CONVEYOR THEN TURNED TO MAKE SURE ALL CONVEYORS WERE MOVING. DID NOT SEE INJURED SO SHUT DOWN EQUIPMENT.

TWO EMPLOYEES WERE CHANGING AIR CLEANER ON GENERATOR. AIR CLEANER WAS STUCK. WHEN EE APPLIED MORE FORCE, THE AIR CLEANER BROKE LOOSE AT SEAL. HIS ARM SLIPPED, RESULTING IN HIS ELBOW ON RIGHT A RM HITTING WALL OF TOOL VAN.
