EE was being lifted on a platform with a forklift to loosen cement stuck in Portland screw. When forklift driver lowered the lift, the platform jerked and the EE lost his balance and fell to his knees. Has pain in his knees

He was on top of the Q Line reclaim hopper next to the weigh pod. He was putting the top back on the weight pod, tightening the belts. The wrench slipped. He lost balance & fell backwards. Pulled muscle in middle/lower left side of back.

Busted bag stuck between conveyors. Employee tried to free it - hand got caught. Took off tips of middle and rings fingers, skin across top of hand and two fingers was pulled off. Tips reattached and skin graft to repair hand.
