Contact with hot objects or substances

Employee drove out into the field to fix a machine. When they got to the machine, the service truck they drove was smoking. Employee went to open the hood to examine the engine to determine the cause of the smoking, but when trying to unlock the hood, the antifreeze sprayed their left hand, causing a second-degree burn on index and middle fingers.

EE went to the warehouse to get new rags and when EE got there EE opened the new pallet, which was wrapped tightly, leaving the straps laying on the floor. After EE picked up the amount of rags EE wanted, EE began to walk away but didn't realize EE's foot was caught on the strap. This caused EE to trip and fall onto EE's elbow.

When using small jackhammer in a transfer chute at mill, bit got stuck in some frozen materials. While trying to pull the hammer out, it became dislodged and hit him in the nose. Received nasal fracture and five stitches.

Employee was using a power mag drill to drill holes laid out on a piece of plate. He shut the drill off but put his hand near the still spinning drill bit. His glove got caught in the moving drill bit and he injured his right index finger. His finger was cut and the bone at the tip was broken.

As a result of a hearing test on 8-28-14, it was discovered that the employee's left ear has a moderate hearing loss at some high frequencies.

Employee was replacing cutting edge on a 988 loader. While removing a plow bolt he used his hand to keep the bolt from turning. When bolt loosened it fell out of lock groove and spun, which cut his left ring finger.

Employee was gluing PVC pipe, he finished one joint and was moving to another. Picked up tools, glue & primer can. Glue can was tipped, leaking on his coveralls & left leg. He realized this & proceeded to wipe the glue off his leg with a lit cigarette in his hand causing the glue to catch on fire. Burns to left leg, knee & hand.

Fall from ladders

While removing bolts from the counterweight of a rented Cat365C backhoe, the 20,000 lb. counterweight became dislodged from its position & fell to the ground. Employee was on a six foot ladder when the wt fell sending him vaulting through the air to the ground, about 10 feet away from the weight.

Employee was cleaning up stuff around yard, he was moving some wooden blocks. When he picked up the wooden blocks he felt a pain in left region of lower back.

Employee was welding @ mill when a welding spark went behind his welding shield hitting his left eye. Piece of weld in eye.

Employee was helping welder in screen #105 rollaway when he got flashed in both eyes. He had handed welder 2 welding rods and the reflection flashed him in both eyes.

Employee was doing winter maintenance. He was climbing over I beam and when he got home that night, he noticed lump by groin.

On Monday 9/24/07 employee and another employee were lifting a truck tire onto back of pick up truck. The next day employee was walking up the hill to pouch in and his back tightened and became painful.

Employee was standing on the ground while tightening a bolt on the loader bucket. The wrench slipped off the bolt sending employee to the ground. Employee broke his fall with his left wrist.

EE WAS CLEARING AREA WITH BOBCAT LOADER, THE BUCKET STRUCK AN OBJECT BELOW THE SURFACE AND CAME TO AN ABRUPT HALT. THE EE STRUCK THE SAFETY BAR WITH HIS CHEST.
