An employee was operating a CAT 982M loader to clean out scaling material from a heading underground. On the last load out the bucket made contact with a blasting cap that had not detonated when the heading was shot. There was no damage to the loader and no injury.

Employee running loader underground and claimed they were jerked side to side due to the abrupt transitions in the roads. They noticed possible hernia on 11/26/25 and reported to management on 11/29/25. Employee was seen by a physician on 12/1/25 and placed on light duty.
Employee was unloading a haul truck at a stockpile. The truck bed raised about 4 feet then stopped and would not go any further. When the driver switched the dump switch to neutral to lower the box, the box dropped with no hydraulic cushion. When the box made contact with the frame it shook the truck. After the incident the employee complained of soreness in thei back and neck.
Employee was cutting conveyor tail rubber with a utility knife. During one cut they slipped and cut their left thumb requiring repair to the tendon and stitches.

Employee was picking up a 10 quart jug of DEF fluid and felt a pop then numbness in right shoulder. Employee was seen by a physician and placed on restricted duty.
Employee using a hammer and punch had a small piece of metal come off the punch and into employee's left inner arm. EE was treated as a first aid at the local clinic. On 8/28/24 EE noticed swelling and discomfort and returned to the doctor. EE was prescribed an antibiotic at this time.

While carrying a ladder the wind blew a door open catching the employee foot causing them to fall. employee fell on to a concrete floor striking their left kneecap. Employee did not seek medical until 11/27/23 where they were prescribed an antibiotic.
Employee was hitting a chisel with a hammer. EE missed the chisel striking EE's left thumb causing lacerations that needed stitches.
Employee noticed rock in the middle of the road, so the employee stopped to move it off of the road. EE didn't realize how sharp the rock was on the back and while moving it they cut open their right pinky finger. The cut required 3 stitches. Employee was not wearing gloves at the time of the incident.
Employee exiting the cab of an excavator/scaler caught foot in the groove of the track and rolled left ankle. Employee went to the doctor on 4/14/23 where x-ray shows a broken ankle.

Employee using a shovel removing spillage on catwalks. Area was provided with handrails to prevent fall hazards. Somehow the employee took a shortcut and positioned self beyond the protection of the handrails, lost control and fell to the ground. Employee was transported to a hospital for evaluation. All xrays and CT scans were negative. Employee was prescribed a muscle relaxer.
While working on a feeder pan the employee slipped causing a welding rod in the welding rod bag to cut EE's side requiring stitches.
While picking up dropped safety glasses employee stood up and knee locked up. Employee stated it was from a previous injury. On April 22nd, 2022 we were notified that the act of standing up further damaged the previous knee injury requiring it to be handled by workmans's comp. Employee did have restricted duty days and will require future surgery.
CAT 770G(4053) northbound on 7W. CAT 770G(4048) southbound on 8W turned east at 70S into the path of the northbound haul truck. Left front of 4053 contacted right front of 4038. Driver of 4048 was not wearing a seatbelt and contacted the ROPS on the right side of the cab and then the window on the left side of the cab. Driver of 4048 was life flighted to a hospital w/head injury

While exiting the CAT 140M grader employee was stepping down to the ground when boot hung up in the step. Employee lost balance and began to fall backwards. Employee grabbed the hand hold with right hand and heard a pop in shoulder. Employee was put on restricted duty for shoulder strain to prevent further damage.

While operating a haul truck on a haul road operator drove through two water holes (appx 3"" deep) and had the seat bottom out due to the air ride not being adjusted properly. Employee suffered radiculopathy in lower back and was put on restricted duty.
While unloading 8D batteries weighing approximately 130 pounds employee felt a pop in right shoulder.
After working underneath a mine bolter employee crawled out from underneath and stood up. In the process of standing EE felt a pop in EE's right knee. Currently being treated as a strain with work restrictions.
The employee approached management on 11/6/2019 and informed us that EE was diagnosed with carpal tunnel syndrome in each hand. EE's physician thought it might be work related. We referred the employee to our insurance carrier for examination. It was determined that is was work related and surgery on the right hand was completed on 3/2/2020.
Employee closed the loader door on EE's left hand middle finger. The closing door pinched/tore off the skin in the affected area. Employee was treated at the hospital and received two stitches to close the wound.

Caterpillar 345 scaler was operating in 21N 43W North mine. Fire started behind cab. Operator was unable to control fire and abandoned machine. Notified other miners and supervisor and evacuated mine.
Employee was on landing cleaning windows of Kawasaki 115Z7 loader. Somehow fell through step opening on railing 7'8"" to the ground. No witnesses. Contusion on right knee. Sprain left hip.

EE started down a flight of stairs in office, slipped and fell 5 or 6 steps to landing. Transported to hospital. Hospital found nothing wrong and released ee to return to work, no restrictions. EE continued to have pain in shoulder. MRI 12/22/2018. On 12/26/2018 doctor informed ee of a rotator cuff tear. EE still is released to work as of 1/7/2019.

