Employee was adjusting forks on forklift to lift a skiff. They used a sledgehammer to help move the fork closer together. When they struck the fork, the fork slid and pinched the tip of right thumb in a pinch point. Received stitches.
Employee was climbing a fixed ladder to enter the 14 ball mill hopper to perform maintenance. The ladder has a hinge with a pin near the top. The metal hinge broke. Employee held onto top part of ladder and fell, landing on EE's feet and then falling onto EE's rear. EE has a hair line compression fracture in the L1 and is on restricted duty.

A 3 inch diameter poly water pipe located in a plugged sump shifted, causing the pipe to flex while the employee was inspecting the plugged sump, this resulted in the pipe to contact the employees chin resulting in a laceration which required stitches.
A 793 C haul truck was ascending a ramp loaded and lost traction. EE attempted to back up down the hill and contacted a berm which overturned truck. A bucket truck was used to help the EE exit the truck. The EE was entrapped in the cab more than 30 minutes. EE was suspended pending outcome of investigation.
On drill 304, drill crew was tripping out tooling and preparing to remove the drill bit. The bit had been welded to the collar and the driller was using a handheld grinder to cut through the welds. As the grinder broke thru the welds, the grinding disc failed and a fragment struck a the driller on the finger causing a laceration.
Employee was helping change a shovel shank. When the support wedge came loose, EE's left hand was under the wedge and contacted EE's index and middle finger causing minor fracture on both fingers.
Employee was rodding #4 rod mill. The rod hung up and employee attempted to push the rod with the heel of palm and caught right pinky between the rod and the roller. Fractured pinky finger.
Employee was aligning a pump and felt a strain in their left shoulder. Doctor placed them on restrictions on 8/12/25 pending a MRI.
Removing belly pan and it slipped when it came loose causing a laceration on index finger requiring stitches to finger.
Employees has been diagnosed with ""trigger finger"" on EE's left index finger. EE had surgical repair on 7/30/2025
Employee removed rear tire from service truck to work on brake caliper. EE stood up and backed into the tire EE removed. EE lost EE's balance and in the process stepped down on EE's left foot which resulted in a lower leg fracture.
Employee was pressure washing the inside of a centrifuge, when their foot slipped, and they cut their chin. Anti biotics were given for preventative measures. No stitches were required.
Employee was exiting the drill and when stepping down off the boarding ladder employee rolled right ankle. Sprained ankle. 3 days of restricted duty.
Employee was diagnosed with carpal tunnel of both hands. Had surgical repair on 1/22/25.

Employee helping with change out of Loader teeth. Tooth shifted in transport tote and caught tip of left pinky finger resulting in sutures.
Employee performing maintenance on dust collector #11 and placed finger in pinch point causing a laceration on left index finger resulting in 6 stitches.
Haul truck was traveling down a ramp, it began to slide and tipped over onto the driver's side. The miner was wearing seatbelt. EE states EE was not unconscious. However, a witness believes EE may have been unconscious. Employee had no visible injuries and was checked by off-site EMTs and found no injuries. MSHA suggests we complete this form.
Employee removed their hard hat and safety glasses in the break room and felt something fall into their eye. They flushed it but later that evening they still felt some irritation. Went to eye doctor in morning and removed a particle and gave prescription eye drops.
Employee was cutting a rubber liner for a cyclone. While cutting the rubber liner with a utility knife, EE's hand slipped and EE hit EE's thigh with the knife causing a laceration which required 3 stitches.
Employee has numbness in right hand and was diagnosed with carpal tunnel. They had surgery on 7/15/24 on their right hand.
Employee was stepping off bulldozer and missed the step causing employee to strike right knee on the push arm. Sore and swollen knee. Placed on restricted duty.
Crusher operator was taking a drain pipe down as it was plugged. When EE uncoupled the pipe coupling to remove the pipe, the pipe slid and contacted EE's left ring finger resulting in a laceration and fracture of tip of finger.
Employee was boarding EE's haul truck and as EE stepped up to the ladder EE felt a pop in EE's right knee. Originally it was a strained knee but was later determined to be a torn meniscus. EE had it repaired on 6/18/24 and is currently off work.
Walking up an incline on the 18 belt to perform ground checks and slipped on walkway. Went to have knee checked (had previous surgery) on 3/13/24. We were informed on 3/20/24 that employee received a cortisone shot. No restrictions. Medical treatment
Employee was removing a fitting off of an air hose with a 18"" crescent wrench. While applying pressure to loosen the fitting EE felt pain in EE's right elbow. Was put restricted duty until heals.
Employee aligning motor in lower pump house. While using a sledge hammer they missed where they intended to strike, resulting in left ring finger being contacted between the sledge handle and the mounting plate. Required stitches and the tip of finger is fractured. Currently on light duty.

