Walking on side of hill to move power cable miner slipped on rocks and felt (pop) in right foot causing pain and difficulty walking. Right foot pain/sprain.

Miner was laying on ground cutting exhaust pipe on 2009 ford van equipment #6119 A piece of the cuttings from a cordless 18v. Makita reciprocating saw got by his safety glasses and entered left eye

While doing field service on hoist a defect was found on the wire rope and had to be retired from service. A new wire rope has been ordered, the miners have been notified and the refuge chamber is operational.

Tying in primers to shoot round, small rock dislodged, struck rt forearm. Laceration - 5 stitches

STEPPED UP ONTO FOOT BRIDGE & FELT RT. KNEE ""POP"". INITIAL MEDICAL EVALUATION NEGATIVE. SUBSEQUENT DR. VISITS RESULTED IN ARTHROSCOPIC SURGERY TO RT. KNEE W/LOST TIME & RESTRICTIONS UPON RETUR N. SURGERY 8-1-01. LIGHT DUTY 8-30-01.