Employee was closing door on powder truck. Pinched middle finger on right hand between the door and door frame. Broke bone in end of finger.
Employee was climbing to repair belt. Thumb was caught between 2 rollers resulting in laceration requiring stitches.
Unknown injury. Employee had carpal tunnel surgery on left hand.
Moved torch tanks into new holder and experience pain in left shoulder. Holder for torch tanks was located in the side of a flat bed truck approximately 3 feet off the ground.

Employee was turning a water valve. Another employee observed him turning the valve multiple times. Second employee went to help. First employee was awake but not responsive to verbal questions. Medical help was call and employee was transported to hospital. The only information from the hospital is a possible stroke.
Mechanic picked up a piece of 3/4"" plate steel and felt groin pain. Visit to doctor required no treatment and resulted in no restrictions. After doctors reevaluated over the course of several weeks, it was determined that a hernia existed and would be repaired April 2, 2015. No work restrictions existed before the surgery date.

Mechanic was starting a portable gas generator and recoil did not release. Said he felt a ""pop"" in shoulder. Has strained shoulder.
Unknown cause of injury. Employee is a mechanic who has had observations of shoulder but has had no medical treatment and has had no restrictions. However, the doctors want to do some repair of the shoulder June 16, 2014.
Haul trucks were entering heading on 47E and exiting on 50E. Trucks were traveling on 70S to get to 47E and an empty truck did not turn on 47E and continued on 70S and was struck by a loaded truck leaving the face at the intersection of 70S and 50E.
Operator strained his shoulder. Believes he did it on the ladder of a 988F Caterpillar loader. Shoulder Rotator Cuff injury. Awaiting MRI to assess damage.
Traffic was entering 1E and exiting 1S. 1W was closed 1N was open. Highwall material sloughed onto canopy causing a rod attaching the canopy to fall blocking part of the portal making it unsafe. The mine was evacuated. Miners were instructed to enter 1N and exit 1S. 1E was closed. Mining resumed.

While removing drill steel from magnet, finger was pinched between magnet and drill steel.
Haul truck struck pillar on exit route to portal. First aid only given to driver. Damage to right front and side of haul truck. Amount unknown at this time.
Truck driver was hauling a load from the face to the dump box and reported that he fell asleep and struck a rib.

Released come-a-long and pinched hand against roller
The employee was descending the ladder of his haul truck when he felt a sharp pain in his left shoulder. He immediately reported the injury and was examined by a doctor. It was diagnosed as a slight strain and the employee was put on light duty until further examination.

Weathering and scaling of the pillars made area on top of pillars to small to handle roof load. An area of about 9 rooms fell out that was approx. 6' thick.
EE was loading explosives at the face on 16 W of the North Mine. A rock fell from the face and landed on EE's right small toe on 9/11/10. EE did not go to the doctor until 9/24/10. Diagnois was fractured small right toe.
Employee was shoveling waste material when he felt a sharp pain in his back.
EE alledges he slipped two discs and got a bone spur in his back when he picked up a chair at lunch time and moved it to a different location. EE did not tell anyone about the injury until after he got an MRI and returned from vacation on 2/8/2010. EE did not know exact date of injury.

EE was loading explosives at the face. EE tripped and fell on his left hip. He told supervisor on 9/11/09. On 9/11/09 EE went to doctors and found out it was a bruised bone in his hip.
Wrench slipped, right little finger caught bracket and right little finger bent to the side causing a carpal bone fracture of the right hand near the little finger. injury was working on belt tensioner of 771D Quarry truck.
Worker was prying on belt with pry bar. Felt pain in right shoulder. Did not tell anyone until 7/7/09. Did not go to dr until 7/14/09. Strained right shoulder.

Explosive misfire: drill operator, while drilling Co. Rd. mine, heading 14E & 75S, machine drill steel struck an unexploded explosive, consisting of a cap, stick of dynamite and small amount of anfo. No damage to machine or operator.
Was getting out of co-workers pick up at lunch time & wind must have blown something in his right eye. Particle got imbedded in eye and had to be removed by a dr.
There was a rock stuck in a conveyor belt, EE went to hit it with a sledge hammer, missed, his his middle finger on left hand on the rock & cut it open. 7 stitches.
EE was going to get in his drill and slipped on the step and pulled his shoulder, his boots were muddy. Streched tendon. RTW on 5/1/07, but has a few minor restrictions.
Annual hearing test - Test indicates a standard threshold shift. Our information indicates this is not work related. Filing under protest.

Catwalk on the haul truck was a little moist from exiting the mine. When EE was getting down from the catwalk, his foot slipped and he caught himself before he fell, resulting in an injury to his shoulder.

Employee attempted to drag a cardboard box out of the back of a pickup truck. The cardboard ripped and he fell off the back of the truck, hitting his head.
EE was lifting a steel plate in the plant and felt a pull in his groin. At the time he thought it was just a pulled muscle and did not need any medical attention. On Dec 1, 04 ee went to the Dr and found out he had a hernia. The next day he filled out a company incident report. No days were lost and no restricted duty until he had surgery on 12-27 04.