Employee was ripping with a D8 dozer and while backing up and looking behind them, the dozer caught a frost slab and bounced down causing pain in their neck. Was put on restricted duty for 3 shifts.
Employee was hosing in the mill basement and twisted right knee. On 11/17/23, we received Doctor notes indicating light duty. Employee has been placed on light duty until further diagnostics and possible repair.
Employee was leaving the line up room at start of shift, foot stepped off sidewalk and lost employee's balance, fell down and put right arm out when falling. Was given a full release but turned out to have a tear in shoulder resulting in a surgery on 11/9/23.
Lifting a check valve shell with the over head crane and was applying tension to the choker with EE's left hand when the choker shifted and pinched EE's left middle finger resulting in a laceration requiring 2 stitches.
Employee had set up a spool of wire on two jack stands to pull electrical cable and when walking by the jack, EE caught foot and tripped. EE reached for the reel holder to catch self to prevent EE from falling. EE reported EE strained arm. The strain was later determined to be a bicep tear which EE had repaired on 3/13/23.
Employee was helping change out a cooler assembly on a crusher oil cooler. A sharp edge on the frame contacted EE's right index finger causing a laceration that required stitches (4). Lack of gloves were a factor.
Employee was carrying some mill parts into the mill and their foot slipped. Employee did not fall down, but while attempting to maintain balance their back tightened up and reported a sore back. Employee is currently on restricted duty.
Employee was moving liners to install on a conveyor belt. Felt a strain in the abdomen. Originally, thought it was a pulled muscle. Had a hernia repaired on 2/17/23. Returned to work on restricted duty on 2/20/23.

Employee was assisting a co-worker to roll over an engine mount when they felt a strain in right elbow. Originally diagnosed as a strain. Later it was diagnosed as a tear which required surgery.
Mechanic was in the engine compartment of a Haul Truck trouble shooting a sensor. They stepped on a valve cover and their foot slipped. As they reached out to steady themselves, they hit their right hand on the exhaust pipe causing a fracture to the right pinky metacarpal. No stitches or surgery required. Employee has been placed on light duty.
Employee was pushing on a bundle of rods for the rod mill and the bundle shifted which pinched employee's right pinky finger. Resulted in a fractured finger tip.
Employee was walking across flotation floor and lost balance, falling backwards and striking back of head resulting in laceration requiring stitches.
Employee was pulling an oxy/acetylene cart to work station. Employee slipped and lost balance losing control of the cart. The cart fell back and struck employee's right shin causing a fracture.
Employee was walking into fuel bay and tripped, then stumbled and caught self on the table while contacting EE's cheek with EE's fist creating a chipped tooth. The chipped tooth required dental repair.
Employee was changing a belt on a pump. While pulling on the belt, they pulled left middle finger into the sheave cutting the finger resulting in 2 stitches.
Employee was helping change feeder chains in the course ore feeders and pinched the tip of left pinky resulting in 2 stitches. Employee was placed on restricted duty until wound heals.

Truck Driver was helping move some trailing cable. As driver was walking to the cable in the Continental Pit, they slipped on the snow and landed with left arm out stretched and shoulder was sore. Doctor gave them restrictions until they can have a MRI.
While completing a tire repair on a 240 ton haul truck EE was lifting a lock ring up to re-install on the wheel, while lifting the lock ring EE felt pain in right lower back. Technician did not report the injury until 10/6/21, upon which time a complete investigation was initiated.
Employee has shoulder/upper right arm issue from 27 plus years as a mechanic. Workers compensation informed employer on 10/12/21 that the condition is compensable. Currently on restricted duty.
On 9/16/21, Employee was disassembling a turning motor on a rod mill when the mill job was complete. When lifting the motor, it shifted and struck the outside of EE's left foot. Diagnosed with a bruise at the time. On 11/18, employee informed us EE had a CT scan done and there is a hairline fracture on left metatarsal that was not seen on 9/16/21.
Employee and partner were lifting a worn liner and in the process of setting the liner down, the employees left pinky finger was under the liner. EE fractured the tip of EE's left pinky finger. Placed on restricted duty.
Employee was replacing floor decking. Employee's partner was using a torch and employee was using a pry bar. Employee placed left hand on a hot spot where employee's partner had recently cut. Second degree burn. Employee was placed on light duty.
Employee stepped on a grinding ball and strained back. Currently on light duty.

