A 8,000 gallon diesel fuel tanker with an estimated 1,000 gallons or less in it when it caught on fire. A gasoline powered engine ran the fuel pump on the tanker. The fire dept. was called and the fire was extinguished in approximately 45-50 minutes. The investigation was unable to reveal the exact cause of the fire.

EE WAS USING A SIX FOOT CHEATER BAR AND WRENCH TO CHANGE A TIRE, THEY WERE PULLING ON BAR AND WRENCH WHEN IT SLIPPED AND EE FELL BACK AND HIT THE TRUCK CUTTING HIS RIGHT EYE.
THEY WERE REPLACING HAMMERS ON THE SPINNER. THEY WERE DRIVING NEW HAMMERS IN AND HIS RIGHT HAND GOT CAUGHT IN THE SPINNER AND MASHED HIS HAND, CAUSING STITCHES TO BE TAKNE IN THE THUMB, BACK M IDDLE FINGER, CAUSING 15-20 ABRASIONS ON HIS HAND AND MOST OF THE SKIN TO BE TAKNE OFF IN SPOTS OF THE FINGERS.

EE WAS REMOVING A COUPLING FROM DRILL STEEL OUR EE WAS HITTING IT AN THE TRACK TO LOOSEN IT, HE GOT HIS FINGER CAUGHT BETWEEN THE COUPLING AND THE TRACK IT CUT HIS RIGHT CENTER FINGER AND PART OF THE TENDON.
EMPLOYEE WAS COMING DOWN FROM PUTTING A BELT ON THE IMPACTOR ON SCREENING PLANT. AS HE WAS COMING DOWN THE LADDER HIS FOOT SLIPPED AND HE CAUGHT HIS FOOT AND LEG AND TWISTED HIS KNEE CAUSING HIM TO DAMAGE THE CRUCIATE LIGAMENTS. THE PART OF THE BODY THAT WAS HURT WAS THE RIGHT KNEE.
EE WAS PULLING OUT FUEL BARREL TRAILER, AS DOING THIS THE TONGUE ON THE TRAILER PULLED OFF AND DROPPED ON THE RIGHT FOOT OF THE EE AND MASHED IT, HE IS NOW ON CRUTCHES.
