EE was driving a forklift when EE became unconscious. EE crashed the forklift into the passenger side of a semi truck. At this point, responders were notified. An AED was used and CPR was performed. The EE was revived and taken by ambulance to a local hospital.

EE was carrying pallets sheets on level ground (concrete floor) did not see pallet sitting on floor and tripped over it. EE went to doctor on 1/7/16 came back to work with no restrictions. EE went to doctor for followup on 1/15/16 came back with restrictions since that day EE has worked 3 days and been on vacation the other 2 days.

While removing fan from heat exchanger a bolt broke causing the fan housing to come loose unexpectedly and swing into EE right leg, pinning it between fan and beam.

Maintenance man was painting a railing and he lifted approximately 50 lb guard with the help of another employee. After these two activities he reported pain in his abdomen.

Miner was using a bobcat skid steer to clean clay/debris off the railcar tracks in the plant. The bobcat came to a sudden stop when the buck caught a joint section on the tracks. The miner was secured in the bobcat with the seat restraints and the force from the sudden stop caused pain in his lower back

Three employees were installing a shive (16"" OD x 6 belt) on hammer mill drive. Shive slipped off of taper lock and smashed employees finger between shive and shaft causing a fracture on tip of left middle finger and lasceration.

Employee was putting a 100 # bag of product on a pallet when he slipped on loose material and strained his back. This happened prior to the Thanksgiving holiday and had hoped a couple of days rest would take care of it. When he returned on Monday after being off for 4 days he still had some pain. I sent him to the Dr. who thinks it is a strain and we have put him on light duty.

EE was cleaning up in draft chamber area when hot clay dust went down into his right boot causing 2nd degree burns on foot.

Employee was walking across lot when he stepped on a piece of coal. The piece of coal rolled and employee lost balance and fell and landed on the corner of a bobcat bucket. He was sent to the Dr. and the Dr. thinks he has a contusion.

EE was helping another EE empty a tub of dust weighing approx. 300 lbs, when he felt a pain in his low back. He went for a MRI on 3/20/06, we are waiting on the results of these tests to determine level of injury.

THE COAL MILL WAS TURNED OFF AND THE NATURAL GAS PILOT LIGHT WAS TURNED ON. AFTER SEVERAL MINUTES THE EMPLOYEE'S WENT DOWN TO THE BASEMENT TO DO AN INSPECTION OF THE MILL. AFTER LOOSENING THE BOLTS ON 2 DOORS, THE TOP DOOR WAS TAKEN OFF FIRST AND THEN THE BOTTOM. AS THE BOTTOM DOOR WAS TAKEN OFF THE EXPLOSION OCCURRED.

Ee reported that in the past few weeks he has noticed a lump on his stomach. He requested that we send him to the Dr, that he thinks he may have a hernia. He thinks it may have happened over time when he's been pushing & pulling heavy objects. He went to Dr on 10/6/04 & was diagnosed with ventral hernia. Can not specify a specific date or time when injury occurred.

EMPLOYEE WAS CLIMBING DOWN FROM FRONT END LOADER WHEN HE LOST HIS FOOTING ON A STEP AND FELL. HE HAS A CONTUSION ON HIS BACK. DOCTOR HAS TAKEN HIM OFF WORK FOR 3 DAYS AND WILL SEE HIM AGAIN ON MONDAY 9/27/04.

Diagnosed on 4-19-04 with ankle sprain that requires surgery. ee twisted ankle 8-15-03. ee stepped on pallet edge and twisted ankle. went to co. dr. dr. said it was a ankle sprain, it would take time to heal. ee reported jan 04 that ankle still hurt occasionally. sent to another dr. who also said would heal 4-19-04 dr tested and said ligiment needs to be repaired.