Employee was working on a pipe and pulled on a pipe hanger, lost balance and left arm scrapped on some angle iron resulting in a laceration which required 3 stitches.
Employee has a repetitive illness issue with left elbow. Was given restrictions and received some treatment on the elbow.

Employee struck a power line with the bed of haul truck. EE exited the truck 43 minutes later after it was confirmed the power was deenergized. A citation was issued (9471229) for failure to immediately report on 2/7/2020. The citation was contested and upheld.
Employee was using a grinder to remove an old pipe. The cutting wheel got bound up, bounced back and cut EE left palm causing a laceration which required 8 stitches.
Two laborers were pulling a pallet out of a pickup truck, the pallet edge caught on the plastic molding of the tail gate, causing a 24 inch blind flange to slide forward and pinch the right pinkie finger of one laborer between the top of the pallet and the blind flange. The injured employee received 4 stitches to the top of their right pinkie finger.
Employee was helping feed 5/8"" tripper cable on the shovel when EE felt EE's shoulder lock up. 4 months later EE's doctor diagnosed a small tear in EE's right shoulder which EE had repaired.
Employee was cutting a band that was securing a bundle of rods for the rod mill. When EE cut the band, the band struck EE on the left forearm causing a laceration which required 6 stitches.
Doing a walk-around at beginning of shift and slipped on the ice, landing on elbow and shoulder. Eventually received an MRI and Doctor issued restrictions on 5/2/19. Employee currently on Restrictive Duty.
Employee has epicondylitis of right elbow caused by repetitive trauma from being a mobile equipment mechanic. Had surgical repair 1/21/19.

Employee disconnected a hydraulic hose and tucked that hose behind other hydraulic hoses. As employee was working the hydraulic hose slipped out and hit left ear lobe requiring sutures.
Employee was installing new impeller on a pump when right hand was pinched requiring three stitches.

Employee was rebuilding a GIW pump. EE and partner were removing the back plate from the shaft when EE placed right hand between the back plate and the pump causing a pinch point resulting in stitches to the tip of EE's fingers on right hand.
Employee was working outside of mill and placed hand in pinch point of cart resulting in laceration of finger requiring 3 stitches.
Employee was holding a piece of plastic while partner cut the plastic with a circular saw. A piece of plastic bounced under safety glasses and scratched employee's eye. Employee was given a prescription eye drop. Returned to work that day.
Employee was unrolling a tarp on a train car and went to loosen a ratchet strap, while doing so EE released the hand crank which struck EE in the lip. EE received 3 stitches.
Employee was changing retract cable on shovel and cable struck employee on the shoulder resulting in one day of restricted duty.
Employee was boarding Haul Truck and reached up to grab the hand holds to begin ascent up the stair case and felt a pop in right shoulder. EE is currently on restricted duty.
Employee had just adjusted impeller on a pump and caught finger between belt and sheave causing a laceration to tip of middle finger on left hand. Received stitches to tip of finger.
Employee collecting data from monitoring well and dropped cover/lid on right pinky finger. Resulted in two stitches.

Employee was installing a rock knocker on a haul truck at MRI in Butte, MT. Using a boom lift to access the work area. After finishing task EE started to boom in toward the unit. During the rotation of the basket, while looking over the control guard, the employee's head/neck became caught in-between the bed box and control guard.

Employee was removing a weir bar and felt a pull in his left shoulder. Sought medical treatment a few days later and was given anti-inflammatory meds. On 6/18 he received a shot in the shoulder.

Employee was climbing up the 3-step ladder to access the 793C Cat haul truck and felt a pull in his right shoulder. Has been diagnosed with a shoulder strain and given restrictions.
Employee was helping move cable to set up the shovel and while lifting the cable, felt his right knee pop. He recently had a non-occupational right knee surgery earlier this year. He was given restrictions and physical therapy. Did not miss work.

