Struck against a moving object

Employee was in an articulating haul truck, was traveling down the haul road at the Quarry. The victim was moving the haul truck down a steep grade, around a turn, when the haul truck traveled through a berm and off a short drop-off. The victim was found hours later by the owner of the company, the victim was not wearing a seat belt.

Employee operating front end loader tried to turn around on a sloped road without controlling speed need to make the turn safely, resulting in the front end loader tipping on its side.

Employee was working splitter machine when they turned and hit an elbow on a rock and cut it open.

Employee wiped their eye with gloves that had been contaminated by metal shavings the day prior, which required medical attention.

An access plate on a MEC splitter became stuck out of normal position. As the employee attempted to free the plate, the plate came free smashing the employee's index finger on the left hand. The injury resulted in 14 stiches with limitations and a follow up scheduled ASAP.

Mechanic was using a sledge hammer to get a pin out of a thumb cylinder and felt a pain in stomach.

While working on the back of a stone splitter, employee put a stone back on to the table to be re-split. He had his hand in the wrong place and the middle finger was smashed between two rocks on the table.

Miner was pulling a hydraulic hose off an excavator. A zip tie had to be cut to get the hose off. While bracing with his right arm the miner attempted to cut the tie with a pocket knife. The knife slipped in the upper cut, stabbing his right forearm.

EE was moving equipment down a snowy/ice packed access road to the quarry. He dropped his glove in the cab. He unbuckled his seatbelt, took his eyes off the road to retrieve the glove, and drove off the embankment. Head hit the windshield.

While splitting a rock on the rock splitter table, the rock fragmented sending a piece into the left forearm of the worker. The arm was punctured by the fragment.
