Employee reported that EE's shoulder was hurting on 3/22/24 from an unknown incident on 3/13/24 sent to the Work Comp Doctor on 3/25/24.

Miner was using Hi-Vac, adjusted hose and hurt back. - This may end up becoming a non work related injury due to degeneration.

EE lost footing and hit shin on an edge of a concrete block.

EE was sliding a Propane cylinder into rack and it caught on a lip jamming EE's shoulder.

EE ascended pallet as EE stepped up EE felt something pull in EE's left knee.

Employee slipped in snow while holding a propane tank. Hurt shoulder.

EE slipped while descending stairs. Stairs were clean and dry. EE was not carrying anything. Handrail is in place and was used.

EE slipped on a muddy hill and fell on EE's shoulder.

EE strained back moving outrigger plates for crane.

A chute that was leaned against handrail slid/fell. The flange of the chute contacted the EE foot just beyond the steel toe.

Health Van notified company of employee's inability to wear a respirator. Abnormal X-Ray.

A threshold shift was detected in the employees annual hearing test.

Miner tripped on a bag strap, fell and injured shoulder.

Miner was shoveling, hurt back.

Employee slipped when getting off front end loader. Employee caught self with left arm straining a muscle in shoulder.

EE was driving a forklift when EE became unconscious. EE crashed the forklift into the passenger side of a semi truck. At this point, responders were notified. An AED was used and CPR was performed. The EE was revived and taken by ambulance to a local hospital.

EE cut hand on piece of metal while buffing it.

EE was carrying pallets sheets on level ground (concrete floor) did not see pallet sitting on floor and tripped over it. EE went to doctor on 1/7/16 came back to work with no restrictions. EE went to doctor for followup on 1/15/16 came back with restrictions since that day EE has worked 3 days and been on vacation the other 2 days.

While removing fan from heat exchanger a bolt broke causing the fan housing to come loose unexpectedly and swing into EE right leg, pinning it between fan and beam.

Maintenance man was painting a railing and he lifted approximately 50 lb guard with the help of another employee. After these two activities he reported pain in his abdomen.

EE was grinding metal in the kiln.

Two maintenance men were moving several, varying length 8" I beams around the shop and to the work area approximately 100 feet away.

Repairing water hose. Hand slipped and cut palm of hand on hose clamp.

Miner was using a bobcat skid steer to clean clay/debris off the railcar tracks in the plant. The bobcat came to a sudden stop when the buck caught a joint section on the tracks. The miner was secured in the bobcat with the seat restraints and the force from the sudden stop caused pain in his lower back

Start having back pain while bagging.

Using a 3 inch hose over shoulder vacuuming over a wall. Vacuum suction stopped jerked hose back, lost balance and fell to the floor.

Using bobcat to haul a bucket of chain. Bucket of chain fell back into cab of bobcat on right knee.

Walking up steps from maintenance shop by hardinge mill area fell backwards. Held onto guardrail and pulled right shoulder during the fall.

Bagging at the east sacker, turned right ankle.

As the employee stepped into the compressor room, left foot slipped off step and all his weight landed on left leg.

Three employees were installing a shive (16" OD x 6 belt) on hammer mill drive. Shive slipped off of taper lock and smashed employees finger between shive and shaft causing a fracture on tip of left middle finger and lasceration.

EE was using a cutting torch, cutting conduit off basewall and the conduit sprung loose and struck EE i the right leg.

Employee and another employee were trying to move the oversized return chute in a very amall area. When they were trying to lift the heavy object over a handrail he felt a pain in right shoulder.

EE was unstrapping steel, when bundle shifted and injured EE's right thumb.

Employee was bagging 100 pound bags and placing them on a pallet. After bagging for a while his low back was hurting.

Slipped on ice and fell down.

While walking down ramp into kiln basement, employee slipped on ice/snow and fell onto back.

EE stepped onto platform to add oil to equipment, when he hit his head on another piece of equipment.

Employee was putting a 100 # bag of product on a pallet when he slipped on loose material and strained his back. This happened prior to the Thanksgiving holiday and had hoped a couple of days rest would take care of it. When he returned on Monday after being off for 4 days he still had some pain. I sent him to the Dr. who thinks it is a strain and we have put him on light duty.

EE was cleaning up in draft chamber area when hot clay dust went down into his right boot causing 2nd degree burns on foot.

Employee was walking across lot when he stepped on a piece of coal. The piece of coal rolled and employee lost balance and fell and landed on the corner of a bobcat bucket. He was sent to the Dr. and the Dr. thinks he has a contusion.

EE was helping another EE empty a tub of dust weighing approx. 300 lbs, when he felt a pain in his low back. He went for a MRI on 3/20/06, we are waiting on the results of these tests to determine level of injury.