Employee had a fatal heart attack at the mine. Heart attack was from natural causes.
Employee claims his right shoulder is sore from over-use. Has been a tire man for 40 years. Being treated with physical therapy and cortisone shots. Employee did not have an injury. We were notified on 1/23/14 that Worker's Comp was accepting the claim.
Employee walked across basement floor and slipped. Fell back and struck back of head. Laceration with stitches required. Returned to work the following day with restrictions.
Employee bumped a beam and dust fell down in his eyes. Rubbing of his eyes created a scratched cornea. Eye Doctor prescribed eye drops.
Replacing liner in rod mill. Liner was laying on rods and employee with partner was rotating the liner to align liner with bolt holes. While handling the liner, pinched tip of two fingers between liner and rods. Resulted in fracture to tip of fingers.
While changing pump barrel on primary pump, was adjusting sling and pump barrel shifted catching thumb between barrel and base. Resulted in fracture to right thumb.

Employee was washing down the floor when he backed up and tripped causing him to fall. Employee received a laceration to his lower right arm which required stitches.
Had an employee reaming in drill rods to the bottom of the drill hole, when he thought the drill rods were at the bottom the employee unchucked the head. Which caused the water swivel to go down and pinch his hand in between the swivel and the head.The helper then locked out and tagged out the rig.The two then drove to hospital to have his hand looked at.
Employee changed oil and fuel filter on Komatsu dozer and was descending dozer when he mis-stepped and fell backwards to the ground landing on his rear and back.
After hosing down floor in crusher building, employee slipped and fell to floor. Injured knee and back. Returned to work light duty.
Moving a diffuser in a bag house and the decking shifted causing the employee to slip/fall resulting in scrapes and bruising under arms. No medical treatment however, employee placed on restricted duty.

EE was unloading lime from trailers. While doing so he had lime enter the top of his gloves. The reaction of sweat and lime caused burns to his wrists. He reported the incident by phone late in the afternoon. He described the injuries as minor and denied need for medical attention. The next morning his left wrist was red and appeared infected. I sent him to get medical attention.
Changing ropes on shovel and when employee loosened rope from drum, the rope pushed his right shoulder back. Felt no pain at time. Next day his arm was sore. Diagnosed with a strained shoulder. Placed on restricted duty.
Employee and his partner lifted a crowd sheave and there was a sharp edge on the piece of equipment which lacerated the palm of his right hand. Employee received stitches in his palm.
Employee was cutting a rope with a pocket knife. He was cutting towards himself and the knife punctured his left knee. Puncture wound required cleaning and stitches. Employee placed on restricted duty.
After working on 8/10,employee complained of left elbow problems on 8/12/2011. Was seen by Doctors and was given restrictions for possible tendonitis in the elbow on 8/30/2011.
Employee was greasing a shovel when a crack formed in a valve causing grease to enter the skin on his right index finger. Medical treatment was required and employee placed on restricted duty.
Employee informed supervisor that he was not feeling well and that he had bad heartburn. Employee was taken to the Emergency Room. The employee passed away later in the day while under care at the hospital.
Employee was placing scrap into bin and caught his finger between bin and scrap. Received 3 stitches on middle finger.
Cutting a piece of sheet metal and pulled sheet across saw horse to position. Felt pain in low back. Was prescribed physical therapy.

Employee was reaching for a sling which was frozen in the ground. When he pulled on the sling, he felt a pop in his left arm. He sought medical attention on 3/25 and was diagnosed with a bicep tear on 3/29. Has not missed work at this time. Placed on restricted duty.
The victim had his hand crushed when the supervisor mistakenly thought he was signaled to raise the drill pipe. When victim notified the driller to stop the accent, the sudden deceleration caused the pipe to fall out of pipe sling coming down and crushing the victim's hand between pipe and the hog trough.
Employee was walking to parking lot and slipped while crossing the road. Strained his right knee. Was placed on restricted duty for three shifts.
While operating the crane, the employee turned off the heater disconnect switch and when he pulled down on the switch his hand contacted part of the disconnect box resulting in a laceration on his hand. He recieved 7 stitches on his left hand.
Employee had a cortizone shot in each hand on 2/8/11 to treat possible carpal tunnel. He informed us on 2/22/11 that he recieved the shots.

