Employee had to stretch to get a plastic cover over a super-sack; in doing so, the employee slipped and fell. Upon impact, the employee's left thumb was jammed into the side of the pallet. The employee was able to stand up but was unable to move EE's thumb without pain.

On 06/23/2025, a 54-year-old employee with 7 years of mining experience was working in the mill near screen 2 packing off bags of finished material When picking up a bag to place it on the pallet, EE felt a strain in EE's back and told EE's on-shift foreman, stating EE's back was bothering EE. The employee later informed the foreman EE was going to the clinic.
While feeding material into a material bin with a feed screw. the employee used a shovel handle to push the material into the feeding screw. The feed screw grabbed the handle and the handle hit the employees face and the employee received a cut that required first aid. There were no stitches; however, a 7 day cycle of antibiotics was provided.
Employee was packaging 50lb bags only 2-3 bags, happened at beginning of shift. Employee stacking height was 4.5 feet. Employee stated EE was swinging bag into place on the pallet.

At roughly 4:30pm on July 22nd, 2024, an employee with a little over 6 years of mining experience tripped on the 2nd step as the EE was ascending the stairs in Shed 8. EE fell on the stairs injuring knee, shortly after which the employee left work and went to the doctors where EE was diagnosed with a contusion on knee.
Operator was feeding raw material into a feed bin when a small granular piece of graphite flung out of the feed bin striking the operator in the eye. The operator was wearing prescription glasses at the time of the accident. Operator requested a hospital visit due to pain in the left eye.

After annual audiogram testing,1 employee had a reportable hearing loss in their left ear.I reviewed the Hearing Conservation Program,retrained,refitted and offered further evaluation-the employee noted they were having issues with their left ear after a recent day of swimming.Per guidelines,the HCP review was still completed,employee denied further care and evaluation offerings.
Employee was operating a stretch wrap machine when the material they were wrapping became unbalanced toppling over and pinning the employee against a nearby railing. Employee requested medical attention, ER doctors report minor swelling on employees back.
Employee was changing socks on a dust collector, during the process excess graphite built up on employees hard hat. While exiting the job, dust broke free from hardhat and entered employees left eye. Safety glasses were worn at the time of accident.
A maintenance employee was working on Roller Mill parts in the maintenance shop. The employee flipped over a cross member and caught their thumb between the nut on the cross member and the end of the drive shaft sitting nearby. This crushed the employees thumb and caused a severe laceration. The employee was taken to the hospital for treatment and received 9 stitches.

Employee placed a pallet of material on stretch wrapping machine off centered. As the pallet rotated around the wrapping machine it grazed the top of the employees foot due to overhanging the wrapping platform. This caused a contusion on the top of right foot, employee was placed on restricted duty due to injury.
Employee was mounting and dismounting a forklift to perform normal tasks throughout the shift. The repetitive motion of getting on and off the forklift caused an assumed sprain in the employees right foot.
Employee was driving a forklift with their forks lowered to ground level when they struck a raised section of flooring. The forklift came to halt which cause whiplash type injuries. Employee was wearing a seat belt at the time of the accident.

Employee was pushing a 55 gallon drum of graphite off the fork of a forklift to discard the drum into a hopper. While pushing the drum, it tipped backwards which pinched EE's finger between the drum and fork causing a laceration on EE's left middle finger. Employee received 9 stitches and is expecting to be on light duty for 12-14 days.
Employee was walking from the lab back to production where they slipped on a rock that was on the pavement. This caused them to slip and fall, employee rolled/sprained an ankle.
Employee was stretching a bungee cord while securing a super sack to a discharge chute when they felt a pain in their back. ( reoccurring back injury )
Employee was descending a staircase when employee slipped near the last step and fell backwards. Employee injured / jarred their lower back. Unspecified injury of muscle, fascia and tendon of lower back.

Employee slipped and fell near the packing station. Employee broke a rib. The accident was not reported and we were not made aware of this until we received a doctors note on 10/12/2020 pertaining to the employees absence. Accident is still under investigation.
Employee was loading stock into a screening system with a shovel when they felt pain in their back.
Employee was riding along on a stretch wrapping machine turntable when EE lost footing and wedged foot between a wall and stretch wrapper. This resulted in a fracture in the right foot.

Employee had results of reportable hearing loss in both ears found during our annual physical. Employee will be offered to be sent for a second opinion and be retrained on our hearing conservation plan. EE will be offered alternative hearing protection with mandatory requirements to wear that protector at all times while on site.
EE struck elbow on a forklift that was parked behind EE when EE lost grip opening the discharge spout on a super sack while filling a machine.
Employee slipped and fell while walking. Injury to left side.

Employee working in screen 2 area was rearranging empty pallets. EE felt pain in right bicep / elbow. Most likely the injury was a result to shifting the empty pallets. This didn't turn into a lost time accident until 12/26/18.
Employee working in the packing house stated after continuously palletizing 50lb bags, EE felt pain in the middle of the back. The pain didn't go away and continued for a few days and the pain transferred into EE's right hand which became numb and tingly. This accident was not reported to me until 09/26/2018.
Employee had a result of reportable hearing loss in right ear. This employee was sent for a second opinion which resulted in " moderate hearing loss" in right ear. Employee was refitted and retrained on the use and care of hearing protection and provided hearing protection offering greater protection and was retrained on our hearing conservation program.
Employee was using a sledge hammer to put 3-4" anchoring bolts into a concrete floor. While swinging the hammer the employee injured their left shoulder. This resulted In a left shoulder sprain and the employee is estimated to be out of work for 2 weeks per the doctors request.
Employee lifted a 75lb 55 gallon fiber drum of graphite onto a 4" high scale instead of tipping and rolling the drum onto the scale. As EE lifted the drum 4" to place on the scale EE pulled the back out. EE became immobile and an ambulance was called. EE was released for light duty the same day. EE was released for full duty without restrictions on 5/30/18 & reported back to work.

Employee was filling a blender by emptying 200lb fiber drums full of feed stock. This resulted in a back sprain. EE didn't report the accident to me or anyone until 11/10/2016. EE reported it to me because ee came to work on 11/10/2016 and while getting dressed before shift ee became immobile due to this injury and we made ee seek medical attention.
Outside yard forklift operator was dismounting forklift and stepped on a 1.5" rubber ball (debris)(used for tumbler screening) which caused EE to slip and fall injuring shoulder and back. One doctor released EE back to work full duty for the shoulder injury. A different doctor is keeping EE out of work for the back injury.

Production worker was cutting a bulk back and cut right index finger. EE left work early to seek a doctors opinion, they glued finger closed and bandaged it. EE returned to work for the next shift.
Worker slipped on a stairwell due to wet and graphite slicked boots from rain and surface dust on staircase. Worker had bruising, pain and mobility issues at the time of accident and requested to see a doctor for evaluation. The doctor put the worker on light duty and recommended rest so the worker decided to take a few days vacation and was released for full duty on 3/6/2016.

While getting off a forklift he slipped on the graphite and pulled his back.
A maintenance man was removing a belt from a motor when he lost his grip of the belt and fell on the gear box hurting his left shoulder.

While putting a pipe on, the pipe slipped and pinched left ring finger between the pipe and the floor. Top of the left ring finger was cut.
The employee was sorting pallets, which entails lifting pallets. While lifting he started feeling abdominal pain.
While putting material in blender using a shovel his left shoulder began to hurt. He did not report the incident until the following day. This has been a reccuring injury and might have been a preexisting condition. He had surgery on his shoulder on 09/28/2006.
Employee's right index finger was caught in the locker room door while closing it. It has a deep nasty cut and requires stitches.