THE COAL MILL WAS TURNED OFF AND THE NATURAL GAS PILOT LIGHT WAS TURNED ON. AFTER SEVERAL MINUTES THE EMPLOYEE'S WENT DOWN TO THE BASEMENT TO DO AN INSPECTION OF THE MILL. AFTER LOOSENING THE BOLTS ON 2 DOORS, THE TOP DOOR WAS TAKEN OFF FIRST AND THEN THE BOTTOM. AS THE BOTTOM DOOR WAS TAKEN OFF THE EXPLOSION OCCURRED.

EMPLOYEE REPORTED ON 5/3/2005 HE HAD PAIN IN LEFT SHOULDER, LEFT HAND AND RIGHT ELBOW. HE CANNOT SPECIFY AN ACTIVITY WHICH CAUSED HIS INJURY. HE HAS BEEN SENT TO THE DOCTOR. DR. IS TREATING IT AS A STRAIN. HE HAS BEEN SENT TO A HAND SPECIALIST FOR A FOLLOW-UP.

Employee was filling and placing 100# bags on a pallet in the grinding plant area when he felt and heard a pop in his upper back.

EMPLOYEE REPORTED ON 2/14/2005 HE HAD PAIN IN WRIST AFTER USING GUN TO SHOOT RING IN KILN. SENT EMPLOYEE TO Dr. HAD SAID WRIST WAS FINE WAS SENT TO WORK W/O RESTRICTIONS. 5/4/05 EMPLOYEE REPORTED THAT WRIST WAS HURTING AGAIN. I SENT HIM BACK TO Dr. WHO HAS REFERRED HIM TO HAND SPECIALIST.

Ee reported that in the past few weeks he has noticed a lump on his stomach. He requested that we send him to the Dr, that he thinks he may have a hernia. He thinks it may have happened over time when he's been pushing & pulling heavy objects. He went to Dr on 10/6/04 & was diagnosed with ventral hernia. Can not specify a specific date or time when injury occurred.

EMPLOYEE WAS CLIMBING DOWN FROM FRONT END LOADER WHEN HE LOST HIS FOOTING ON A STEP AND FELL. HE HAS A CONTUSION ON HIS BACK. DOCTOR HAS TAKEN HIM OFF WORK FOR 3 DAYS AND WILL SEE HIM AGAIN ON MONDAY 9/27/04.

EMPLOYEE COMPLAINED OF SORE LEFT KNEE AFTER KNEELING TO DO MAINTENANCE WORK. HE SAW A DOCTOR WHO WANTS HIM TO HAVE AN MRI. EMPLOYEE HAS BEEN DIAGNOSED WITH DEGENERATIVE JOINT DISEASE AND A STRAIN TO LEFT KNEE.

EE WAS POKING MATERIAL IN CHAMBER (KILN) WHEN CHUNK OF MATERIAL FELL ONTO BAR, CAUSING BAR TO HIT SIDE OF EE'S FACE, BREAKING HIS JAW.

Employee was tightening bolt on coal mill when he lost his footing and had onset of low back pain.

Diagnosed on 4-19-04 with ankle sprain that requires surgery. ee twisted ankle 8-15-03. ee stepped on pallet edge and twisted ankle. went to co. dr. dr. said it was a ankle sprain, it would take time to heal. ee reported jan 04 that ankle still hurt occasionally. sent to another dr. who also said would heal 4-19-04 dr tested and said ligiment needs to be repaired.

DATE OF INJURY IS THE DIAGNOSIS DATE- REPETITIVE MOTION INJURY- LEFT WRIST CARPAL TUNNEL. EE THROWS UP TO 100# BAGS ONTO PALLETS. HE COMPLAINS OF WRIST PAIN WHEN DOING THIS. DR. DIAGNOSED HIM ON 9/26/03 WITH MILD CARPAL TUNNEL.

WAS HELPING COWORKER PICK UP MOTOR & FELT PULL IN BACK.

SETTING UP FORMS TO POUR CONCRETE HE SLIPPED IN MUD AND FELL BACKWARDS. CAUGHT SELF.

WHILE CHECKING COAL MILL OIL ERUPTED & BURNED RIGHT ARM.

WAS ADDING OIL WHEN HOT OIL "BURPED" OUT ON TO ARM.

TRYING TO LOOSEN BOLT ON SCREEN.

WHILE USING A DRILL, DRILL BIT CAUGHT AND TWISTED RIGHT HAND.

SWITCHING CLAY ELEVATOR FROM WEST TO EAST BIN WHEN FELT PAIN IN SHOULDER (LEVER FROZEN).

COMPLAINS OF WRIST PAIN.

CLEANING OUT 8 X 20 BIN. BOTTOM STEP BROKE - BRUISED RIB & ARM.