Employee was using the over head crane and was attempting to hook a sling. He pushed the suspended crane hook into position with his left hand and felt a pinch in his left shoulder. Dr. recommended physical therapy and released him back to work.
Employee was installing a new impeller on a pump. When he was pulling on the belts he caught his fingers in the belt and the large sheave. His middle right finger was fractured. Returned to work the next day on light duty.
Employee was loosening a bolt on the drill. When completed, he stepped down from the platform and did not notice a metal box behind him. When he stepped down, he contacted the corner of the box on his lower left rib and his body weight against the corner caused a hairline fracture in his lower rib. He is restricted in work.
Employee was throwing away an empty 30 gallon grease drum. When he lifted the drum into the dumpster, he caught his finger between the drum and the dumpster causing a laceration to his middle finger resulting in 5 stitches.
Employee was walking up stairs and missed the first step. He fell against a wall and hit his left shoulder. The following day he wanted to seek medical treatment. A hairline fracture was noted on his upper arm. Restricted duty.
Was using a pry bar to pry off old rubber liner in pump. Pry bar slipped and pinched right 4th finger. On 4/6/10 employee went to see doctor. Small fracture at the tip of finger was found.
Changing liners in rod mill and after finishing a row noticed tightness and mild pain in his lower back. EE has been placed on light duty.
Using an impact wrench and socket broke causing a cut on the employee's right palm. Required 4 stitches. Returned to work the following day.
Removing hydraulic hose on the Secondary crusher. The hose had a kink and when they uncoupled the hose, hydraulic oil sprayed in the employees' face. He was evaluated at the clinic. Eye was flushed, no material was imbedded. An antibiotic for preventative purposes was prescribed.
The employees' partner was grinding close by and some particles got in his eyes. The following day he thought he might still have something in his right eye. Evaluated at clinic and no particle found. An antibiotic for preventative purposes was prescribed.
Removing bolts with an impact wrench and felt a pain in lower back. Has been given full duty, however he did receive epidural steroid injections several months later.
Employee claims to have carpal tunnel in both hands. Mostly in the right. Employee had surgery on right hand on 1/5/2009. Employee is a millwright.

Employee was changing a quick connect coupling on a water hose and his hand slipped. His hand contacted a metal railing which resulted in a laceration to the right ring finger. He recieved 5 stitches on the tip of his finger.
Employee was helping two Maintenance employees install a new pin in a hydrualic ram. While alinging the pin, the employee lost his balance and he pinched the tip of the right ring finger between the ram and mounting bracket. He recieved two stitches on the top of the finger. Returned to work on the same day.
Employee was lifting a door cover to free a rock and when he closed the cover door, his right pinky finger was pinched between a lifting pad eye and the door resulting in a fractured right pinky finger. He returned to work the following day on restricted duty.
Employee was working on an air conditioner on a D8 Dozer. He was kneeling and looking in the engine compartment. When he stood up he could not straighten his left knee. While relining the ball mill, employee laid his right hand on the liner handler and caught the tip of his pinky finger between sliding cart and the handler. The tip of his pinky need stitches. Employee returned to work the following day.
Employee was in the process of lifting a bag of calcium chloride and he had pain in his lower back. He has a previously herniated disc in his lower back. He did not return to his next shift.

Employee was walking from one building to another when he slipped on some snow and ice. Employee fell and landed on his right hip. Employee did not report for next scheduled shift.
Employee was attempting to connect two pipes together while working from a crane manbasket. His hand was on the rail of the manbasket and pinched the tip of his left index finger on the pipe. Two stitches were required.
EE was changing hoist cables on a Bucyrus 495 HD shovel when he slipped and fell approximately 3 feet. As employee fell, he tried breaking his fall with his left hand causing a small fracture in his left wrist.
Employee was helping clean an office by moving, carrying and placing boxes in a pickup truck. That afternoon while working at his desk he felt tightness in his back. Employee felt that he had overworked his back. Over the weekend the employee stated that pain and paralysis persisted. On Monday morning the employee sought chiropractic treatment.
Washing test tubes in lab, one broke, set aside and later cut right hand on broken glass. Received 3 stitches.
Employee was cutting on a pump shaft on a work bench with an oxy/acet torch when he heard a pop. He attempted to extinguish the fire but could not. He evacuated the building and called the fire department. The fire department recommended the gas bottles burn off. Possibly a leak in the hose or slag created a leak.
Employee was sitting in and riding on a bus to his work location. He was resting his arm on top of his lunch box when the bus drove over a rough spot in the haul road causing the employee to bounce in his seat. Employee's elbow then hit his lunch box causing a small hairline fracture of his right shoulder's ball socket. Employee returned to work with limited duty.
Employee was helping his partner loosen the nuts on a pump. His partner put a cheater bar on a pipe wrench and the employee stood up and contacted the bar on his forehead resulting in a laceration requiring 3 stitches.
Employee was tested for carpal tunnel and has carpal tunnel syndrome in the right hand. He had surgery on 6/5/08.
As employee was replacing a worn out shovel tooth he twisted his back straining his lower back. The employee was placed on restriced duty.
employee was carrying sample boxes on hand dolly up stairs and slipped. Received contusion to right elbow. Employee was placed on restricted duty the next day. Employee did not lose any time from work.
While Secondary crusher number three was down for maintenance, an employee was inspecting the rings of the crusher for an oil leak. While inspecting the rings a rock rolled down the slide and struck his hard hat resulting in a laceration on top of the head. He received five stitches. Employee returned to work and finished his scheduled shift.
Employee was removing old conduit with a four inch hand held grinder. While attempting to adjust the guard he contacted the grinding wheel. This resulted in a laceration to the right index finger. Employee received three stitches. Employee did not miss work.

Employee was exiting his personal vehicle in the parking lot before shift started. He placed his left leg on the ground and while exiting caught his right foot on the floor board causing his right knee to pop. At this point the Doctor believes it is strained. He was placed on restricted duty. He did not miss any work.
Employee was accessing the splitter box and slipped. He was holding a beam to steady himself and his weight pulled on his left ring finger. Intially it was diagnosed as a strained tendon of the finger. On 5/15/08, minor surgery was performed on the tendon. Surgery was done on his off-rotation and he will return restricted duty on 5/22/08.
Contractor driver was tarping load of moly for shipment. During course of tarping the cargo the driver caught his feet in the tarp, lost his balance, fell off flatbed trailer deck to the ground, landing on his left hip. Driver transported to hospital. Driver did have 5 fractures to left hip area.
While installing a new crusher liner on the crusher mantle, employee placed his left hand between the crusher and the mantle to verify the tolerance distance. The employee was holding the overhead crane remote in his right hand and accidentally pressed the control button which shifted the crusher liner resulting in a broken left index finger with lacerations.
Employee was changing belts on a sand pump. One of the belts was twisted. As he untwisted the belt he caught his finger in the pinch point between the sheaves and the belt. The skin on the tip of his left little finger was lacerated and removed. Stitches were required to repair the tip of the finger. He returned to work on his next scheduled shift on 6/1/07.
Employee was using an impact wrench to remove nuts from the ball mill bolts. He was bent over and when he reached to his left he felt his right knee pop. Doctor felt it was just a strain on first visit. It was later diagnosed as a torn meniscus. Employee had his knee scoped on 7/9/07 for repair. First shift missed was 7/9.
Employee was exiting a D8 Cat dozer and as he stepped down to the ground, his left knee twisted resulting in a tendon tear in the knee. The employee stated he may have slipped when he stepped on the toe step located on the pushing arm. There was no snow or mud on the equipment. The employee has previously torn his right knee twice.
Employee was reporting for work and walking through the parking lot when he slipped and fell on his right hip. The fall resulted in a fracture in the upper femur. 6"" of snow fell during the night and a Bobcat was removing snow in the parking lot. Employee was walking down the path that the Bobcat had removed when he slipped. Employee intended to retire on 2/15/07.