SACKING MATERIAL OUT WHEN MATERIAL BLEW UNDER SAFETY GLASSES INTO EYE.

HOOK ON E-Z LIFT BROKE, VAC HOSE FELL.

OPERATING JACK HAMMERING LEDGE-PIECE BROKE LOST BALANCE-STILL HAD TRIGGER SQUEEZED AND JACK HAMMERED ON LEFT FOOT.

LIFTING GRATE TO BUILD CATWALK AND HURT LEFT ARM.

WHILE BAGGING PAIN IN FOOT REPORTED AS INFORMATION ONLY NO TREATMENT REQUIRED ASK FOR TREATMENT 8/9/00

1ST EE TURNED ON GAS. 2ND EE LIT 1ST 2 BURNERS AND THEN WENT TO LIGHT LAST BURNER. HE PICKED UP PAPER THAT WAS USED TO LIGHT BURNERS. AS HE WAS WALKING AWAY 1ST EXPLOSION OCCURRED. APPROX 30 S EC LATER 2ND EXPLOSION. NO INJURIES. EST $100 DAMAGE.

INJURED WAS WORKING ON A SCREEN. HE WAS LAYING ON THE TOP DECK PUTTING IN A WEAR PLATE. HE WAS TWISTING WHILE PUSHING A BOLT IN SAME TIME HE WASLAYING ON A PIECE OF OVERSIZE BAUXITE. WHILE PUS HING IT HE HEARD A POP IN HIS RIBS.

GOING INTO BIN TO SWEEP BIN STEPS BROKE & HE FELL INTO BIN

EMPLOYEE WAS UTILIZING RAMP FOR EQUIPMENT. RAMP HAD LOSE FINISHED PRODUCT ON IT. EMPLOYEE WAS WALKING DOWN THE RAMP AT AN ANGLE. HE WAS NOT UTILIZING HANDRAIL. HE LOST HIS FOOTING WHEN THE FIN ISHED PRODUCT SLID OUT FROM UNDER HIM.

INJURED CAME OUT FROM UNDER CATWALK AND STOOD UP STRAIGHT AND BUMPED LEFT SHOULDER ON CONTROL BOX.

HURT LOWER BACK WHILE BAGGING.

FELL AND CAUGHT FINGER BETWEEN 2 PIECES OF PIPE BREAKING THE FINGER.

CUT RIGHT HAND ON AUGER SHEAVE.

SLIPPED ON ICE, FELL AND INJURED BACK.

HAND SLIPPED OFF BAG STRAINING MUSCLES & CAUSINGBONE SPURS

EMPLOYEE WAS WALKING THRU THE KILN MAINTENANCE SHOP. HE TRIPPED ON A CHAIN BY THE OVERHEAD DOOR AND KILN SHOP CAUSING HIM TO TWIST AND BRUISE HIS LEFT ANKLE.

WHILE BAGGING,EMPL SLIPPED & FELL ON PALLET LIFT CAUSING CONTUSION OF LEFT FLANK AREA.

I NAME S BACK HURT AFTER HE TOOK SHOWER THE NIGHT BEFORE AT 400 AM HARDLY TURN IN BED OR GET UP. WENT TO HOSPITAL. OFF 1-4 THRU 1-6 AND RETURN TO WORK 1-9 THRU 1-19. X RAYS SHOW DEFECT THE LUMBAR SPINE HE HAD BEEN BORN WITH IT. GO TO THERAPY AND GO BACK TO DR 1-23.

PERFORMING MAINTENANCE ON ABBY DRILL USING A GRINDING MACHINE AND GOT A METAL CHIP IN HIS RIGHT EYE.

I.NAME DIRECTLY CAUSED INJURY TO I.NAME A UTILITY WORKER, BY DUMPIN A PARTIAL 930 LOADER BUCKET OF WATER ON OR BESIDE I.NAME WHILE HE WAS CLEANING THE STEPS UNDER THE STACKER BELT AT THE ROTA ROTARY KILN. THIS CAUSED HIM TO BE PUSHED BACK INTO A CONCRETE WALL, BRUISING HIS LEFT SHOULDER.I.NAME WAS SENT TO THE COMPANY DOCTOR ON 7-8-85. THIS IS A LOST TIME ACCIDENT WITH A LOSE OF 8 T

WHILE OPERATING THE LOADER, THE MACHINE HIT A LARGE PIECE OF MATERIAL WHILE THE OPERATOR WAS DUMPING THE BUCKET. THIS CAUSED THE LOADER TO JERK AND CAUSED PAIN IN THE BACK, NUMBNESS IN FINGER OF LEFT HAND, STIFF NECK AND PAIN IN LEFT SIDE.