Employee using a 9"" grinder, grinder touched the ground and bounced up contacting his left wrist. Guard was in place. Grinder caused a small laceration which required a few stitches. Employee returned to work the same day.
While driving into work on the access road to the employee parking lot, the employee lost control of his personal vehicle and ran his vehicle into the embankment next to the road. The employee received a laceration on the right side of his head requiring stitches. The employee was treated and returned to work the same shift.
Employee using a utility knife to cut tie wraps off of cable. Cut his left little finger with knife requiring five stitches.
Employee was handling 12"" HDPE. Lining up two pieces of pipe, to weld pipe. Placed hand between the two pieces of pipe as the loader was positioning the pipe. Pinched 3rd and 4th tip of fingers on left hand between the pipe. Required stitches on tip of finger.
Employee was helping stack railroad timbers and strained his back. Employee did not return to work the following day.
Employee was in the process of removing a victaulic clamp from a pipe. The pipe contained air pressure when the clamp was removed. The employee recieved a small laceration on the right middle finger resulting in two stitches. The employee did not miss any work.
Employee was working around an HDPE pipe fusion machine and caught his lower legs in a pinch point. Employee received a swelled left knee and a contusion to the left leg. He has been taken off work until the contusion reduces.
Employee had descended the ladder on shovel 820 and was attempting to slide the ladder back into place. The employee grabbed the ladder with his right hand and attempted to push the ladder back into place. When he exerted force against the ladder, he strained his right bicep. The doctor placed him on restricted duty. Employee has not missed any shifts.
Employee was performing clean-up next to 7 belt and was pushing a wheelbarrow under two support beams. After walking under one support beam he struck his head on the second beam. No objective medical findings. Employee was placed on restricted duty. Employee did not miss work.
Employee, dismantling old electrical wiring in ceiling, while descending the ladder caught his left ear on a piece of wire that was previously used to hold a suspended ceiling. The wire caused a laceration to his ear resulting in stitches to the ear. No restrictions or days lost.
Loading 12"" flanges into pickup bed and caught left little finger between flanges. Resulted in laceration that required stitches. No work missed. No restricted work.
Employee was using a wrench to loosen a nut on a Cat Dozer. While applying downward pressure to break the nut, the nut broke causing his hand to strike his tool box. He recieved a laceration to his finger requiring stitches. Employee did not miss any work.
Employee was changing a cutting edge on a patrol. As he was moving the cutting edge, he hit the end of the blade on the patrol. He felt a pop in his right elbow. It was diagnosed as a sprain. The employee returned to regular duties however, elbow pain persisted over the next 8 months. Doctors opted for surgery and found the tendon to be torn. Surgery was 12/14/06.
Employee was working on a ball mill and using an impact wrench. Set wrench on mill shroud and was bending over when the wrench fell off shroud and hit employee in lower back. Employee worked for 9 days and then reported he was having back pain. Doctor placed employee on restricted duty on 3/20/06. Employee has not missed any work.
Employee was fueling the water truck and slipped resulting in a twisted left ankle. He was given restrictions to keep weight off of foot. Employee did not miss any work. Employee returned to work that shift. He was restricted for 7 shifts.

Employee was shoveling dirt on the 7 belt in a small area and felt pain in his lower back. Employee sought medical treatment and has been put on light duty as of 12/23.
Employee was lowering rods for rod mill onto the rod handling deck. The rods shifted when they were set on the rod handler and the employee's finger was caught between two rods resulting in a laceration to the finger tip and fractured the tip of the finger. Employee returned to work the next day on light duty.
Employee was installing a shaker arm on the dust collector. In the process of installing new pins. Lined the shaker arm up with the pin hole and was about to insert the new pin. Left index finger in the pin hole and employee let go of the shaker arm causing a laceration to the tip of the finger. Employee returned to work on 12/2/05 with restrictions. Did not miss any work days
Employee had completed night shift and was in the electrical office talking to cross-shift when he collapsed in the office. CPR was started, EMS was called, and employee was transported to the hospital. The employee was pronounced dead shortly after arriving at the hospital from a heart condition.
Employee was cutting a 6"" x 6"" plate off of the shovel bucket and used a pry bar to remove the plate after cutting. When the plate came off the bucket, it struck employee on the nose causing a laceration which required stitches.
Employee was changing feed head liners on rod mill #3 and reached forward to lift the tool attachment. Employee strained his back. Employee was given work restrictions until back strain subsides.
Employee was in the process of greasing mgna-torque on shovel 818 with grease gun. He pinched his finger between the handle and tube causing a laceration to the left middle finger requiring sutures.
Employee was packing a trip cable up onto the shovel when he felt his back pop. Employee has been working with back pain and has now gone for surgery on 5/9/06 for a bulged disc. The employee has missed time as of 5/9/06.
Employee was loosening a nut from a bolt and the wrench slipped. He felt a pop in his left shoulder. Employee thought his shoulder was just strained and continued to work. He has been diagnosed with a torn rotator cuff and had sugery on 10/17/05 making it reportable. His first shift missed was 10/18/05.
On 7/17/05 employee was putting plastic pipes together using a come-along and choker. When putting tension on come-along, choker slipped and caught his hand between the pipes.
Employee was adding oil to compressor. He took off the cap and oil blew back and covered employee. He went to hospital 7/12/05 for treatment. He received medical attention because foreign matter got in his right eye, making this reportable.
Employee over exerted himself while cranking and pulling down on the rod mill charger. He felt a sharp pain in his lower abdomen. He was taken to medical care where it was diagnosed as a groin strain. Employee missed his next shift making it a reportable.
The employee has been experiencing numbness in both hands. The exact nature of the ailment is unknown. On June 16th,2005 surgery was conducted for carpal tunnel on both wrists. A fax from workers comp on June 20th,2005 indicates the acceptance under the Workers Compensation Occupational Disease Act making this reportable.
A sudden gust of wind caused material (12' aluminum boat) to spin around and break off from the crane hook. The wind floated the boat in the air 20' and struck employee. He was struck in the back and the right side. On 5/25/05 his right leg was found to be fractured.
While putting the 1-1/2"" hose back on the water truck, employee struck his right hand on the discharge pump rail. This caused swelling to the right hand. On June 1st, 2005 the injury was diagnosed as a fracture, making the incident reportable. No days away from work. This report was found not to have been filed with MSHA until 7/1/05, so we are reporting it now.
While changing clip on conveyor belt, employee's right hand was lacerated. He was holding the bottom half of the clip, when the drill bit struck his hand. The bit tore the skin. Employee went to Express care and received 3 sutures and returned to work with no restrictions.
While organizing cable storage shed, EE moved a 60# shackle at chest hight to another hook. He felt pain in navel area. Taken in for medical care and returned to work. On May 3rd a surgical physcian confirmed an operational umbilical hernia on employee. His operation in scheduled for May 10th,2005
On 4/16 EE had filled water truck & turned to walk away. His boot stuck in grate; his body turned but lower part of leg didn't. EE noticed pain & swelling. Was taken to ER & released to work, but was working with discomfort. Diagnostic scoping of left knee was done 6/29 during his 7 days off. On 7/5 employee reported to work with restrictions making this incident reportable.
While climbing a fixed ladder on the side of the drill house, ladder and safety cage broke causing EE to fall to the ground. First aid was given on-site along with emergency care. EE was transported to hospital where he received sutures for lacerations. X-rays were negative but does have soft tissue damage.
Employee was removing a panel from the Westinghouse breaker,when he felt a twinge to his right groin. During the weekend twinge started to get a little more sore. Doctor's report of 3/24/05 confirms that employee has triple hernia's and surgery is scheduled in April. He has not lost any work at this time.
EE cut the pad of the right thumb while removing a hydraulic filter from the inside screen of the hydraulic oil filter container. Employee was not aware that the screen flange was very sharp and he was not wearing gloves. The laceration required 3 sutures after work.
EE was walking back to a piece of equipment when EE tripped. The area was next to the standards of the electric shovel. Footing was level with no large obstructions. When EE fell left lower forearm was lacerated and required sutures. The sutures made this incident reportable
While changing out a hoist cylinder on a Haul Truck (Co#428) a come-a-long slipped. This action caused the cylinder to pinch the employee's right hand against the left rear inside tire. the employee was cut between the thumb and index finger. Medical Care cleaned and stitched the area.
Employee was reaching into a tool bag and lacerated his right palm on a sharp object. After first aid, he examined the tool bag to find the sharp edge of the rubber punch was left unprotected, and placed where it would injure an employee.

While helping another employee take a broken part of a drill steel off a good steel, the broken part fell through the opening in the deck. Injured went under the deck and gave the broken part a push. As he pulled back his right arm he cut the wrist on the top of the broken steel. He was taken to the Emergency Room where sutures were applied. Reported 11/9/04
Employee was working on a switch point putting nuts and bolts, then pulled the switch handle. Employee caught his left little finger between handle and pavement. Resulting in x-rays and sutures to the laceration on the tip.
